Most people are not on Medicare, but a lot are. And most private insurance guidelines are very similar to Medicare. So, I've provided the Medicare guidelines here. MRI (Magnetic Resonance Imaging) of the back for back pain typically require that specific criteria are met to justify the medical necessity of the procedure. These criteria help ensure that the MRI is ordered when there is a reasonable expectation that it will provide valuable diagnostic information. While the specific guidelines may vary slightly based on the region and the individual insurance plan, common indications for an MRI of the back for back pain under Medicare guidelines include:
Failure of Conservative Treatment: The patient has undergone conservative treatments such as physical therapy, medications, and rest, and there has been no significant improvement in the back pain symptoms.
Neurological Symptoms: Presence of neurological symptoms such as radiating pain, numbness, tingling, or muscle weakness in the legs, suggesting potential nerve compression or other serious conditions.
Trauma or Injury: The back pain is a result of a recent trauma, injury, or accident, raising concerns about spinal cord or vertebral injury.
Suspicion of Structural Abnormalities: Suspected structural abnormalities such as herniated disc, spinal stenosis, tumors, infections, or inflammatory disorders that require further evaluation.
Monitoring Known Conditions: For patients with known spinal conditions or a history of spinal surgery, MRI might be necessary to monitor the progression of the disease or assess the post-surgical status.
Unexplained Severe Pain: Severe and persistent back pain that is not explained by other diagnostic tests or clinical evaluations, warranting further investigation through imaging studies.
It's important to note that Medicare guidelines are specific and may require documentation from the healthcare provider to support the medical necessity of the MRI. The physician ordering the test must provide detailed clinical information, explaining why the MRI is necessary based on the patient's symptoms and medical history.
Patients and healthcare providers should always check with Medicare or the specific insurance plan to understand the exact requirements and ensure that the procedure will be covered. Meeting the necessary criteria and providing comprehensive documentation are key factors in getting approval for an MRI of the back for back pain under Medicare guidelines.

Excellent video, this young lady who is in the service should be given assist by the Government. Love your videos which are helping me as I am going to have L4/5 S1 fusion in 10 days just very worried had to have stress test advised ok to proceed with op finger crossed. My is grin, pain butt , thighs both dropped gate right leg.

Love watching your videos. I am 80yo and have DDD and stenosis. I has progressed for the last 20years and the last 3 have been very bad. Got X-ray's and MRI in 2019 and they were quit concerned but gave no suggestions as to what to do other than laminectomy. Trying to avoid surgery.

Dr Dan , Please I need your help with diagnosing something as the doctors I have went to couldn’t diagnosis my problem. I am a 49 year old female , I have chronic back pain in my right lower back which seems to radiate towards right hip joint and further towards medial part of the thigh , The pain feels like something is stretching internally , and a pinching type of pain , When I get up from the bed i start feeling the pain and then after exercising for a bit it goes away in an hour and severity of the pain decreases when i am moving and pain is more when I’m standing. I have this pain since 7 years. I had a MRI in 2018 and now recently last week , both the MRIs showed mild disc changes on the LEFT side but all the pain that I have is on the right side. Also this pain went away for some months when I started using Herbalife products (including protein powder and a lot of products like calcium ,multivitamin etc.) But then I stopped using Herbalife products as I got to know they were harmful. And after I stopped using Herbalife products the pain came back. I’m really confused as to what has happened. I’d really appreciate it if you could help me with this

I’m 40 year old male with a large hernded disk. Im in good shape not over weight and would love to to a video with you about my back and sciatic pain. I have tried to find younger people with this issue and a lot of times it’s an older person and hard to relate. I have a up to date mri and trying to figure a corse of action. I own my own lawn care company and have time off right now and debating on whether to get surgery or wait to improve. Thank you George.

Hey Doctor Dan, I have some questions.......I just had an ALIF to take care of my Spondy and foraminal disc bulge that was giving me L5-S1 sciatica symptoms. I've never had issues in my life as an athlete but I injured myself 8 months ago. My concern was that they do the discectomy for the ALIF from the front and remove roughly 90 percent of the disc before the end plates are prepared and the cage is placed. Is it possible to still leave in the part of the disc that's compressing the nerve since it's foraminal and posterolateral and kind of buried in the back. My surgeon said that's not possible because the cage gives you new disc height and the remaining part of the disc just hangs loose before being dried up and absorbed by the body. I guess my fear is that my sciatica will remain once I'm healed. I'm 2 weeks post op and I feel decent, some minor tingling in my leg is still present here and there. Is it true that it could take time for the nerve to heal because it was heavily compressed for 8 months? I haven't taken anything for pain and I'm still okay. My hips and upper glutes are definitely really tight and in pain after laying down for awhile. I didn't have back pain before the surgery. What do you think?

Is a postoperative MRI after microdiscectomy something that is recommended? Just from a medical standpoint, not from an insurance standpoint?

MRI is not typically needed after microdiscectomy unless there are recurrent or persistent symptoms. If MRI is done after surgery for any reason, then contrast needs to be given to distinguish between residual disc herniation and scar tissue.

My back doctor doesn't respond to my back pain issues I tell them about. No x-rays, no cts., no mris. no injections. I also have a sharp pain in my lower back& leg pain.
