This is the 1st of 14 sections posted on RU-vid. Together they form a complete three hour lecture series on shock. This lecture is designed for students of nursing, medicine and related disciplines. The aim is to understand the physiology and pathophysiology, so we can apply these principles to patient care.
Shock
A state with significant reduction in systemic tissue perfusion, resulting in decreased delivery of oxygen and reduced removal of waste products, leading to tissue injury.
Blood pressure = cardiac output x peripheral resistance
Cardiac output is dependent on venous return
Cardiac output = heart rate x stroke volume

Pressure is not a driving force, it is not even a force. It is a result of the heart pumping against a resistance - it is not a primary factor but rather a secondary result. Blood pressure can be increased by vasoconstriction but this will result in increased resistance and decreased blood flow. It is the flow that is important not the pressure. The lungs carry the exact same blood flow (5 L/min in adult) as the systemic circulation but the pressure is only one-fifth that of the systemic. Blood flow is important, blood pressure is what can be measured but it is not the driver of flow.

Why in case of Shock sympathic system is not working so that there will be Vasoconstriction increase in peripheral resistance thus bp increase?

I'm suffering. What should i do? I got fired from my job and it triggered this entire event. I haven't slept, i've been vomiting, my pulse feels very weak, im clamy, i feel cold. Im shaking, crying, and things are not making any sense. please help.

+tj1990 Dear tj, Im so sorry yo have been suffering in this way, you describe feelings which are truly horrible. The features you describe are all consistent with human distress. I pray that the Lord our God will bless you and keep you. In addition, I think we need to realise that depression is a genuine problem with the human brain, and is is worth considering medication, if your doctor thinks you need this. I wish you well, John

Dr. John Campbell Thanks for the reply. Its been a few months. To follow up, I checked myself into a hospital the next day. I got a chance to recover and get some help. I got treatment, still ongoing. I since got another job and it really wasn't the end of the world. I should have thought about how employable i was, and how many skills I had. I really wasn't without a job very long at all. I just had a tough upbringing and grew up in poverty. When i finally felt like i had somewhat escaped it, it seemed like it was just going to swallow me back up and i was going to be back to living on the streets and doing anything to survive. Luckily, and probably through the grace of God, everything turned out fine. Again, thanks for the reply. And you have a great youtube channel.

+tj1990 Thanks for letting me know tj, you are right, sometimes it is hard to examine ourselves objectively, with sober judgement. Clearly, you are very employable, but were unable to appreciate this for a short time. I delighted you are now working again.
