Absolutely true. If we do not have trusting teams and the organisation lacks the ability to self-improve, it breaks! That is inevitable.

Reminds me of when I worked at Hollister Co. in high school and college. They’re notorious for being dark with loud music and when customers would ask us if we could turn the music down, we had to say we couldn’t. We didn’t even have a volume switch in the store. Volume was only controlled by home office.

There's also a key difference in the business model of Four Seasons and United Airlines, in the sense that Four Seasons is a luxury service while United Airlines is more of a "general public" service. The difference is that, even thought financial power is not totally correlated with good social behavior/manners and high civility, it is correlated enough so that general public enterprises will have to deal with more uncivilized and less intellgent people, mostly because, in general, uncivilized and dumb people have a harder time getting rich or being accepted by people of higher intellect and more refined lifestyles. Rude and oboxious rich people are generally rude and obnoxious when they're antagonized or disrespected by others, and that tends not to happen in most places/businesses that cater to the rich. Just emphasizing that I'm in no way saying that "rich people" are better and "more intelligent" than people who are more of an average level in terms of capital, or even poor people, but the simple fact that they're rich and tend to be content frequenting places that are more refined and have more strict social rules, and are less likely to "lose their minds" and risk being outcasted of that lifestyle, except when they're music celebrities who became rich by being anti-social drug using thugs, but that is a small minority of rich people.
