sjogrens syndrome is an autoimmune disorder which can affect many organs of the body especially moisture producing glands especially salivary glands and lacrimal glands, also it involves joints and patient has difficulty in walking , running and carrying out different daily life activities.
This patient came to my OPD with severe dry eyes, her eyes were red, but she has used systemic strong steroids (along with topical steroids) for prolonged time, due to which she has developed steroid induced glaucoma.
There was no vision in right eye while she had counting finger vision in left eye, she presented with foreign body sensations, redness and pain, on slitlamp examination I found that she has severe dry eyes but she her cornea was thin and previously she already had corneal perforation, which was self-sealed due to iris and pseudo-membrane was formed.
I advised topical lubricants, antibiotics, pressure lowering medicines and cyclopen, advised to avoid straining and follow up after one week.
After one week of follow up visit, there was no hypopyon in her eye, redness was relieved and there was no pain.
Patient was very happy, but she came after few months again with decreased vision and pain along redness.
On examination, I found that she has developed new self-sealed corneal perforation, as few days back she fell on her face, so again I advised same medicines and advised amniotic membrane graft.
So Sjogren’s syndrome is a continues fight and patient has to deal with it, patient must keep artificial tears all the time in pocket or purse and must keep take water frequently in order to keep both eyes and mouth moist.
