In part three of this five part series, physicians from New England Baptist Hospital and their internationally renowned Spine Center review the various treatment options for spinal stenosis.
GW. The best video I´ve seen on SS. I´m an 86 yr. old living in Central America on a miserable UK OA pension. Diagnosed with SS at L4, L5 about 4 years ago. Laminectomy shortly after proved ineffective. 3 epidural blocks since, only moderately useful. Swim 40 minutes or so every 2/3 days which helps a bit. Will try to increase excercise. Perhaps a walking stick would help.
Wow.. When I was diagnosed with Spinal Stenosis, one of the first things was Physical Therapy. I told the therapist, these exercises were causing massive pain, she told me to still do them figuring she knew what she was talking about I did them even though the pain was incredible. After two days of physical therapy, I had to be hospitalized. The "PT" had caused massive inflation thus increasing the pain. So DO NOT believe that physical therapy is ALWAYS o.k., it is not.
I agree with you 200%! I injured myself sliding on the ice on March 4th 2020, thinking i would get better exercising but i only gotten worse even with the recommendation of my physiotherapist... Now i need a spine surgery asap... My right leg is useless and excruciating pain from toes to hip and lower back.
@@sylvialoya632 Objectively studies have shown that PT is a poor treatment option for Lumber Spinal Stenosis, same for ESI unfortunately. However I'm glad to hear you have seen improvement and hope you are still going strong!
Indeed, I have been through the same! PT can sometimes make your situation much worse! The reasoning and conclusion here might be that if the root nerves are already inflamed you are merely adding more irritation to the nerve! PT will work better when you are maybe in a state of remission from the most intense pain!

I can stand up. move for 10 min, then flat on the floor for 20-30 min, I can reach that pain level of 8 very quickly. That's when I'm down for longer. Sitting is only good for an hour and all of this is accumulative to produce pain and substantial fatigue. It takes a few days to recover from accumulative. I now plan carefully and listen to my body to keep moving. I refuse surgery and opioids. As long as I can still get up and walk it is better than grinding and fusing bone that is already degenerating. I meet that subtle threshhold point and back off. It's like a game and I plan to win.
When the MRI came back with a dx of spinal stenosis, got an automatic reference to a neurosurgeon who of course said I needed it , did not even discuss the MRI, and when i had the temerity to say i would think about it, said under his breath as i was leaving ""you will get weaker". That guaranteed that if I ever had surgery , it would not be with him. I was just a fat fee for him for an hour of work. Not all MD"S are what one has a right to expect, and I am so thankful to NEBH for giving me the information that no one has up to now, and doing it in such a low key, tranquilizing manner. These people know what they are doing.
I lost my job due to Lumbar Spinal Stenosis and denied disability 3 times!!! I have had physical therapy and currently on Oxy but I hate taking it!! I only take it when the pain is excruciating!! I am in pain everyday!! Walking, driving and sitting long periods are painful. I have to limit activities. Water aerobics felt good in that water. I hate that I can't walk without pain. I currently can't work due to Lumbar Spinal Stenosis!!
A recent TV article on Australian networks has highlighted the unwanted and very negative results in patients taking drugs in the Lyrica type and Gabapentin. I was put on a trial regime on these drugs as an adjunct to my opiate meds. The side effects of the above drug types were worse than any benefits they might have. I have a medical background, so I was able to assess the undesirable side effects. 2019. I have ongoing spinal and foraminal stenosis at multiple lumbar levels, including an additional sixth lumber vertebra which is sacralising and contributing to the enervation issues. Scoliosis and some vertebral rotation at 3 & 4 lumbar all adds to the complexity of any surgical options, especially at age 82 with early osteoporosis thrown in!. Both my neurosurgeon and myself are very reluctant to even attempt a simple decompression at the worst levels.

I agree with you....I have exhausted all non-surgical options and still have a lot of pain when walking any distance. Opioids take the edge off the pain; I don't take a high dose, and they don't last very long, but they allow me to walk a greater distance and do light yard work for a short time. They are not a panacea, but they do have their place when nothing else helps.
Yes we humans think everything can be fixed...but are today's best robotics better than evolution has progressed to? It's clearly very difficult tricky surgery with uncertain guaranteed positve outcomes. Much damage is done in getting to the problem. I've had 42 years of back (disc) problems...every time i've been begging for an operation of some sort the advice has always been to go away and strengthen up which has worked everytime upto now. SS has been diagnosed - numbness in both feet when standing. Can't walk for more than 200 meters without a sit down. Upto 5 months ago i'd walk for 1:20 mins ( 6 miles) every day. Last phone ( 3 years) got 9 million steps on it . In UK today the wait is around 2 years for an operation! Oh well, enough about my problems... i enjoy lots of things so gotta not be too sad.
Thanks for this video. I had X-ray and was diagnosed without spinal stenosis and was giving PT two sessions a week for six weeks but my sciatica and piriformis symptoms still linger after 5 months. I have to do continue to go the gym . stationary bike . Strengthening ham and quads and stretches are doable and challenging to loosen still hurts and tight ...I am going to the Dr again hope to get an MRI, but the DR does not think I need one . How long does these symptoms last? Thank you. A great video production. Have me a lot to think about it . Surgery or not?

doctor i have a L4 L5 S1 i just got my new i had 3 MRI but this one says L5 S1 broad posterior disc hernation which is slightly more prominent than on the prior study deforms the ventral epidural space and there is left greater than the right foraminal extension with abutment of exiting left L5 root and narrowing of the foramina diminished disc space height ventral extension of the disc remains and discogenic chages and scmorl;s node in the end platates.mild facet hypertrophic changes remain L4L5 S1 ,,,,,doc my friend told me narrowing part of stenosis ,doctor can you help me out with this ,,thank i been watching ur video;s there very good ,,thank you,,ps my doc can be fixed with endoscopic laser surgery,doc what do you think,
Scott Drown I happen to have both. I have spondylolithesis in my lower back and I had to have 2 fusions. I also have cervical stenosis and some lumbar stenosis. I have tried PT and I came home in so much pain I had to stay in bed the rest of the day. I attempted this 3 times for one week each time. I also tried hydrotherapy. Same deal. I also try to walk but to no avail. I do take Lyrica, morphine and hydrocodone. I don’t want to take these medications but if I don’t I have no quality of life.
