The reason why Mermaid Man and Barnacle Boy are able to breath underwater is explained in the season 7 episode, "Mermaid Man Begins". Mermaid Man is able to breath through the star on his nose, which works like an oxygen tank and Barnacle Boy can breath through barnacles on his lungs after Mermaid Man saved him from drowning.

I didn't leave that in my comment on the Television Theory but something else I realized in an older episode in the show gives credence to this theory aside from how blatantly the episode Feral Friends outright confirms it. In the beloved episode Band Geeks we see the fish in Squidward's marching band react to the human audience waiting for the performance. The fish all look disturbed and shocked at the sight of the humans. Patrick says "Those are some ugly looking fish" while Mr. Krabs says "I think I'm gonna be sick." I NEVER understood that joke growing up even though I could appreciate the morbid surreal nature of it on a basic level. But now I realize that that was early confirmation of this theory from the creators that the fish in bikini bottom are being manipulated into being treated as experiments by humans working for the government. And what's even weirder that I realized about this? Squilliam was originally supposed to be doing the performance for that bubble bowl. Which makes me have to wonder...is Squilliam secretly in on this? And then there's also the weirdness of the Santa scene in the episode Christmas Who.

What if sandy is the French narrators pet? It could explain a lot if that is the case.

I think mermaid man and Barnacle boy are real superheroes in this universe (they are obviously parodies of Aquaman and Aqualad crossed with Batman and Robin). Maybe the showmakers (in this universe theory) thought aquatic themed superheroes would be easier to sell to aquatic creatures. Especially since Mermaid Man is shown to have the power to communicate with “the creatures of the deep” (the other residents of the retirement home)

I didn't notice the size discrepancy of the characters either but supposedly in the origin story episode of those two they fell into a vat of radioactive chemicals that basically gave them an urge to breathe underwater it's better explained in the origin episode for mermaid man and barnacle boy but that's basically what they went with there was a lot of different origin stories in that episode so I don't know which one is concrete
