Dr. Stephen O'Rahilly provides a biomedical perspective of obesity, and evaluates which genes could potentially shift the balance towards obesity.
Easy access to nutrients has contributed to the increase in obesity in the human population. But, what is obesity and why isn’t everybody fat? Dr. Stephen O'Rahilly provides a biomedical perspective of obesity, and evaluates which genes could potentially shift the balance towards obesity. As he explains, one becomes obese when the balance between energy intake and energy spent is shifted. Surprisingly, mutations that lead to obesity in humans aren’t in genes involved in metabolism and energy storage, but failure in satiety signals in the brain that result in people eating too much. The excess of energy intake over energy expenditure leads to obesity.
What is the consequence of obesity in human health? Physically, obesity can result in lower mobility and sleeping disorders. But, in humans, the link between obesity and metabolic diseases isn’t straightforward. For example, not everyone that’s obese becomes insulin resistant. As O'Rahilly explains, the probability of an obese individual to have a metabolic disease is linked to the capacity of adipose tissue to store the extra fat. Mutations that decrease fat storage in adipose tissue increase the chance of metabolic diseases, like insulin resistance, even when the person is not obese.
Speaker Biography:
Dr. Stephen O'Rahilly is a professor of Clinical Biochemistry and Medicine and Head of the Department of Clinical Biochemistry at the University of Cambridge where he also directs the MRC Metabolic Diseases Unit in the Wellcome-MRC Institute of Metabolic Science. He qualified in Medicine from University College Dublin, and continued his post-graduate training at Oxford University and Harvard Medical School. In 1991, O'Rahilly joined the faculty at Cambridge University where he studies human metabolic and endocrine diseases. O'Rahilly is known for his work in identifying novel extreme human metabolic phenotypes, and identifying genes important in metabolic function and dysfunction.
For his scientific contributions, O'Rahilly was elected as a fellow of the Academy of Medical Sciences (1999), and the Royal Society (2003). He also became a Foreign Associate of the National Academy of Sciences (2011), was knighted in the 2013 Birthday Honors for services to medical research and was the 2019 Banting Medal Recipient for Scientific Achievement of the American Diabetes Association.

I’m leanish because I don’t eat as much as other people. I can control my appetite because I am not particularly obsessed with food. Not claiming any type of superiority here, just how it is.

To lose weight, I followed a vegetarian oil and fat free diet for 23 years. In the end I was my heaviest ever. Then I discovered the Keto diet. I went low Carbohydrate, and added eggs, nuts, cheese, fats and oils, but remained vegetarian. But no fruit or vegetables that grow underground. The effect was dramatic. I lost 10 kg over a period of a few months. Careless eating brings on weight gain. One has to have the discipline to follow the diet for always. Food is for nutrition and not for entertainment!

You are on to something. My mom and dad worked in a factory hard. We all ate the exact same meals at home with no snacks etc. My dad was skinny and my mom fat.

Everybody is different. I was fat from childhood. My DNA results confirm what I always suspected. I have many different fat genes. Genes vary widely, even within families. My siblings were thin.

Great presentation! I have two questions: 1 I am currently fasting, and feel hungry. However, I know that the feel of hunger disappears after some days of fasting. Delicious, moorish, available, cheap, calorie dense, non-satiating food. Okay, other things may cause us to overeat such as boredom, anxiety etc but at the end of the day it's the aforementioned that will get you in the end.

Food seeking missile - You have won the Internet sir! Seriously, A wonderful presentation, this is great information. And brings up some interesting questions about how society views adiposity. By the way, thank you for that term, I'd never heard it before. I was wondering if leptin could be used as a short term appetite suppressant, for use during, say the traditional holiday seasons when overeating is more prevalent? Or as a short term suppressant to reduce the distraction of appetite during stressful periods?

Not to sound rude, but the question you asked is in the video. The simple answer is no, leptin cannot be used as a "suppressant". Very, very few people lack leptin altogether. And for the vast majority of us who do produce it it doesn't act as a regulatory agent (meaning it doesn't have a direct correlation between levels and how much we store fat or ingest).

if you want short term appetite suppresion, GLP-1 injection (liraglutide) is pretty effective. your appetite disappears. its like ~£250 for a month worth of injections

Bizarre. We know what is the controller of fat storage. Insulin will store fat if it is elevated in the body. If you eat anything that stimulates insulin you will store most in the liver and muscle but if its more than your requirements to stay the same weight, your liver will package those extra calories as fat. What stimulates insulin more than anything else? Processed food of course. It has no nutrients, its not satiating, and food companies have made it as addictive as possible. Yes other hormones are involved but its not all a leptin deficiency. That would be like saying that since they gave out the USA food guide, humans developed a leptin deficiency. No its hyperinsulinemia. And insulin is also involved in heart disease, hypertension, cancers and of course PCOS. Read Dr Benjamin Bikman’s book “Why we get sick.” Go to his facebook page InsulinIQ, check out his website. It is a rare person who has leptin deficiency. We have learned this from Dr. Robert Lustig an endocrinologist, researcher, lawyer. You can check out his RU-vid video, Sugar The Bitter Truth. So far it has been viewed 11 million times. Get on a low-carb or Ketogenic diet. Start lifting weights. Stop eating all the time. Twice a day is perfect. Fast at least 16 hours a day and eat 2 meals in the other 8 hour window. Do not allow another fake food into your mouth. Eat real food. Stay away for breaded garbage, sugar products, bread products and starches. Read Gary Taubes book Good Calories, Bad Calories. Read Zoe Harcombe’s book The Obesity Crises. Go to her website and read the studies. Read the books from the scientist who study this by Dr Jeff Volek and Dr Stephen Phinney. Go to the website virtahealth.com and read their studies. Follow Dr Eric Westman from Duke University. The major reason that most doctors will not look at this evidence is because their research is all wrong. So you have to deny the truth. You have cognitive dissidence. The studies are pilling up. Wake up.

Narcs are the problem. To handle them, people over eat, over drink, masturbate excessively. That morally superior skinny person you spoke of early in the vid. Yup. He is a narc. I got my first clue to this when I deleted some narcs off my social media. I was sitting here one night - copping another load of criticism and abuse, something that apparently I should be able to tolderate, and I said to myself "Whyyyyyy? Why am I listening to this shit?" So I deleted him and another one just like him. Next think ya know, I could feel my alcoholism float away from my body. After 50 years of alcoholism, I suddenly no longer wanted a drink. I've had alcohol since then, but not often, and I have no trouble stopping myself. Right now, I'm narc free again, so I thought I would give up cake and get my blood sugar in order. I've been eating sweet food since I was 1 year old, but again, no narcs - no problems

A complex chain from DNA through an individual ‘ s body type and familial background to his or her society all have an affect . It seems to me that in the Uk too much attention is paid to the phenomenon of obesity as a visible and measurable marker of Ill- health rather than studying why some obese individuals maintain relative good health whereas others (perhaps a majority) develop health complications. A huge industry of gymnasia, personal trainers, fitness experts , dieticians has developed to mirror the exponential growth in the availability of effortless , and often nutritionally deficient, food. Targeting “obesity” per se as your intervention point is misguided as has led to damaging psychological effects of body image and increased vanity. Californian cults demonising either carbohydrates, dairy products, meat or even fruits just complicate what is already a multi - faceted issue. Integrating exercise into your routines by which I mean walking to work rather than driving to the gym. And being a little conscious of your energy intake rather than meticulously counting calories, based upon a varied and balanced diet , such as the Mediterranean diet is surely the way forward. But are the political and commercial powers likely to promote this.?

There are two main reason for obesity. One is that eating something, which triggers digestion, means the PNS is ignited and that also facilitates resting metabolism. So eating something, especially something greasy, which takes longer to digest, is a form of self medication. And safer than drugs. The other is a coping habit. I found that there are two coping habits one involves deeper breathing which raises the metabolism. This means there is more work for the brain and that causes distraction so the stressful thoughts are suppressed. This will lead to lower weight, thinness. The other is shallow breathing, which causes lethargy like the early stages of sleep. This means the person is less aware of the stressful thoughts. However this leads to lowered metabolism. If we burn less we will store more. So weight gain is a side effect. One can overcome these coping habits.

Could it be an epi- genetic phenomenon as a result of famine? My great great grandmother the daughter of a rabbi was severely morbidly obese even though she was adopted out of the family indicating genetics of some type. Also her daughter morbidly obese though not quite as morbidly obese down to me the grandchild nearly 40 years old 115 pounds ARFRID eating disorder

Great talk . But we know salt was never an issue. Finally we moving towards the hormal regulation model to treat obesity. Lets start with food first before pharmacological interventions. Lets have food which keep our hormones in balance.

Very interesting lecture and reassuring for those of us who have a predisposition towards obesity that it's not just a moral failing . However, whilst we await magic bullets I suggest that very cheap and effective therapeutics already exist . William Banting already gave us a clue over 150 years ago . Basically cut the carbs - particularly the refined ones . Combine this with time restricted eating and occasional longer fasting periods . I think that people with a tendency towards obesity have extreme insulin responses towards grains and carbs etc . I was a high wholegrain vegetarian for many years and despite exercising intensely and regularly was getting fatter and sicker . Barry Sears work in the " Zone " showed me carbs will inevitably lead to hyperinsulinemia/ metabolic syndrome but it was only when I combined this knowledge with Intermittent Fasting following my watching Dr Jason Fung's videos that I was able to normalize my weight and appetite . Giving up bread in particular had a profound effect on satiety in my case , and I might add that ultimately I was eating very high grade wholegrain artisan bread . I find that I can go for a day barely thinking about food if I have coffee with a small amount of butter in it or green tea with coconut oil in it . I would like to see this doctor's team look at the effect of insulin on obesity - after all one of the diagnostic features of type 1 diabetes is this dramatic weight loss .

When you are working for the drug companies as most doctors are nowadays that is where the focus will be because if they could (which they cant) find a "cure for obesity" in pill form they'll ALL be rich! In the meantime they will continually bilk the insurance companies for all of the medical testing they can possibly order up.

I think we all have individual hunger triggering foods - mine are grains generally ( wheat in particular) , oats and alcohol ( particularly as beer ) . It would be good if this well meaning and highly educated doctor had some success with low carb and IF himself . He is obviously type 2 / insulin resistant / NAFLD himself - whether he knows it or not . He is in a great position to influence people to advise non medical interventions for metabolic diseases.

A lowfat whole foods plant-based diet is very good for promoting a healthy weight. It has lots of fiber and bulk. I eat that way, and I like being stuffed after meals :)

Any habit can change and it's easy, just live consciously not compulsively. Don't let your addictive nature control your life, it will lie to you. Instead simply plan your actions logically and sensibly in advance and stick to it. To live consciously a yogi Sadhguru has great insights.

So many factors - it is important to KNOW THYSELF. With me, it's carbs. for others, it's fat. These genetic factors can give a TENDENCY one way or the other, but there is no cookie-cutter solution that works the same for everyone. (That's why I don't want a government bureaucrat managing my health.)

If your obese it’s without a doubt a mathematical equation. You consume more calories than you expend. Move more or eat less, ideally do both !

Obesity was a rarity, almost unknown until the late 20thC when food processing became common. It's not about calories, its about the carbs man! Sugar makes you hungry switching off satiety and commanding the fat cells the store fat. The speaker loves his pastries. All he needs to do is do a KETO diet for a month and his belly will disappear, and what is more he will not be hungry as he would find on a calorie constricted diet. Genetics is COMPELTELY irrelevant to treatment since you cannot change your genes. The twin study is not relevant since the dietary environment was basically the same for each. It's all very interesting to know (as we already did) that some people get fat more than others, but so what? People who are fat need to know what is it that they are eating that makes it so easy for them to fatten. Why are the foods they are eating not satisfying them? Protein & fat satiates; carbs make you hungry. Once you know that you can loose weight without feeling tired, depressed, and hungry. There is a very good reason that we have evolved the sugar insulin response. In autumn we are designed by nature to build fat for winter storage, by eating fruits. In the modern world, however, winter never comes, and sugar is available all year round. Worse still the advice is low fat, high carb which makes you fat. Food processing has removed fat and added sugar. Taking drugs is the worst thing you can do, breaking the body's own systems to allow people to eat regardless of common sense can only trend to more T2D, and atherosclerosis. Sugar is harmful, taking drugs to enable more sugar consumption is a mistake. Leptin is suppressed by the sugar insulin cycle. Stop eating sugar. Eat fat, grow slim.

Let us talk a bit about population genetics here. Yes, there are so-called 'obesity' genes. Now, how many of the UK population has it? Let us also talk about phenotypic expression, in general, & gene expression, in particular. This equation is commonly taught in Genetics: Phenotype = Genetic + Environment + (Genetic x Environment) Since the influences of genes, environment (i.e., all factors other than genes, e.g., nutrition, exercise, etc.), & the interaction of genes & environment can be quantified, how much exactly is the contribution of genes, the environment, & their interaction in the expression of obesity in a population? I posit that genetic testing for obesity is in order & be done as a standard so to clarify obesity diagnostics that is due to genes. That way, it will give us a clearer picture as to whether genes is the most probable causation to any individual that is obese (or morbidly obese).

You can become obese also eating healthy food, because it's also a question of math, if your intake is more than what you burn you will get fat (because fat is a storing mechanism for the body), BUT generally speaking greens, legumes, carrots (and soo on), [who are not processed with extra additives and sauces (so food with higher calories)] have a very high content of fibers, which gives sense of fullness, which are mostly expelled by your body, work on your insuline (which makes you feel satisfied) and so on.. so generally speaking yes, you can become fat by eating healthy, but you would have to eat very big quantities, compared to process foods (or even ultra-processed foods, such as Industrialised bread, Pre-packaged meals, Breakfast cereals.. ). Some food has a high quantity of calories, but they are also healthy, such as nuts, almonds, bananas...

It won't work. Most fat people have plenty of leptin, but their bodies can't respond to it. You are more likely to be leptin resistant than leptin deficient.

What is so difficult about eating twice a day, eating from the same sources consistently, and making sure the foodstuffs are at least 90% plant based, whole food ingredients? Is it really at the point now where even the best minds are in themselves growing fat as a way to subversively influence and perpetuate the ignorance of the masses? As members of the human species, of course we have the same biology. Once people have done telling you their list of problems why they are so fat, you have only to look in the fridge to find out what's really going on.

Thankyou for sending the video, I enjoyed watching it. I am quite interested in the science behind energy consumption and metabolism. I really do think it's within everyone's power to enlighten themselves, it's just that people aren't encouraged to think for themselves enough.

No, leptin deficiencies are pretty rare. Most obese people simply are resistant to leptin. Something has caused their hypothalamus to stop detecting the leptin signals produced by their adipose tissues effectively.

Yo, Dr Steve, you are insulin resistant. I can tell from your belly. Why isnt everyone fat? Because some of us do not eat processed food full of sugar, starches and imflamatory so called vegetable oils. We eat real food. Meat, fish, eggs. Vegetables optional. We are fat adapted fat burners producing ketones. We are evolved to be this way. The reason obesity is evident in antiquity is because they had to subsist on starches (grain) to feed large populations which induced insulin resistance and chronic disease like we have now. The danger of processed food must be understood.

Its fundamentally flawed to equate the path of developing obesity therapeutics (and the need to use pharmaceuticals vs intensification of lifestyle change) to the path of hypertension therapeutics. Youre comparing apples to oranges.

Thank you. That was very interesting indeed. It is so good to have so much detail. I tend to the natural school of thought so if we can eat real whole foods and ditch the junk most people can lose weight and feel healthier and add in a bit of intermittent fasting too. However I know lots of people find that very difficult. I do think in the UK since Covid 19 we need to look at the costs to the NHS from obesity and if we need more of a stick rather than a carrot approach - perhaps people must pay up front for medical care if they are over weight and they need to be aware most of those who are not elderly who get really awful covid 19 symptoms are over weight and you are destroying the NHS and probably l leaving your children with out a parent if you don't eat much less.

What about attempting to change the food environment instead of the stick? A Big Mac might cost £15, and apple might be 2p. Want the Big Mac delivered to your door? There is a £10 levy that is added to that. The apple is delivered free. I wonder if that might impact the rate of obesity.

He’s not close to, by every medical metric he is obese. People don’t understand how easy it is to lean into the overweight category let alone obese because our views on what’s normal in weight has been skewed so much. 70%+ of people I see would be at least overweight, yet they don’t think so, and others don’t either. Unless you’re a giant blob of fat most people don’t think you’re even big. It’s scary.

Fat people do NOT eat too much. Causes of increased obesity: omega 6 seed oils & sugar (fructose). Both cause insulin resistance, which keeps you from burning energy. When you can't burn it, you have to store it. HOWEVER, you can look thin & still get insulin resistance. It's the liver fat that matters. Weight gain is only a symptom.

Intresting but i do not agree with your statements of hypocaloricdiet and exercise. everybody NEEDS to exercise, and that's the end of the story. And that's that's something that is needed to be programmed in, through out good training practise. It's a matter of developing your brain structure, and that is difficult with older ppl ofc. and the hypocaloricdiet is temporary that more or less in the name..... you can't be hypocaloric forever...... i till be dead honest.... stupid reasoning around diet and exercise otherwise good recap of previously known empiri.

80% of people who loose weight will regain the weight back in 2 years . The body is clearly sabotaging their progress . If you have never struggled with fat loss you wont understand. You get hungry when you are not supposed to get hungry , you rarely feel full and after you loose weight your body makes you 4X hungrier so you can go back to your weight. This is all worsen by a bad diet of course but leptin resistance plays a big part. I do believe genetics plays a big part . My adoptive parents are skinny but I have always had 20 pounds extra , I’m pretty sure I would have been obese in America. I have started to taking Ozempic and I finally understand how nromal people feel. I feel satiated after eating and I don’t need much food to feel full. It’s not about self control it’s hormones. I bet If you gave me 10 million I would still regain the weight!!! Because my body will make me hungrier and hungrier

But it's certainly also partly because lean people make conscious choices that keep them lean, including choices to regulate their environment so they don't overeat. I'm lean myself, and that's important to me, and I make conscious choices to stay that way. Lowfat whole foods plant-based diet; weightlifting and aerobic exercise. I could get plump if it didn't matter to me much. I've gotten pudgy in the past from overeating, but lost weight again before I got fat. And it's also true that lean people tend to be more in a culture and environment that doesn't promote obesity. This guy has an interest in emphasizing the different susceptibilities that different people have to obesity, rather than the conscious choices that people make, since he's obese himself.

You may already know, how there has been fully professional, medically endorsed, documented scientific research that has conclusively PROVEN, how overindulgence is NOT the cause of obesity, and that dietary reductions DO NOT produce effective, corresponding loss of weight. Where you go to pull up and read these professional studies, I don't know. I read about them in 2 hardcover books in the library. I wish I would have written down the titles and authors, but I didn't

Yes, as a skinny person who once believed that it was that simple, I was happy to learn something new here that's changed my perspective. Food has become so much easier to access that people with particular genes controlling appetite become more susceptible. I really like that GLP-1 study, the only issue is how long it takes the body to burn that excess energy. Perhaps if a doctor were to notice that a patient was getting obese, they could perform a genetic screen and then prescribe them the proper signalling factor, and prevent obesity. This is really exciting research.

Taurus Londoño i even did more than just watch the video, i also understood what was said! To help you out i'll summarize... Person A and Person B both consume the same amount of calories and burn the same amount of calories as each other the amount of fat they will store will not necessarily be the same due to genetic factors BUT... If Person A consumes a certain amount of calories and burns a certain amount of calories PERSON A will store less fat if they consume less calories, or if they burn more calories, or both. In other words, EAT LESS and MOVE MORE. It is actually very straight forward biology and physics.

@@stupidas9466 Calories in/calories out has been debunked. It is NOT a matter of "eat less, move more" as you imply. We are not bunsen burners so our bodies don't process food in a straight forward way, it is much much more complicated than that as our bodies work through a much more complex series of chemical processes. Law of thermodynamics does not apply here. More research is coming out that shows that the topic of obesity is not what we necessarily think. Though I do think you are just bigoted and can't accept any of the new science coming out about obesity that shows that it isn't necessarily an obese person's fault that they are they way they are. There are more coming out that even shows that it may be the fault of one's _genetics._ therefore obesity might someday be claimed as a trait and thus our bigoted attitudes toward them will be seen as wrong; actually, that is starting to happen now.

nah i’m skinny as hell and all i eat is pizza, candy, and milkshakes and play video games all day with very little exercise. All the unhealthy food just gives me acne but i don’t gain any weight. I wouldn’t be surprised if alot of overweight people were healthier than me

@@bigflat1238 I think a lot of overweight people just have a natural tendency to overeat. I eat whenever I desire to and stop whenever I get full or it gets boring. I notice this whenever I go to a buffet with my heavier friends, I always finish eating first. Obese people definitely should be eating healthier foods and exercising regularly but I don't think thats going to keep off a substancial amount of weight if their brain tells them that their starving

not necessarily, my aunt was an extremely skinny lady, but somehow she got thyroid disease and failure. She gets fat now no matter what she eats. It isn't really necessarily a matter of overeating or directly jumping into conclusion that they overeat. Some people do get diseases, problems, heartbreak and use food as comfort (as which they overeat too), some people tend to eat less but still gain weight easily because of genetically slow metabolism. One of my friend is extremely skinny, and I swear she eats too much junk food for every 10 minutes and keeps saying that she is hungry. If a person with genetically slow metabolism ate the same amount as her, I'm sure enough that person will die from obesity. Also, obesity can sometimes be from environmental factors, less sleep = less leptin. Stress. The main thing leading to obesity and destroys the person. Sometimes a person is living in a bad environment or unsafe place forcefully, where they can't really go out of the house and get fat because of lack of physical health. Yeah, the main problem is overeating, but not necessarily always.

it's been with us since we moved into cities. hunter gatherers are on average not even close to as fat as us. Not sure this guy has been reading up on the latest nutrition info. just fast, quit eating so much, and get a tiny bit of exercise. quit eating inflamatory foods. that'll reset your set point and control your hunger.

Stephen Hazel people really do need to stop eating inflammatory foods.most asians keep normal body weight until they move to a country like America. The food itself is contaminated on many levels. No one is getting fat on rice, fish, meat, veg and fermented foods--all foods normally in an Asian diet. As new younger generations of Asians eat western fast food and sit in front of computers all day, they are also now getting fat. its really sad what people think is healthy food. I agree with you on that!

I have rarely heard so much nonsense about obesity except maybe from vegans. Yes, a few people are geneticslly obese. And it is not calories in versus out. It is purely what macros we eat when. It is about what drives up insulin for how long. Insulin stores fat and stops lypolisis. Control insulin through diet and you control hunger and obesity. Looking at the body of the speaker he clearly does not understand obesity.

So you're saying that I can eat as much as I want and not gain a pound provided my insulin is controlled? You're saying that when people gain weight it's nothing to do with energy intake versus expenditure, but it's because their insulin levels are off? Eating too much and sitting around isn't what makes people fat, it's what their eating. So people can continue consuming the same amount of calories and not exercising, they simply have to eat the correct calories? I really really want to know how that works please enlighten me
