I'm a recovering addict and had an ovarian cyst rupture. It was horrific pain, and I had to explain over and over that I don't want opiates for my pain. She then still insisted I get an iv in "just in case I change my mind" that I still had to fight her on. She hadn't even offered me tylenol. It takes an unbelievable amount of willpower to stay clean and sober. It's hard to make those calls, and I'm really glad he's talking about it

Dr Mike, I think us in recovery need more content discussing pain and addiction. This discussion has weighed on me as I approach my surgery in December. Would be a great thing to know how to discuss our addiction concerns with our physicians/surgeons in situations where medications are unavoidable.
@summerdais325
@summerdais325 Год назад
It will help if you identify your biggest concern and prioritize them in a list. Example: surviving surgery adequate pain coverage relapse This is common and can look like talking to your surgeon and anesthesiologist. You would say something like my biggest concern is surviving surgery followed closely by a fear of maintaining my recovery while getting adequate pain coverage. Sometimes narcotics can't be avoided. That said, they will encourage you to take as little as possible. If your fear of relapse is high and your surgery allows, then they may suggest using a scheuled suboxone or methadone regimen with a taper. In case they don't mention it, max out your Tylenol from all sources to 3,000 mg/day to protect your liver and exercise caution using ibuprofen to protect your stomach. Patient education is vital. Some people don't understand the dangers of these OTC analgesics. You probably do. If not, please ask your doctor, a pharmacist, etc. You got this! Do you have social support to surround you during your surgical recovery? If not, I will try to offer up some suggestions. -disabled/former nurse with a special interest in addiction and pain/palliative care.
@aggiesanvil
@aggiesanvil Год назад
@@summerdais325 I have support but none of my support has an understanding from addiction experience. Also I have chronic pain which was the cause of the addiction, but why I am maxed out daily on OTC (I do rotate and take breaks from OTC meds). I kind of laugh because I now realized that this is my 17th surgery and never has "surviving surgery" been even thought about. It's always been pain. Old me would be excited over IV meds. New me has already scheduled this procedure to be done while I am awake because general anesthesia meds are my biggest trigger, those 10 seconds of euphoria before going under (like SteveO mentioned the beast awakening). As candid as I am about it, it doesn't seem like most physicians get it or empathize. I mostly feel judgement, so maybe I am delivering my concerns poorly.
@Thoran666
@Thoran666 Год назад
@@aggiesanvil I've found most docs are quite chill about it. When you discuss a procedure with them say: "I'm sober from xx for yy time, is this going to be a problem?" They should know what might trigger an addict and especially if you are polytox that information can make the anaesthetist's job a lot easier. I've heard stories from people that needed 5-10 times the normal amout of a sedative because of their drug histroy and building up a resistance.

Hey I had surgery last year after 10 years of being addicted to pain meds. They leaned more on Ketamine in the hospital. I had major surgery it was really painful. They removed a big piece of my fibula and all the soft tissue around it. It was brutal and when I left the hospital they have me 7 days worth of percocet and 3 days worth of fentynal I had no choice but to use it because it was really painful. In my experience, if you're healthy in your recovery you can do. Tylenol is really a lot more powerful then I ever knew.

Everyone says this like it's so easy. It's not. Dental care is so freaking expensive even with insurance. I'm on disability and I need a heart transplant. Medicare doesn't offer dental care. And dental care is soooo important especially when it comes go cardiac health. But without insurance I can't afford it. I don't recommend it but I've had to pull my own teeth because either didn't have the money to go to the dentist or they wouldn't do the work because of my heart. If you can find one, the dental schools do offer great resources for dental health at a much discounted cost because they are students doing the work overseen by licensed dentists. Again, because of my heart they won't work with me....and that I absolutely understand. Dental hospitals should be covered under medical health insurance because chances are when you're at that point where you need a dental HOSPITAL, it's a MEDICAL EMERGENCY.
@Aquelll
@Aquelll Год назад
Also not having your teeth healthy can be a contradiction to more serious surgeries like open heart surgery because of the risk of infection. So dental health is really important for your overall health. And I live in Europe and I pay dental care in my taxes so I am not getting into the discussion of the cost of it.

@@hypercrow9563 You’re absolutely not alone in this battle. Constantly fighting it can be so exhausting that there’s just nothing left, but every day that you continue to fight truly *is* a victory! I know it’s way easier said than done, but if you can try to take off some of the pressure about the impending sense of doom (and/or the stupid freaking nightmares of teeth falling out), it can help you be able to put that “one foot in front of the other” into action. When Steve was discussing his burn ordeal and his hard choice to accept pain medication, I was so proud of his mental process in respect to his sobriety. I have a friend whose son was sober and went to the hospital and told them no pain killers. They gave him some and that was the end of his sobriety. I understand what a challenge it can be to maintain that level of commitment and applaud his success. I also applaud Dr. Mike for giving him this platform to speak to a large audience about his health struggles including oral care.

I’m SO HAPPY he mentioned flossing. Flossing is absolutely CRUCIAL to maintaining good oral health. I think some people think they can get away with not flossing (because people have said as much) but the truth is, no one can. Millions (literally) of people are walking around with gum disease and don’t even realize it. The bacteria in the plaque WILL eat away at your gums and tooth enamel if you don’t regularly remove it. I was always very strict about my dental hygiene. I had great oral health. Then I was in an abusive relationship and stopped caring for myself. I didn’t perform good dental hygiene for 2 years. It caused me to develop periodontal disease and I had to have multiple cavities filled, 2 root canals, and even had to have a tooth pulled (luckily my very last molar, which isn’t super noticeable). I was crying when the dentist pulled it for a couple of reasons: 1) I had always maintained great oral care and I was so upset how quickly my gums and teeth deteriorated when I stopped, and 2) It was a reminder of the power and control my ex-boyfriend had over me for so long, to the point I was in such a dark place I didn’t even shower or brush and floss my teeth. Now every day I run my tongue over that empty space and it reminds me how important it is to brush and floss every day, and it also reminds me that I survived a nightmare and came out the other side stronger, wiser, and happier. Long story short: Get help if you’re being abused, and FLOSS! You don’t realize how important it is until the dentist tells you that you have periodontal disease or multiple cavities or need a root canal or a tooth extraction. Neglecting flossing or brushing doesn’t seem like a big deal at the time, until you realize the severity of the consequences of your actions by neglecting to do so. ❤❤❤ Be sure to love your teeth and your gums ❤❤❤

I know exactly what steveo is talking about regarding pain management and addiction. I am in recovery from drug addiction as well, four years sober. I had a third heart surgery three weeks ago. It has been by far the most painful experience of my life. I was very hesitant about the use of pain meds at first, but they do have a place. I did need that help the first week post op. But That Dilaudid clicker woke all my opioid receptors like crazy! The doctors offered me a few more days of my new friend that Dilaudid, the addict in me really wanted to say oh yes please! But I know, for me that leads to no good. And the longer I’m on pain meds the harder to get off pain meds. So I stayed strong, said no thank you, Tylenol only for me please from here on out. I was tapered off the pain meds, still on the Tylenol ( it really does work) and doing just fine! Omg and also… I have a friend who has the worst breath, just all the time, it’s hard to ride in the car with this friend the breath is soooo bad! I bet it’s the same type of bacteria Steveo is talking about. I really wanna say something to my friend but I’m so nervous, I really don’t want to offend. I don’t even know how to bring it up and gently say something. Maybe this video can be the catalyst to jump start the convo? It’s one of my regrets as well, not taking my dental heath seriously at a younger age. I’m now 41 and had to have 12 teeth pulled and wear top and bottom partials for not caring about my teeth before. Brush and floss daily kids! You will thank yourself later, trust me. Great vid! Thanks!

Dental pain is terrible but I’ve had chemical burns which were far worse. There’s a difference between my jaw killing me in a single spot with radiating pain and sharp pains and knowing it’s that’s spot and relatively that it’s a tooth issue. And thinking: ‘oh my god I’m on fire and there’s no flames I see my skin dying over the course of minutes and I’m not hallucinating but this is not normals and idk whats happening I’m burning I can’t touch anything and I’m shaking and it’s at an 11/10 over a large part of your body...’ Different fear levels surface area and pain. You can see it which makes it so much worse and the pain doesn’t pulsate it’s just existing on all nerves ends at once and then as your blood rate picks up you feel more pulsing effects. Taking care of burns feels easier then tending to dental issues as it’s not made common practice. If you don’t know how you got the chemical burn or what it is neutralizing it is a bitch to guess at and there’s a panic stage of ‘mystery major problem’ and rapid fire solving. The chemical burns or standard cooking burns are whatever to fix and managing them is alright the pain lingers for a long time and there’s not great pain-relief options. I’m currently unable to get dental or health looked at and my impacted wisdom teeth have started to encapsulated on one side into an abscess while also leaving that signature back molar/gum line pit/plaque. Thing is terrible but it’s like a 2/10 for daily issues and draining/flushing has been fine with no signs of infections/tissue damages. Special diet and tooth cleaning to keep the molar from getting a cavity while the encapsulated one slowly rots dissolves and falls out. Saline rinses but not overdoing it if ever an issue there’s minor dental things marketed for like elderly people that help with pain/sensitivity/smell/bacterial stuff etc as their teeth quality has different special requirements that sometimes help if you have other dental issues

I had this same problem. It didn't pop up a ton, but i started brushing my tongue like crazy, to the point of damaging my tongue a lot, because i thought that was what the bad smell was from. But it turned out i needed to floss 🤣 finally figured it out and it hadn't been an issue since. Floss people. You need to floss.

I've been sober for almost 5 years now and have had to have pain meds a couple times and surgery. Only time I've needed anything was for a horrible abscess in my jaw. They gave me IV ketamine and I immediately said "yeah nevermind, I'm good on that" lol. I did just have them sedate me til surgery, mild benzo, then after surgery felt 100x better and had nothing but Tylenol and ibuprofen since (and my normal meds.) That was up there for pain, even including pre-sobriety. I've had open heart surgery, an amputation, etc. Face/tooth pain when really bad, is bad. Had to have 11 teeth pulled at once and my jaw scraped lol. Felt great after though, what a relief haha.

Jesus. Tylenol is hepatotoxic, even the leaflet says it can't be taken for prolonged periods, it's just temporary pain relief. Motrin is ibuprofen iirc so while they can be taken concurrently, I don't think months of maximum daily dosage is safe either

dental assistant here 👋🏼 i work in oral surgery, and I’m so glad that he explained the importance of flossing. i can’t tell you how many patients come in needing extractions bc they don’t floss. it doesn’t only cause cavities, but the bone will slowly deteriorate. which will make it difficult to replace the tooth with an implant without bone grafting. the patients will say “But i don’t understand i brush every day!” your toothbrush cannot get ALL the bacteria and food that’s stuck in your teeth. When you floss, if you floss kind of as if you’re going above the tooth, you will see what comes out of there. Even after you brush. It’s also important that you visit your dental hygienist every 6 months to avoid long term gum disease or extractions. Please please please floss!

I'm living proof that everything you said is 100% correct. I'm 54 years old and I've visited the dentist twice a year for as long as I can remember. I'm kinda OCD about brushing, flossing and tongue scraping. When I floss, I'm always amazed by the amount of food that's stuck in there! (I have tight teeth.) The payoff is that my gums are in excellent shape. 💪🦷 I've never had gingivitis and my gums don't bleed during cleanings. I've only had three cavities in my life: two of them were when I was 7 years old, but one when I was in my 40s. (How? Where'd I go wrong? 🥺) I still have all of my teeth, except for my wisdom teeth and an upper right back molar (#2) that I cracked from clinching my teeth, even though I sleep with a bite guard. I have fibromyalgia and the constant pain makes me clench my teeth. Not only did I crack that molar in two places but I regularly have to replace my bite guards because I crack them, too. I have too many health problems to mention, but the _one_ good thing I've got going for me is my teeth! 😁 Please take care of your teeth, people. And FLOSS! It doesn't take that much effort and it's _sooo_ worth it!

When the smell hit me I literally covered my mouth and said omg it’s your breath! He got a dentist appointment the next week and had to have some kind of surgery/procedure to get all of the infected teeth out and deep clean to try and get rid of as much bacteria as possible. It was one of the worst smells I’ve encountered and I felt so bad for him…

Definitely shouldnt come to a youtube comment section for this even if it's that of a doctors comment section. 100% go see a doc in real life and get you're answers there. Now, for the funny man internet guess You should definitely chill out with the sports. Too much of anything can be very bad for you and I personally think 4 sports all at once and having ti practice so much everyday giving you very little recovery time for your mental and physical health. If you really don't want to visit a doc, if I were you I'd just drop down to 1 sport after taking a week or 2 to recover.
