Strattera (Atomoxetine): a Non-Controlled ADHD medication without the Abuse Potential of Stimulants

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Strattera (Atomoxetine) a norepinephrine reuptake inhibitor that can be used in the treatment of ADHD for adults. This medication does not have the abuse potential that you see with stimulants like Amphetamines (Adderall, Vyvanse) or Methylphenidates (Ritalin, Concerta) and it is not controlled. It does have various side effects (and monitoring vitals and seeing your primary care provider for regular visits is recommended). With that said, it may be a safer option than many ADHD medications, and can be surprisingly effective. Dr. Pagnani discusses that it's not uncommon that he evaluates adults who trialed this medication, but did not have it increased to typical doses that are effective for adults. He also discusses that some individuals who are poor metabolizers of P-450 2D6 may do well on very low doses (and become toxic on higher doses), and it's vital that individuals seek treatment from a medical professional, for these reasons.

I’ve been taking Strattera for about 3 weeks and I’ve had some of the best days of my life. I have energy and motivation to follow through with completing projects and things I’ve needed to do for years. I have not had any side-effects at all. I even have Gastroparesis and this does not bother my stomach. I started on 40mg and added 25mg in pm. I am fascinated with the significant brain changes and I am just grateful something works.

Te consulto que tipo de Tdah tienes , yo tengo mala memoria a corto plazo, mala concentración o enfoque, razonamiento lento, sueño de mala calidad, ansiedad, soy muy inatento y tengo hiperactividad, movimiento de piernas , me aceleró cuando hablo etc, estoy tomando metilfenidato de 54 MG y no mejora esos aspectos, saludos

​@@fernandofran2360Hey, my adhd symptoms are extremely similar to yours. I tried Ritalin (methylphenidate) after switching off of adderral (self-diagnosed heart concerns lol) and boy, it did NOT work for me. It let me focus on things I shouldn't. I'd play madden mobile and refresh for trophy drops for 6 hours, neglecting everything else. I was like a zombie. I switched back to adderral (at 15 mg instead of 30mg) and it was night and day. Adderral worked very well for me, aside from some personality changes (a bit of an elevated ego). I am now getting back on medication 4 years after going cold-turkey. I cannot get back on adderal until I get an EKG (at my doctor's request), so I requested to try an alternative in the meantime. He prescribed me Strattera (atomoxetine). I will be taking 40 mg for the first 7 days to adjust, then increase to 80 mg per day after. I am now at the end of day two. The first day, for roughly 4 hours, I was very dizzy, tired, and a bit nauseous. It wore off after that, and I did not experience those effects today. For me, it has been great so far. It is like a switch flipped overnight. Before treatment, I was getting distracted 20-50 times per hour and getting up from my desk 5-10 times per hour, I am getting distracted maybe 2-10 times per hour and getting up 0-2 times per hour for water or bathroom. My mood went from a persistent sense of existential dread to one of hope and relative happiness. I expect this will normalize, but it is welcome right now. Depending on how this month goes, I may stick to Strattera. Stimulants are in short supply anyway, so I am happy to find a readily accessible alternative. This is my experience, and yours may differ widely. I'd recommend discussing this with your care provider.

I'm so incredibly grateful for this medication. Once I reached a stable dose at 60mg, I've noticed a massive increase in my ability to retain and retrieve information and have dropped my adderall xr from 45mg to 25mg/day. Adderall allows me to get started and stay more focused on tasks, but Strattera keeps my mind from floating away midstream. If I couldn't stay on the combo, I'd choose Strattera hands down. I like getting stuff done, but Atomoxetine genuinely feels like the antidote to early cognitive decline. Tip: take it 15 minutes after eating something with protein & fat to avoid the nausea and headaches. No other symptoms for me except feeling happier due to decreased dementia fear.

I’m glad it worked for you with few side effects. I’ve been taking it for a month and I feel tired all the time and hot. My psych started me on this because she said stimulants are addictive and it would be better but I really don’t like the way it affects me. I just feel so dull compared to before

I have the liver cyp2d or whatever it was. But I am older than 65. I was on stimulants of several types since 1989, on and off from moves, since new doctors don't want to prescribe them. I usually took the lowest dose, or would forget a midday dose and not want to take it later for fear it would keep me awake several hours later. I don't know if strattera is good for people over 65. Allergic to clonidine patch, it worked wonders for my OCD and anxiety. Less anxiety means more able to concentrate. Oh, well. I am still working in a fast paced work environment.

I took this for a year and it tightened up my joints and ligaments and I had small sores and swelling everywhere-- I am still trying to recover--- I won't EVER take this EVER again.

Thank you for your video. My 12 year old just started today at 25 and he threw up after about a hour and a half after taking it. Poor guy hoping tomorrow is better. 🤞

@gordocarbo His primary dr who has been his Dr since hw was born. He was diagnosed over 6 years ago. My son decided he wanted to try medicine again.

@@seannakellystrattera is not a good medication at least for me. I have too many side effects including nausea like your son. I just got diagnosed at 18 and i don’t know about taking meds anymore. I just started this month. Did your son’s side effects get better?

my dr. just started me on it today, but she only gave me a 10mg dose. I am 5'9 and 277lbs.(fat boy now) This does seems very low compared to what you are saying. I have heart issues so I am very concerned about the dizziness, increased blood pressure and increased heart rate that may happen. My resting heart rate is around 90 while sleeping. Cardiologist did clear me for strenuous exercise. I have had heart issues since i was 17 and have a very big anxiety towards medicine that might affect my heart as i am one of those that is constantly aware of my heart. I have been self medicating with beer in order to calm down and sleep at night for the last several years and am concerned also about the beer, drug interaction. I tried to talk to my dr about it and she literally said nothing and told me nothing about this med and just left the room. I have been seeing her as a PC for 5 years and she was going to give a stimulant until i asked about my heart. she looked at me all puzzled and said what about your heart and had to look it up. (just took off the halter monitor a couple of weeks ago and have a cardiologist that she referred me too) so i really distrust her judgment on the meds.

The paper I got with my strattera says that it can cause "sudden death" in patients with heart problems 👀 definitely get a new doctor

Strattera is what they started me on after being diagnosed as an adult. It worked well but had horrible side effects for me. Constant cold sweats, twitching, insomnia issues. I couldn't even tolerate what was the minimum dose at the time of 18 mg. I had to split the capsules into the same size empty ones to make my own dose. Fortunately I was able to get on methylphenidate after that.

If a medication such as strattera causes sexual side effects, those side effects should resolve shortly after discontinuing. Sometimes these types of side effects can affect someone's psyche (for example, cause them to be nervous about sex, and in-the-moment this could affect performance) and therapy may be helpful in these cases. However, in general these medications should not permanently alter someone's physical abilities.

My doctor has me on Bupropion XL 150mg, Atomoxetine 40mg and Escitalopram 10mg. I have lost appetite, ED, sweating and my heart rate seems high.

UPDATE: I had to stop at day 4 of use. I felt real spaced out in my head and like I was floating. That only lasted for about 2 hours while it was the strongest but it's not worth it. Today was the strongest effect. Yesterday wasn't as bad. Either way it didn't do much for my ADHD. Just trying this now. On Vyvanse it helped but screwed up my sleep I could only get 6hrs of sleep before I would wake up. My filter was also gone and for a change would actually get snappy with people (including my boss) which was unusual. Hopefully this works. Day one at 40mg I feel a head buzz like weed and a bit calm like a bit of weed. Hopefully this works. Btw I don't like weed because of my lack of motivation and emotion blunting so hopefully I don't get that with this. The SSRI part freaks me out. My mood is just fine with needing any emotional meds.

Well when I started sertraline I was in a state the 1st 2 weeks, but after it was like a switch flipped. You really gt let these medications balance out before knowing what the long term effects are. Saying this as a person who is also extremely sensitive to medicine.

Wellbutrin (Bupropion/Zyban) also treats ADHD, depression and quit smoking and it is more effective and also not controlled. Also it increases 2 neurotransmitters that help not just one.

I was on a Wellbutrin for 3 months. It had zero effect on either my ADHD or mild depression. So I was recently switched to Strattera (atomoxetine) Already it seems to be more effective. So it definitely depends on the person. With Wellbutrin, it seemed like I was taking nothing.

I almost got crazy with Wellbutrin. Never felt such a rage. I wanted to hurt everyone who would bother me. Also I felt no fear of nothing nor anyone. Had to stop at my second month.

I was on Wellbutrin for almost 1 year. It made me worse and gave me an anger problem on top of the original problem (ADHD). I finally just got weaned off of it!

Started on 40mg (low starting adult dose) ended up with a heart rate 20-30 beats faster and chest pain and tightness. Went down to 10mg, after ~3 weeks ended up with acute liver injury *possibly* in part due to this medication.

It's funny because I also have bipolar disorder and this stuff triggered manic symptoms but I had no issues from stimulant meds.

Utilizing medications like strattera (and stimulants as well) in bipolar disorder can be complicated, and it's very important to be working closely with a mental health professional. Some individuals with bipolar disorder may be more likely to have their mood destabilized with a medication like strattera, as it builds up in their system (and stimulants generally are mostly out of someone's system by the end of the day, and over-night). However, everyone is different (some patients do become manic with stimulants), and that's why supervision by a medical provider is so important. Thank you again! - Dr. Pagnani

I'm also bipolar and am supposed to start atomoxitine. Stimulants always had bad side effects with me. Especially medikinet (ritalin).. vyvanse gave me horrible acne and stopped working for me at some point and i felt like i was having horrible withdrawal symptoms when i stopped taking them, even though some swear that you can't have any from vyvanse. But when they did work i felt manic lol. I'm excited to see what atomoxetine does for me. Alot of people said it is great for executive dysfunction which is my biggest problem

My blood pressure increased a lot on just 60 mg. I have high blood pressure that is controlled with medication and usually is around 110-120/60-70. With strattera at 60 mg, it's 140/80 on average. My heart rate resting before strattera was around 60 bpm. Now it's 80-85 when I'm most relaxed. That's concerning. It's bs that I had to wait 5 months to get to this dose only to have to either reduce the dose or come off... and this was all because the practitioner didn't want to put me on a stimulant because of my blood pressure... only to put me on a medicine that messes up my blood pressure even more.

The side effects are not worth it. Nausea, vertigo, erectile dysfunction, cold chills. Stimulants work much better with no side effects for me.

I took it for one day, and it was a disaster. It put me under and made my fatigue a dozen times worse. It caused me to have a horrible, eternal day up at work. How this sedative gives anyone concentration and “energy” …🤣🤣🤣 is a mystery to me. And this horse**** about “You have to keep taking it, then in 13 years, you’ll begin to get the benefit” is beyond insultingly ludicrous.

4-6 weeks isnt 10 years. It's common with drugs of this class and antidepressants. Really all drugs take time to see at least a decrease in side effects

What ive read about Strattera, SANE normal people will find its side effects to be hell. And the others accept the side effects like its normal to feel like shit, just to be able to focus abit?(Most of these people seems to be prescribed atleast another antidepressent or adhd medicine at the same time, this might be hiding the severe damage Strattera is slowly causing your brain and body, this is just speculation ofc, but it seems to be a common occurence of "good effect" and "multiple prescriptions", whilst single strattera perscriptions seem to be more common with the people who almost died or felt like dying on strattera). Strattera seems to have a risk of causing liver damage and also looks like there is research showing it being neurotoxic(high doses, but still concerning considering its severe side effects to the brain and mind) Id stick to Amphetamines, atleast we know that they arnt that bad. *Disclaimer im not a doctor or a pharmaceudical professional. So i wont try to sell you these cancerpills.

I find the non stimulant set fascinating. So. Straterra raises blood pressure as a side effect. Guanfacine on the other hand has the side effect of lowering blood pressure . Stimulant meds only have side effects for patients with prexisting or severe cardiac problems. Because of risk of heart attack. Am i being too simplistic with this theory but would having regular blood oressure checks just be something that goes with having adukt adhd reguardless of what medication your on ? Would heart health be a primary considwrstion / leveling factor in the ongoing treatment of medicated patients ? Also. Should young adults be automatically prescribed stimulants even if they recreational use weed ? Considering its the lowest risk to there oversll heart health ? .

Thank you for your comment. Just like any psychiatric medication, some patients do have significant side effects. I've had patients not tolerate this medication (or not have positive effect) and also some patients tolerate and do very well with it. @@RittenhousePsych I started taking for about a 3 weeks, I feel some positive effects, but the most notable and annoying side effect is urine retention, I'm not sure if I should drop it altogether.

for me it's been great, the only side effects I get are dry lips and sleepiness. those don't show up always. at thr beginning taking it gave me stomach burn but 3 months later it stopped

Yeah this Strattera ruined my life and it’s been years since I. Took it and still can’t sleep and having other health problems. Feels like I got set up I don’t trust meds
