patient 55 years of age presented with right sudden painless loss of vision , on examination he is suffering from central retinal artery occlusion, he is hypertensive and has shortness of breath, he takes brakes in just 1 km walk, hard to climb on stairs, i advised carotid dopplers, HBA1C, fasting blood sugar, CBC with ESR.
IF more than 65-70% of artery is blocked, then i will refer him to intervention especially to vascular surgeon, they do endarterectomy, which has its own merits and demerits, otherwise they will be put on antiplatelet therapy.
