Is studying clinical medicine starting from a presenting complaint, building a diagnostic framework with relevant illness scripts and then going on to diagnostic workup and therapeutic reasoning the best way? Is it the only way? Is any one way better than any other way?

Well, that is not a very easy question to answer. Based on my understanding, diagnostic experts rely heavily on frameworks (for example, diagnostic schema) and robust illness scripts. I personally don't think there is a "right way" or "best way" to learn diagnostic reasoning or to practice diagnosis every day, simply because every clinician is an individual with different cognitive strengths and tendencies. I think it is best to learn a number of approaches to diagnosis and to understand the cognitive processes that underpin diagnosis to maximize my own ability to reflect on my diagnostic reasoning. @@sisyphushappy5200 I think there is a mixture of pathways to achieve excellence in clinical medicine. There are a broad set of skills which are well elaborated in any contemporary set of milestones, competencies, etc for medical trainees. If you are specifically looking at diagnostic reasoning, a symptom-based approach is an excellent starting point. However, experts have extensive knowledge structures that probably do not rely on symptom-based schema alone if I had to guess.
