Even if the degree of pulmonary stenosis is mild, wouldn’t there still be a right to left shunt? How would systemic resistance be higher than pulmonary resistance if there is even a mild obstruction to the right ventricular outflow tract?

My son got Tof.absent pulmonary valve.pda.and he is six years old now.we got to know about tof when he was sixteen day old.but has no semtem like blue in skin colour.his Wight is 14 kg now.we don't understand what to do.we go for surgery or not.please help us.
