Im pretty sure a ragdoll only spawns in T-Pose when the animations are blending, or the program responsible for tracking the model for ragdolls is confused. For example: If you're a disguised spy, the ragdoll program tracks your disguise rather than your spy, so it's going to consistently believe that you are infact a Scout up until the moment you die, in which it'll suddenly swap you back. But the program won't know that you weren't a scout, so now it has to make you ragdoll as a spy, and since it had no reference of your spy since the model was a scout just now, it just puts you in the default pose.
