I think the main issue keeping the scenes from being more realistic is how clean and new the clothes, set pieces and everything else looks. If for example sanitars jacket was more worn and dirty he would fit into the setting a lot better

Did anyone notice at the start of the gunfight, both a BEAR guy with USEC? I am I missing something? Look closely from 16:41 , there is an unidentified shotgun dude (probably USEC, behind him a BEAR guy wearing black clothes and a third guy wearing a USEC cap). Undoubtedly interesting.
