Wow.... Simple implant case can go wrong this much. However the lady is lucky to be taken care by you. Very interesting case and excellent presentations. Thank you for sharing great case with us.

What were these strings on the left and on the right of her bottom jaw? You🫵🏼 Doctor Bolten are truly the best Oral Surgeon out there! I am having 3 implants placed next week. 2 molars in my upper jaw and one in my lower jaw right below the other implants. I will make sure to ask the Surgeon not to make them too long! LoL….😳😂 Oh yeah, and like you mentioned before in another video to be gentle on the soft tissue!🫶🏼

So is this pretty common Cause I was thinking about getting some implants. Cause I don't have no upper or lower on the right side and also on my Left side not so much question Where are you located at the office

This is a very uncommon scenario. The vast majority of implants have no complications and patients are very happy. This is an extreme example, which is why it is a good teaching case. I practice outside of Detroit, MI. Thanks for watching!

Unfortunately I can't speak about any litigation as a result of this case. It is a very sad scenario and I felt terrible for this patient, but thankfully at the end of treatment she was happy with the result

What would be the cause of osteomyelitis in this patient?

Hi doc, thanks for the question! The osteomyelitis was a direct result of the implants and their associated infection. She is a reasonably healthy individual given her age and is not immunocompromised. This is not the first case I've seen where infected implants resulted in osteomyelitis, it can happen unfortunately.

The wires in the ramus I think there are for orthognatic surgery for ivro done way in the past and maybe if the patient done orthognatic in the past the vascularity of mandible maybe compromise due to striping of master during ivro

4:14 doc was the same antibiotic continued orally after iv antibiotics or u changed the antibiotic?

Thanks for the question! The antibiotic was changed after the IV infusions were done to oral Amoxicillin, which she was on for months. This portion of her care was managed by infectious disease specialists. Thanks for watching

Any consideration of nerve lateralization?

Definitely a good thought! I did consider it, but there was a large vertical defect I wanted to correct for ideal implant placement. There would have been enough bone if I had lateralized the nerve, but the implants would have been too low in the arch and very difficult to restore . The crowns would have been excessively tall and a nightmare to maintain hygienically

Nerve lateralization would not be my first option here. Khoury method augmentation may be, or hip graft as we see here. Actually i have never performed nerve lateralization, but I may have to in future. So what is your nerve lateralization protocol?

Maybe the initial dentist did their best but made a mistake in planning or execution of the case. Perhaps all were unlucky despite protocols being followed. There was no radiograph in this video showing the depth of the prior implant placements from which we could judge that they were placed too deep and risking nerve damage. These complications are uncommon but known risks of such a procedure for which patients are normally consented.

@@keithratcliff7896 All great points...we can follow every protocol and adhere to the highest standard of care and still have complications. Obviously I wasn't at the original surgery, so I have no idea how it went or what the circumstances were. Thanks for watching!

I've seen those little loops at the angle of the mandible before... what are they from? A non resorbable suture or metal tie? And why on both sides if she had osteomyelitis only on the left? Is it totally unrelated?

Third molar extractions I assume... but what are those loops for?

Great question! The loops are from previous orthognathic surgery. We use plates and screws now to stabilize bone segments, but back in the day they used stainless steel wires to reduce fractures and hold bone segments in position. This surgery was done decades ago and the wires are unrelated to the osteomyelitis, just a coincidence. Thanks for watching!
