On map selection, have you tried to scroll all the way down and then up again ? It should come out that way.

You have to try playing both the inverse and Endless versions of any level at least once. The stage will then show up above Mad Forest on the stage selection screen

You have to enter and exit any map with inverse mode and eneless mode enabled one time each to get it to spawn.

You have to enter and quit any other map for it to reappear at the top of the list

The final boss of this game acts like a godlike creature as the owner of the reality you are at. It toys with you until it either gets satisfice with the entertainment you give to it or proves you are worthy of the powers you have. In any case, it warps reality itself so you can (hopefully) find a vampire to defeat. Also, this is a way for the devs to thank you for completing the game.

The Director is a stand-in for the game developer. The first time you visit this stage, it asks you if the game is Too Easy (and will unlock hard mode for you) or Too Hard (and will unlock endless mode). The second time, it gives you the other option you didn't pick. The third time, you get this fight; clearly, hard mode and endless mode weren't enough for you, you've decided to challenge the dev itself. So it just throws a bunch of wild enemies and particle effects at you, making you feel like you're fighting for your life. Eventually at minute 7 you get the White Hand reaper which normally autokills you. However, this time you autorevive instead, and you're warped into a celestial space, autocollecting thousands of gems and gold and items and upgrades, and you get to watch a lot of numbers go up. This is the ultimate reward for the ultimate fight. Incidentally, the stage is called Eudaimonia, a Greek word that roughly translates to "flourishing" or "well-being". Vampire Survivors is designed around a lot of addiction-inducing features, like incremental progress, killing thousands of mobs, regular frustration, and watching numbers go up real fast. The last minute in the Eudaimonia fight is this effect boiled down to its essence. The developer is basically saying, "to whatever extent this game can bring you to eudaimonia, this is the best you'll ever get from it. You've hit jackpot, the lights are flashing and all your numbers are going up and up. But you can't go up from here. If you want to find a vampire, let's keep playing." And it gives you a collectible, signifying that the game is far from over even though you've beaten it.
