Helping treat a family makes Carol finally realize she needs to be with Doug, so she rushes to the first flight to Seattle to reunite with him.

First broadcast May 11th, 2000
This scene features a surprise, uncredited cameo from George Clooney as Doug Ross. He agreed to return at scale pay (minimum wage) as long as his return was not used to publicize the episode. As a result, the final scene where Carol and Doug see each other was not originally part of the episode, and was added on at the very last minute so that NBC didn't have time to realize what was happening before the episode aired.

Doug’s reaction when seeing Carol was everything. Especially when he was on the dock. He saw her and smiled. So powerful. It was a scene that needed no words, to convey the love they hold for each other.

I never, EVER tire of seeing this scene! It speaks VOLUMES about the character of George Clooney, and his friendship with Julianna that he did this as a favor. Considering he wanted no publicity of his return, and asked only for the lowest payment allowed...what a class act. At the time he'd made it to stardom in films, but he's still known for being there for his friends. This to me is a tv history moment. I still recall the reaction the next day from everyone I met talking about that moment. When it aired you could almost hear the country go nuts with surprise and joy! Almost like when Ziva was learned to be alive on NCIS.

@@dangurney1042 true. But while watching it, nobody knew that. Luca certainly didn't know that. As with real life we can be sad/heart broken/devastated by a breakup but because of that breakup we're able to start a relationship with the person we actually end up with.

I remember watching this live and crying like a baby. Such a classy move for George Clooney to come back and bookend their relationship to coincide with Julianna leaving the show. He had zero obligation to did that. He never forgot that ER made him the global superstar he became. And the fact that they both came back in the series finale shows the regard they both had for that show in the trajectory of their careers. Just a class act. Doug & Carol forever!!! ❤

He even made sure that there would be zero promotion of his return prior to this episode so that he wouldn’t steal Julianna’s thunder, since it was HER final episode after all. Both of Doug and Carol returning one last time in the final season as husband and wife was the cherry on top 😊.

I remember when this episode aired...my wife was a HUGE fan of Doug and Carol as a couple; she had fallen asleep midway through the episode. When Carol announced she was going to Seattle, I woke her up - "I think you're going to want to see this..." She got teary-eyed when they showed Clooney.

I sat here and watched it on Pop T.V. and I've seen this whole series a couple of times so I knew what to expect in this episode, but watching it again teared up my eyes and put an "awwww" smile on my face AGAIN !! It's so romantic. And gives hope to that question of ... If there is one soulmate out there for everyone. One of my top 3 E.R. scenes !!! ❤️

@@lovewhitey2027 Doug Ross resigned from Chicago County Hospital went to Seattle and settled there and worked as doctor in Univ of Washington Medical Center. He resigned after a scandal involving showing a mother how to give lethal dose of an opioid drug to her terminally ill son by bypassing the lock mechanism of a drug administrating machine thereby helping her euthanize him. He is the father of Carol's twin daughters. In reality he left the show to focus on his movie career. He came back for that 1 scene to help conclude the storyline of Carol and Doug and help writing off Carol from the show because Julianna Marguilles was also leaving the show after Season 6.

This was one of those moments that you think you will never get, that because of contracts or feuds or other outside silliness it is too late, that they think we've forgotten, or gotten over wanting what we always wanted, and we haven't, and even as she gives Luka (who at the time I dismissed as a third rate 'Dr. Ross' wannabe) that wonderful speech we still can't quite dare hope that we are finally going to get what we want, even while she's flying towards him and then gets to his home and he doesn't answer the door and then she goes down to the dock and he's there and it might e perfect, but we still don't dare believe - she's too late, he's married, he found someone else... and then you know it is real, it has happened, and everything else that looked like it might get in the way hasn't. And it's real. And it's wonderful.

Monrovia1 yep. NBC was under the impression that’s where the episode ended. They didn’t know that the producers filmed this scene with a small crew and kept it a surprise even from NBC. They found out with the rest of us lol.

That was honestly the smartest thing they did - to keep it to a minimum of people so that it didn't get leaked to press so that fans for once got a true surprise. These things always end up getting leaked. George Clooney is a class act - and he understood the importance of closing out Doug and Carol's storyline. I love that we get a small glimpse in season 15 of how they are doing too.

Nowadays you root for a tv couple, they eventually get together, then one half decides to leave the show and now they just kill them off 😢 no hope for a sweet reunion like this one, it gets kinda depressing when this isn’t even a possibility!! Thank you ER, George & Julianna for giving us one of the best on screen reunions of all time!! 💜💜💜

SO well done. Especially the tease of Doug at the end. You see a figure in the distance but you can't quite tell who it is. He starts walking towards Carol, and as soon as you say, "...Wait, is that really George Clooney?", then cut to a closeup of Carol to draw out the suspense even further. Masterfully done.

Trust me, she did Luka a favor by being honest with him. No one should hold false hopes about the one who got away. It's kinder in the end to hear this and get closure than to always keep wondering why someone you loved didn't chose you in the end or love you back the same way you loved them. 💞 She was fair to Luka by letting him know the truth of her heart.

You have no idea! The director kept the film in his refrigerator at home and it was only added to the end of the episode after the beginning of the show was already on the air! NBC was pissed!

I remember this episode and seeing Clooney there was a huge shock. I wish we had telephone usage numbers after this ending because I bet the switchboards were blowing up because no one saw this coming! This kinda stuff will never happen again! ER was primetime back then aka a huge friggin deal!

I remember reading an article how NBC was PISSED that the producers kept his return a secret. They flew out to Seattle where he was filming ‘The Perfect Storm’ and shot it with a small crew. Had NBC known, they would’ve promoted the hell out of his appearance...

The Perfect Storm was not filmed in Seattle. It was shot on location in Gloucester Mass. It had also been completed long before this episode was ever filmed.

George Clooney is such a handsome looking man. And Julianna is beautiful. 2 fitting characters that made ER worth watching. Im binging it now and she is in labor. Luka carrying her and falling right outside the doors has me in tears. I came here for this reunion. This was a really great show!!!

Completely crushed by my first love when this first aired. I used to rewind and envision it was reuniting some day. This was the pinacle of the show. Went down with all the dumb deaths etc.

They don't know me, but they did this one scene and it made me happy. And now I can watch it whenever I want, I think this is over 300 times. Yep I love this happy ending, and when they are shown again when the 16 year old boy is deceased and his Grandma wants a test to prove he is really gone, his organs are being donated and Staff from Chicago Cook County Hospital are there and we get to see our Doug and Carol one again.

no one knew George was coming back...it was a secret the executives at NBC had no clue till it it aired. The director left the scene out when the exec's watched it and then they added the last scene! Brilliant absolutely love!!!

I have often wondered how they did this. In the edit for a TV show the programme has to be a certain length according to the channel specifications. So unless they added in some of the scenes which then maybe got cut I don’t know how they managed to get it past the executives as they would surely have queried the show being too short

This scene is so beautiful and it always makes me emotional. They were the main reason why i kept watching the first seasons, not gonna lie. I love the show for what it is but they were my favorite thing about the beggining of it, especially their development in season 3. Didn't like a lot of choices in regards to them later on but i was glad they had their happy ending, they deserved it.

ER was my favourite Thursday night show on NBC. I remember watching this & I was so excited to See George!! Even though it was only like 3 minutes or whatever!!! They were both my favourite Characters too!! Oh my George, he is so handsome! I remember after this aired I was so floored to see him!!! I think I also remember reading or something like that it was Shot in Seattle. That they used like 1 camera guy & the crew was small. They wanted it to be a surprise. Also I loved that George came back for this, it really is the best ending for them!! I love this!

One of my favorite tv romances of all time. This scene still gives me chills all over. Hard to believe this was filmed 25 years ago. Timeless. I believe that is Lake Washington. It really is beautiful here.
