In May 2011, the Northern Territory Coroner, Greg Cavanagh, was forced to officially close Mount Bundy Cattle Station due to a death attributed to arsenic poisoning. What ensued was a series of events that brought into question the validity of two government investigations across two state borders that nearly destroyed a family and crippled their business.

These doctors are everywhere in our health system, no matter where you live. Most seem to exist only to bill for their "services".

That’s what’s lacking in medicine. Doctors aren’t doing physical examinations anymore. It’s ridiculous.

I agree. This doctor is too lacks. All he did during the inquiry was to minimize his part and offer excuses. I'm disappointed in this Coroner. That doctor messed up badly, and if you listened to him closely, he practices medicine in a very cavalier way. He has no business being Senior Staff. He just doesn't come off as being someone who really wanted to become a doctor in the first place. He is a danger to his patients and hopefully the hospital holds him accountable with another doctor supervising him because he obviously can't be trusted to do a thorough job of anything; not even simple examinations.

I don't know if that doctor was practicing the correct medical procedure. But his words certainly wouldn't fill anyone with confidence. The overwhelming majority of people would surely be hoping for someone else.

My husband went to hospital with severe stomach pain and was sweating hugely in a cordoned off bed…for four days in dreadful pain …they were treating him for poisoning as they could see that I was twenty years younger than him and the medics made an assumption.….he actually had peritonitis…and I am pleased to say that he lived through it.

My daughter suffered from a gastric tear which was quickly diagnosed and surgically repaired. The surgery saved her. I am so sorry that Ben was not properly diagnosed. It is a real tragedy and a travesty that should never have happened. I certainly hope the doctors, especially the senior doctor, and hospital have learned and changed how they approach these situations. I think there should have been a recommendation for mandatory second opinions from a senior doctor for all situations where a patient is not improving.

I had a Mallory-Weiss tear and almost bled to death before I got to the hospital via ambulance…I cannot imagine the suffering of this young man and his family who are left behind.

Most physicians call in for a consult. They talk amongst themselves. But a Second Opinion is Always suggested. And if you don't like your doctor, fire them. I have fired Doctors while being a patient in the hospital

I'd like to tell Mr Cavanagh that a lot of people look at grieving parents and say to themselves "I don't know how they're managing to even drive to the shops because I'd be a mess." I said that too, more than once. And then I found out personally when my son died. And you realise that you have no choice. You DO go on. You HAVE to do things. Falling apart is not an option.

I found that too, when my son died. Your child’s life may have ended, but the world goes on, and so you have to too. Somehow. It’s so hard to even get yourself out of bed, to just take a breath. Your brain doesn’t want to think about anything other than the fact that your child is gone, and they’re not coming back. Ever. I had to learn to lean on others, and accept offers of help. 7 years later, and life is easier, and i’m functioning. And he is still gone.

Second story 7 years later: my husband wasn't feeling good for a month, i finally convinced him to go to U.C., we demanded a CT scan, we knew the drill by that point, if insurance pays 100%, doctors will order ct. My husband ended up having an emergency surgery. He had diverticulitis, his intestine had a hile, they removed that portion, he had a colostomy bag. He did have some complications, they put him in an induced coma. Finally after 52 days he made it home.❤

My mum had diverticulitis and lost some of her intestines. I glad to hear your husband is recovered. She couldn't eat certain things, like nuts and seeds that could get trapped in her intestine folds.

Clarification is needed about your statement "if insurance pays 100%, doctors will order a ct." Is this private or public or America??? I've worked in public health over it 25 years and never heard this before.

@mikalabaker6110 I hope your mom is well. They don't know what it causes diverticulitis, they just suspect that might be seeds from different foods. My husband was never on a diet.

@viliabone1952 when they see that a patient has 100% coverage on their medical insurance, the doctors order a bunch of exams, they know they're going to get paid 100%. Our son was rushed to the ER by an ambulance, his throat was closed, they said croup (it wasn't),but they continued treating him like he had croup. Camera down his throat, and other different exams. The doctor decided to keep over night for observation. Another boy was there with the exact same symptoms, gave him some inhaler treatments, he sounded much worse than my son, but they sent him home. He didn't have insurance.

The coroner’s report still said Ben had advanced leukaemia. I thought that had been ruled out. I’m just so sorry for the agony he went through as a result. Heart felt prayers for the family.

I listened several times bc that seemed odd, and couldnt decide if maybe it was a weird aussi pronunciation of ischemia (organs deprived of oxygen????) A stretch, but leukemia makes less sense since they ruled that out???

There's a reason for a 'mother's gut instinct' and she was right. Although there was many contributing factors, the gastrointestinal tear was the major cause of Ben's death from what it seems in this video and the specialist who didn't even go and do a physical examination should be held accountable for the lack or neglect to do his job in the correct way and timing of the diagnosis, shame on him!!! Ben still may never have survived but that's no excuse for any medical neglect!!

An excellent production. I simply cannot imagine sitting at anyone’s bedside who has a Level 9 pain reading and have the leading physician not order at very least a scan. This family are heroic.

The mis diagnosis may not have led to Ben's death but it certainly did not allow comfort measures that could have been in place...poor family going through all that on top of their Son's death.

Leaving anyone, let alone a teenager in 9/10 abdominal pain for 17 hours without appropriate levels of analgesia, a thorough physical examination and an urgent CT Scan, is in my opinion, inexcusable. If I was the Coroner, I would have found the Physician at fault. Ben may have still died, I agree, but the medical and nursing care that night was appalling. All that doctor had to do was put his hands on Bens abdomen and he have instantly known he needed to get an urgent CT. Nursing staff should have been agitating and escalating the unresolved pain and worsening clinical condition. If the treating doctor appears to be not providing the appropriate level of care, the nurses can escalate to After-Hours Med Super, DON or Executive on call. Acute unrelieved pain, is a medical emergency and should be treated as such. I am so sorry Ben and his family had to go through this.

Can a person be in excruciating pain while nurses are documenting AND reporting it…all the while nothing else being done? Why the absence of action? Can anyone explain that to me? I am a 40+ year retired nurse in America..If I had encountered this set of circumstances, I would have IMMEDIATELY raised alarm to his doctor to delve further into finding the cause!!! For a nurse…finding the cause most likely will find the cure!!!!!

He made NO recommendations?? How about recommending all patients not improving are given a full body scan?? This poor family. What a horrific loss for them. Not to mention the damage it did to their mental health and lively hood... 😢

The thing with most scans Medicare doesn’t cover or only covers a small portion & they are so expensive it’s ridiculous most people couldn’t afford it. His mental health should have been given priority.

@@The_Crusty_Old_Hag_Next_Door this isn't true. Medicare 100% covers scans for diagnosing and treating emergency or illness. Scans for non-urgent things like torn ligaments - it is up to the clinic how much you pay. Some bulk bill, some you have to pay a portion and some you pay in full.

Prayers & Condolences for Bens family, even now, 10+yrs later, their pain must be very raw 🕯️🙏🏻😢💕 That Doc on the stand didnt get it. He was being asked if the high amount of pain over such a long period was a symptom of abdominal tearing. He said, "yes, but also could be other abdominal pathology". The issue that Doc wasnt getting was that he DID NOTHING. He didnt do any other abdominal pathology, so how could he know if it was a tear or otherwise. Sometimes the most intelligent people can also be the dumbest.

How frustrating, highly stressful it must have been for the parents and family to watch their son in such pain without any attempt being made to find the source of it. The delay in finding and treating the tear allowed more internal damage, which I would have to struggle to forgive.

Mum even early on suggested to Drs that Ben could have a tear gastrointestinally or the lining of his tummy. Mum suggested it to the Doctor. Sad for Ben and his family, friends and loved ones. Bless you all.

The Coroner also needs to be held accountable.....he attempted to appear "average" but he clearly failed in requesting further evidence about arsenic levels.....why didn't he ask the local GP if he was qualified to read pathology reports etc.....and to make no recommendations in his final findings.....how distressing for the family. That poor family....I hope they are doing ok now.....if that's even possible.

This was a fascinating view into the work of the NT coroner. Magnificent landscape and a glimpse into life in the north of this country. Like everything up north, the grand scale of tragedy in this story of the Witham’s beautiful son and brother Ben is representative of the massive pain and loss. Nothing in the NT is small 💛

What about the other station that Ben went to work on the day he came home ill? Was it investigated for arsenic? I am shocked that the neighbouring station was never mentioned again.

I suffered a bile leak after removal of my gallbladder, i assure you that was pure agony (made childbirth a doddle), i can not imagine the excessive agony Ben suffered, those doctors are responsible no ifs but's or maybe's. I can understand the reservations of Ben's parents not wanting a autopsy My heartfelt condolences to the loving parents of Ben. Rip Ben ❤

The coroner could have laid a heavier hand on the doctors who bungled this case. Shame on all of them. I hope a lesson has been learned for the better but I fear they got away with neglect 😢

Agreed. Coroner took the easy route, the tuxedo medic was completely negligent. There was negligence with the arsenic, and I thought that leukaemia was in question. The coroner should have ruled negligence and the family sue the authorities and sued the dr out of his tuxedo. A very shocking report and my condolences to the poor family who endured this malfeance.

Apparrently he got ill the day after working at a neighbor's cow yard (did I get that correct?). If so, an investigation of what happened there would be needed...

I watched the whole video and I had tears I could not believe how many doctors were looking after him but I think the fact that his gastric tare was not discovered sooner it explained a lot to me why he was in so much agony and pain with further complications such a beautiful handsome boy I wish he could have survived. To lose a child at such an age is just unfathomable and I wish to express my heartfelt sympathy to the parents and extended family. God bless all of you and keep you safe. I had tears as well. Very emotional but we'll produced documentary. I wasn't impressed by the "tuxedo doctor"😮 I like the coroner / judge. Down to earth family man with a conscience & heart. Tragic death of fine young man.♥️🌷

@@brendarigotti843 Maybe the kid died later on form something different, but that Doctor's call, or lack of, is exactly what caused his death. Also the steroids that caused the tear in the first place. He lacked a backbone to call any of it out, couldn't even lock up criminals beating their wives

We are conditioned to trust the medical experts & this is terrifying! The family literally watching their son endure excruciating pain & all they can do is watch in horror & cling to hope !! Ben deserved better than that!!😕

Devastating way to lose your child 💔. We had a very good health insurance, 100% coverage, so when i got sick the urgent care doctor told me that i should better go to my insurance urgent care building in the morning, because they can do CT scan, you ask for it. Next morning i did ask the UC doctor for a CT scan, although he saud the don't do CT scans for sore throats. Long story short, I rushed to the hospital by ambulance. I was 2½ days in ICU. Strep throat infection, tonsillitis and epiglotitis. My oxygen intake was only 5%. The doctor at the urgent care after the CT scan results came out and said, "I'm so glad I listen to my patients".

My hubby nearly died because they and I quote Didn’t want to expose him to the radiation. My bottom. This was in Hobart, about 5 years ago he had an appendicitis, they waited so long it burst and gave him septacemia, I didn’t even get an apology. I had to insist on the cat scan on Christmas Eve he was taken in To surgery and operated on. It was a real mess they said. 14 days later he came home about 20 kg lighter, and had a scar from top to bottom of his abdomen. All because they wouldn’t Believe me.

I pity anyone that ends up in a hospital these days cos they are understaffed and don't care anymore. Ben may have died anyway but he did not get the care he should have. I'm only watching a video but I cried for him and his parents and sisters.

Thank you for sharing this, it was such an interesting video. Firstly, my condolences to Bens family and friends from NZ. That saying of no parent should ever have to bury their child is very true. The toll can be seen on all of them. My take on it was that Bens symptoms were treated, except for the one instance, but not the initial cause. I'd be interested to know if the ranch Ben worked on for the day was ever investigated and looked at as the potential source. It wasnt never mentioned at all other than by his mum stating he became immediately sick after working there. Sending Bens parents and family light and love!💜

I'm extremely lucky I have medical professionals in my family who don't take no for an answer when I and family members have been in hospital. And I've picked up lots of medical information. I'm pretty knowledgeable about illnesses and treatments because of said family members

Im a grieving mother of two beautiful children myself ( My 19 yr old biological youngest son ) 💚😇😭 ( & My kids youngest brother 8 ,same dad ) both 2011 four months apart 🩵😇😭 💔💔I think that doctor is definitely partly responsible for Ben's death and should have been held accountable !! I can totally understand his family's anger and frustration, Condolences 🙏😇💔

Our Condolences to Ben’s Family What I observed out of watching this is that sometimes it pays to make extraordinary waves and demand action We have all been to various hospitals and had excellent results,but at the same time could observe some major failures that can happen in the system

That tuxedo doctor may as well have stayed at the function he was at for all the good he did to this poor boy in absolute AGONY. SHAME ON YOU DR.Can you live with your guilt? I wouldn't be able too. 😢😢😢

Sad story great Coroner very compassionate as the illness and stomach pains had gone on for such a long period cannot believe they were so late with C T scan I would call that negligent .

This family is incredibly strong. I cried for them and for Ben. It hurts my heart that they have lost this amazing boy who was obviously on his way to be an amazing man. They will be in my heart. I've had to deal with horrible illnesses with one of my daughters and the hurt never goes away. Thankfully shes still with us, but to lose a child is unbearable. Im so sorry❤. My heart and prayers go to ben and his family. This is one that will stick with me. The strength they showed is so admirable. You're amazing parents and you have taught your children to be amazing too. Good on you. Love from UT USA ❤❤❤

What a purpose-driven life this coroner/magistrate lives/has lived. I suppose if one wants to feel genuinely connected to humanity, his is an excellent choice. This was my first episode and I'm hooked. Thanks, Mr. Cavanagh!

I live in the US and this kind of thing happens more that people are aware of. I can't even begin to imagine the pain that Ben's family have and will experience. The inquest seemed to answer some of their questions but also, in my opinion, pointed out how the medical establishment let them down. I hope that those doctors take this as a learning experience going forward and treat the next "Ben" the with more care.

This is just so sad, I feel so deeply for these parents and the rest of Ben's loved one's. I can't imagine feeling so incredibly helpless as your child proceeds to get worse and then pass away with doctors not doing anything. A tragedy.

First off my condolences to the family. Secondly I blame the doctors simple as that, in 2017 I woke up in the middle of the night in extreme pain laying in bed curled up, i managed to call my sister who was sleeping at the other side of the house and rushed me to Murray Bridge hospital in extreme pain which I have never felt in my life before after 3 hours of taking drugs the doctor managed to get straighten out so can get a CT scan immediately. An Hour later I got told I have a tumour 120mm long and 88mm round which destroyed my right kidney as there was nothing left. I'm one of the lucky ones that survived Cancer, so that's why l question the doctors in Bens case. What a waste of a great young man's life. A simple Ct scan could have gone a long way in Bens health.

The doctor said he often didn't disturb patients on his rounds, but he was specifically called in to see Ben as things were going badly for him. Were the other staff just bringing him for no reason? Why are they so tight with scams and tests, especially wwhen there is clearly a very ill person in front if them?

If this case highlights anything, it is that even with all the advances we have made in medicine, we are still at the mercy of senior doctors who believe they can't possibly be wrong. Also where there are insufficient facilities and experts on hand, mistakes will be made. We count ourselves lucky in the UK to have specialists close to hand when we need them but where the distance to the local clinic can be a days drive away they just aren't, and thats the reality here.

Such a simple solution would have saved Ben's life, with that much pain, a ct scan and the knife, find it strange that never happened, so pleased the coroner showed his feelings,as what a way it lose a loved one, the pain never stops, so you learn to live with it, all the best to the families involved ******* ...

26:30 the answer to this question is YES, the tare in Ben's stomach absolutely lead to the cause of death. If his stomach contents never leaked out into his abdomen then he never would have gotten a fungal infection would he have...? NO he wouldn't. The steroids lead to the tare in the 1st place so everything they were doing for poor Ben is what was killing him they didn't do anything right for that boy. This is a terrible thing for this family to have to live with then they took away any rights to any lawsuits they could possible have by "justifying" his COD.... JUST WRONG!!! RIH Ben ✌💜🙃🙏
