They show the dangerous ones on parades und test the dummy versions. Even N Korea dont test their nuclear weapons in missiles. They test stuff, that could be loadet into missiles and than show the missiles. And even they didnt done general nuclear weapons testing for 6 years. So NO! You dont fire up this particular rocket in a real scenario. Its a propaganda peace.

Yeah, but to his defense, just a round tip would be really bad for a nuclear missile. Too pointy, and it overheats, too round and drag gets higher. Western submarine launched ballistic missiles use a aerospike to "cheat" the air into thinking the whole nose is pointy at supersonic speeds.
