Absolutely agree with everything you said here. It can be very frustrating when you have too many variables and don't know which is causing what. It has been 5 months post H Pylori treatment, and only now after fasting, eating carefully, using things like oregano extract, berberine, probiotics, I am able to see consistency as to what is left to cure. We also are prone to self sabotage through being impatient. Most of these experiments require a 3 month+ window to really understand what's going on and start seeing consistency. I often see people quit too quickly, or change things up too quickly after 3 weeks of trying something.

I read Dr Pimental’s book and I have been following his low fermentation diet for about 2 weeks which was when I was diagnosed. I tried the low FODMAP elimination diet a few years ago. I did not have a SIBO dx but a nutritionist advised me to try the FODMAP elimination in order to see what foods bothered me. Nothing bothered me. Anyway, I started feeling better right away on the low fermentation diet. I started Xifacin almost a week ago. I believe that the most likely cause of my SIBO is Gastroparesis caused by long acting morphine which I have been taking for 4 years for chronic pain. I just had a shoulder replacement. Once my shoulder pain is manageable I am going to try to reduce or eliminate the morphine. My doctor agrees with this plan but he also pointed out that while the Gastroparesis secondary to morphine is the most likely cause, my history does show other possible causes which I can’t control and then I might need another course of antibiotics or a different treatment. How does that sound to you?

I'm halfway through my sibo treatment (rifaximin) and unfortunately so far I followed the "feed them" advice and ate high fodmap diet.. Well I'm not seeing any results at all so far. Seeing this video yesterday made me stop and change back to my "regular" diet (not eating high fodmaps but also not following strict low fodmap). This is so frustrating! I never encountered another subject with so much conflicting advice on the internet, as I do about digestion issues. It made me lose hope a bit for the treatment, wondering if I ruined this round by eating the fodmaps.. I also started biofilm busters along this treatment, but then I've read that they take weeks to break the biofilms, so I was wondering whether it's even beneficial to only start taking them while on 2-week antibiotics course? I would appriciate your take on that. Thank you!

Take a deep breath, and know that whatever you are doing, it helps you learn more about your body and what's going on... if the rifaximin didn't provide any relief, I wonder if: a) you have methane but were only prescribed rifaximin? If so, you need both rifaximin and neomycin or metronidazole. b) if you have a confirmed case of SIBO with a test? If so, did the numbers change with treatment even though symptoms stayed the same? If not, you likely don't have SIBO. THe trick is in asking the right questions, and looking in the right place for the answers...you've got this. @@ConfluenceNutrition Yes, I took the test and it showed high methane and hydrogen both. At least now I know what the problem might be.. I will take the test again after the treatment and will hope it will provide some answers. Thank you, I will check the webinar.

@@lensal8372 Great Len, yes, rifaximin alone isn't as effective for methane and hydrogen SIBO. The recommended protocol is to combine it with another antibiotic. For specifics on that, check out siboinfo.com.

So i read from Dr Pimentel he said to feed them becuase he said that when you dont feed them , yes some die but many go dormant and when they are dormant they are much harder to kill then when they are awak. that being said your opinion does make more sense to me

Hi Dave, yes, I've heard him say that, but I really am not sure whether it's a statement backed by evidence or theory. ANd either way, in each situation, we still need to confirm whether that advice is true for ourselves. That's why I focus so much with my clients on determining whether any piece of advice, from food, to supplements and medication, is correct and right for you at that time and place. The question is does it can be false positive test for SIBO, if I have low stomach acid (chronic gastritis) and maybe because of that low gut motility (they found I have IBS through colonoscopy). So I'm not sure if I should first increase stomach acid so do another 4 months of stomach diet and treatment, or I should do a SIBO test because the issues I have cannot lead to false possitive SIBO test, just because the portion of food is sitting in small bowel and it makes those gases all the time. Because I know the SIBO can cause gastritis and IBS, so I have both, but I'm afraid that I will spend a lot of money on test, it will become false possitive I will use antibiotics to erase good bacteria and then spend more money on suplements :/

It doesn't matter if it makes sense to you or not, it shows that you are not a medical professional and don't know enough about the microbiome if your argument is as straightforward as "Feeding the bacteria while treating it will make it harder to kill them" no its quite the opposite, we know from studies that bacteria can hibernate and protect themselves when nutrition is scarce making it harder to kill them. Your argument really is A to B logic without considering how the bacteria really acts.

Thanks for your feedback. I base my suggestions on what I see work with clients. The feeding the bacteria recommendation has often sent clients into drastic flares that make the problem a lot harder to deal with. It may be right for some. My main point is alwaysy that we have to adapt the "research" to what actually works for people's bodies.

Gong for a walk with a bulging disc also flares symptoms yet is the recommended way to deal with it because of the way discs work. Reducing symptoms cans feed sickness.@@ConfluenceNutrition

There are thousands of people in this country suffering from SIBO. I wish there was a clinic or you could go and stay for two weeks and be monitored while you are being treated to make micro adjustments to the protocol that you were on.🤷🏻‍♂️

@@ConfluenceNutrition I’ve done these calls with other people promising to help, but unfortunately, they just took my money. I am retired and partially disabled, and I can’t afford to risk reaching out to anyone else in the hopes that I will get better.

Hi there! Would you recommend using Sunfiber PHG w. herbal treatments? You mentioned it increases efficacy "during treatment" but I've only seen that noted when using rifaxamin. I'm on a 6 week course of Candibactin but have the PHGG leftover from my failed rifaxamin rx earlier this year. thank you! great channel

Wouldn't feeding the sibo a little bit be best, like what you said about the guar gum? If you are eating lots of iso feeding foods in high volume, then the antimicrobials/antibiotics would not be able to keep up. If a completely low formal diet is working, then introducing tiny amounts just to keep the sibo from going dormant or hiding during treatment would make sense, right?

I could go both ways on this, and it would depend on how the client was responding. Most people dealing with SIBO are already dealing with so many symptoms, it just doesn't make sense to me to poke the hornets nest. I focus on helping clients make some improvement in how they feel before we begin to push things that may make things worse.

Dear Amanda, my husband was tested positive for H2 Sibo and wants to start herbal antibiotics treatment asap. He is already more or less on low fodmap diet for years, but needs HCL Betain (including Pepsin and Bitters) without each meal to digest calmer. Can he continue talking HCL Betain (+Pesin) during the SIBO treatment? Thanks you 😊

Hello, I was diagnosed with Methane predominant SIBO and was given Xifaxan along with Neomycin for 2 weeks. I have not started the treatment yet because I heard I should follow a LOW FODMAP diet, avoid dairy and eat more lean proteins along with light foods. What do you suggest? Thank you for your time!

Hi Evelyn, the low FODMAP diet can help reduce symptoms, but I'm a strong advocate for only trying one intervention at a time. One can start the rifaximin without changing anything diet wise, and see what happens. Or one can start the diet, and get that going and THEN start the antibiotics. Either choice is valid. I’ve had ongoing issues with IBS symptoms (mostly diarrhea but also heart palpitations and anxiety) for the past several months. I’m currently waiting on results from a FIT test for food sensitivities, the GI Effects test and will be doing a SIBO breath test as well. If I get all these results at the same time and SIBO is positive, should I ideally make no changes until treating it? (I’ve already cut out gluten, dairy, caffeine/coffee and alcohol but am still getting flair ups semi-frequently)

Hi Janelle, great question. If you have time and you're waiting on test results, this is actually an ideal time to do some work with your diet to see if you can pinpoint which specific foods are triggering your symptoms. Getting this clarity helps you minimize your symptoms. The key thing here is to only focus on one NEW thing at a time. Hope that helps :)

@@ConfluenceNutrition Would I add in one of the things that i've currently cut out? I'm honestly scared to drink coffee again as heart palpitations are scary. I never used to get them. They suddenly started happening along with GI symptoms that then last for a couple day to a couple weeks.

@@janellemilazzolau1288 If you're really missing your coffee, or any other food, yes, you could try reintroducing to see if that was truly the trigger. Just start with a VERY small serving to start! That way, if you react, it won't be very big.

No, just hydrogen. For methane you need to take rifaxamin AND metronidazole together (or neomycin instead of metronidazole)

Hello, what are your recommendations on taking probiotics during the treatment. I was told to take probiotics during treatment, but like with food I am not sure if it will affect my treatment or not. Not sure how this will also affect my results when I redo the test for SIBO.

Many practitioners recommend this, and it really depends on the client. Some people do very poorly on probiotics, while others do great. I generally suggest doing what feels like it helps most.

Can u do stevia or does it act like sugar? After treatment will I ever be able to eat sugar from time to time again? 😩 I have gastroparesis so I’m sure that’s what caused rhe sibo. Doc said my hydro score was 63 and methane was detected but too low to worry about. He’s putting me on Alinia antibiotic 500 mg for 7 days. Not sure what to eat and if I can make gluten-free or almond flour muffins with Stevia or something like that. What about bananas? They seem to be the only thing I can eat when I am super nauseous that calms my stomach but I’m sure it’s loaded with sugar. I don’t have a lot of gas and bloating, and when I do, it’s usually minimal, and just for a few seconds after swallowing and eating, but I chopped that up to gastritis and my Gastro paresis. The only reason I ask for the Sibo breath test is because of the gassiness that is fairly new. But it’s still very minimal. I don’t know if the score of 63 is off the charts high or pretty low. What are your thoughts?

Stevia should not aggravate the bloating like other sweeteners, but it may or may not work for you individually. A good starting place with diet is to test out the low FODMAP diet. Alinia is not a typical SIBO treatment antibiotic. Usually, the prescription is rifaxamin for 14 days. 63 ppm is maybe about average, to medium high. You've got this!

dear Amanda, I am taking rifaximin for what seems to be SIBO and IMO. im eating basically carnivore with minimal fruits. my hydrogen seems to be going down but my methane goes super high sometimes on my AIRE 2, sometimes as much as 5 which corresponds to 25! I'm not sure if this is IMO. I don't feel like im constipated I just go to the bathroom rarely as always on carnivore. I used to burp alot after eating only meat! do you think Rifaximin is not enough? i have some terrible anxiety and nerve pain also. I'm not sure what this fluctuating methane means. i feel so bad im on the verge of killing myself.

Hi Bijan...did you complete an official SIBO test before yoru treatment? Were you high for methane during that test? This is what you should base your treatment decisions on. I wouldn't focus too much on any individual readings from the FoodMarble, this will just make you go crazy! As you work your way through treatment, the situation in your gut is in flux, and can change. Try not to get too lost in the weeds while you're going through it. If your official SIBO test is positive for both methane and hydrogen, then the recommended protocol includes the two antibiotics, rifaximin and either neomycin or metronidazole (flagyl). I did the test i did the test but my doctor hadnt told me that I shouldnt have any fruits or honey the day prior so I had some little fruit and honey. My baseline hydrogen was so high like above 60 although I had done 12 hour fast also. I might have screwed the test up. But the doctor gave me rifaximin because of the inability to do the test. Again thanks. You are awesome.

I personally haven't really found them to be inaccurate. Any test is imperfect, but in my experience, inaccurate results with the test are usually a consequence of user error. If the prep diet is done correctly, and the breath samples are completed correctly, I find the results generally helpful and accurate. I personally recommend the TrioSmart test because it screens for all 3 possible SIBO gases. If hydrogen sulfide is negative, any of the other SIBO kits are fine going forward, to only screen for methane/hydrogen. The FoodMarble is also great.
