Dr. D, I posted a long post yesterday about MGD expression. I THINK I missed a reply from you a while back. I’m sorry. This is my first experience with U tube. I was going through to delete somethings on u tube and just happened to find you. Then as I listened to you, I was so happy that SOMEONE knew about what I was feeling. Then I saw you are in NC I was incredibly happy. I feel I have hope! Now, to answer your question about what I’ve tried. Until now, I have been faithful to what opthamologist told me. OTC drops, heat compress, cequa, refresh pm. He was not dry eye specialist and now I know he wasn’t very concerned. There was a dry eye optometrist in that eye center. He mentioned her 3 times. Then said “she doesn’t take your ins. “. Well, I can’t pay out of pocket so he just said “ok” that’s it. Finally got so bad I asked for free consult. I THEN learned she did take my ins. I saw her twice. She had me do same routine with hot compress, at least 2 times per day, hydroeye 4 pills day, cequa, oasis every hour. The only thing was she wanted me to see rheumatologist and endocrinologist, which I did. Normal labs. Serum drops were discussed and unfortunately I can’t do it. Never discussed in detail (as you have done) why the dry eye. The MGD being root of it. I learned that myself and from you. I didn’t know anything about MGD, etc. until I heard you and then researched it. Then ab” lightbulb “ came on. Now, im searching for help. I n my opinion I truly need IPL expression. It’s been along road and now I have to figure out how to find it until im in NC. my story sounds unbelievable but I can assure you it’s a long, lack of information from my ex Dr. And I will find a way. When? I dk! I have learned so much listening to you. Thank you for doing these videos.

I really appreciate your presentation style, as well as when you share any pics, demonstrations, pros & cons charts, discomfort or pain levels, & patient feedback. I have ocular rosacea & dry eyes. Hydroeye has really helped & in the next couple of months, I’ll be receiving a series of IPL treatments & MG expressions. Thanks for providing info on what this will be like! 👁💜

Hey Gail, excuse me, I've been suffering from MGD for 2 years now. I've done IPL (like 3 sessions last year and 3 this year) and also LipiFlow. Been taking omega 3 supplements,, added some fish into my diet, also drinking cod liver oil daily. What is Radio Frequency? I've never heard about it. Would you please clarify?

@@fabicontin wow sounds like you have been doing a lot, I hope it has eased your symptoms. I have IPL and RF in the same session. Radio frequency is done after IPL. They use a hand piece that is heated to 43 Celsius which is the temp needed to melt oil, similar to hot stone massage. I don’t find it too hot. The hand tool delivers heat to the upper and lower eye lids by circling the wand on the upper and then lower lid 10 mins per eye. The benefits are enhanced function of the meibomian glands and new collagen production. I have noticed a difference with the 2 treatments.

What is the best multifocal contact lens for people with MGD? I have one taut eyelid (my right eye) that prevents me from choosing from trial pairs that I have been trying on.

Dr. D, I have questions. First, I KNOW my eyes are severe to the point of I am just wanting help so bad. I’ve watched many videos regarding IPL, Lipiflow, and tear care. In my personal opinion, I believe I would prefer to go forward with the most aggressive and best. I am thinking it would be IPL and manual expression. As I have said I am currently in Florida. I want to see you so bad however I don’t think I can hold on till July. I need help NOW. I’m s so thankful for your videos. I want to ask you if, by chance you may know a optometrist/opthamologist in Florida who does treatment like you? I know about Lipiflow, I just truly believe I need more. I’ve been in agony since I had those cataract surgeries. My eyes feel so full. I dk what they’re full of but I know I dx with MGD, blepharitis, kereoconjunctivitis(?), DES, and I hurt! I have floaters( that’s minimal) compared to the fullness feeling. I’m sorry, I just feel so hopeless at this point. I’ve been like this since 2020. I always felt dr. Was missing something and now I know. I need a dr. Now. So, if you or anyone in central Fl knows any dr. Who does IPL and manual expression please post. My eye are a full time job. I do have small eyes with very deep orbits. My eyes are gooey. They are extremely dark all around the eyes. I listened to a video regarding uveitis I don’t recall seeing that in my dx. However it seems a possibility of dx. To add to list. I have moments/episodes of my eye , usually one at a time will seem to start feeling hot, red, painful, red, and I will take pics of my eyes to see what’s up. The affected eye extreme redness, drains, and I can’t lay on that side if during sleep time. If so, my eyeball throbs. Idk! No, I’m not a hypochondriac. I have many pics and records to prove it. I’m so frustrated and I wonder if I will ever have my eyes back tolerable. I know it’s a lot. I’m sure some may question my complaints. I just want and need help. So, until I am in NC, I am asking if anyone out there in eyeland can give me a few good names of drs. Dr D, I truly hope to see you one day soon. I try to think about who or where I could reach out for help. Yes, I have ins. PPO. $$ are ???. I’m 66 and retired. Hard enough aging and can’t use anything to minimize wrinkles. I found this kind of weird also. After surgeries my eyes were going farther back in my head (sunken). The opthamologist sent me to lid specialist. What a joke!! He did ct scan of orbits. He found a dx. Starts with an E. I do have it on file. He/secretary was supposed to call me and never called. I didn’t bother. Why? I’ve been through hell in the eye area. I don’t want to give up but at times I feel like it. To Dr. D, and all of you who may have read this post, thank you. I’m sorry 😢 I’m just so tired and in disbelief that I went to the most reputable eye place in central Fl and no explanation of anything. Thank God I have all records or I’m sure some of you in eyeducation world would not believe me. BTW, I have many pics of my eyes as well. I’m sorry but thankful. I guess I needed to vent. Yes, I’m not only frustrated I’m angry. Again, thank you!

My OD has done a few RF treatments in between my IPL. It is with the TempSure Envi machine which also markets that it targets wrinkles. She expresses after the RF treatment. It does not go directly on the lid, though. The heat is supposed to penetrate up into the lids.

Hi Stephanie, my name is Patty. I have severe MGD, DED, blepharitis, etc etc. I just happened to run across Dr D. I was NEVER explained about my dx. As she has done. I’ve suffered since 20/20. You say you have had IPL and expression???? Correct? I’m interested to know if you would be willing to share your experience. Did it help? I live in central Florida and so far I haven’t found a doctor who does IPL and manual expression. I do all I know and have been told to do. My eyes hurt constantly. They fill full always. Blurry, red, burn, etc. I can’t wear any type of skin care, makeup, etc. will you share your experience? Also, if you know a DED specialist in Florida (central) but I’m willing to travel farther, please let me know ASAP. Ty. Patty

Well, I’m just learning you tube so please understand I’m now 100% with it YET. I’m just learning about a lot of this I have to ask. What is a few RF treatments? What does RF stand for? My most important curiosity is does it help you? I’m still searching for a dr. Who does anything more aggressive than Lipiflow. Will you please explain a bit more thorough?

Hey Dr. D. would you think doing tear-care twice consecutively would be better than just once every few months or would it be a waste? I guess i'm wondering if they can get all the thick meibum out the first time or if i should do another one maybe a month after? Thoughts?? I try to ask my current Dr. these things but she seems to always be in a rush and doesn't take the time with patients. Hopefully one day I can go to your office even though I don't live in the area.

i actually like to plan for at least two intial sessions. in the trials for these devices, they did one initial session then the data showed the results last ~6months. But i feel that especially when the glands are really obstructed, its helpful to do a second expression about 8 weeks after the first. would love to see you, but happy to help here as well!

Have you actually had the procedure?? I had it yesterday. I don't think I could take the discomfort again so soon. They make it all sound very clinical and impersonal but the reality of TearCare, at least in my experience, it HURTS. I am still sort of in shock and getting over it. I did not realize there were these other things as mentioned in these comments. Maybe time to ask better questions of the eye dr.

I had miboflow. The doctor told me it was useless though because my glands are shot. The imaging showed the glands in my upper lids are not visible. He said they can’t come back. My lower lids are barely functioning. Is it true that the glands won’t come back?

Yes, once destroyed the gland cannot be rejuvenated.

If you mean prescription drops, me too! It's like the emperor's new clothes - these things don't always work for all patients and no one besides me seems to notice!

I don't agree at all. I did Lipiflow and did zero, EXCEPT loss of $800. Better to take time and do it manually unless you are super rich. It isn't covered by insurance.

I like doing tearcare personally. We can customize the treatment more than with Lipiflow.

Me too! Did LipiFlow back in '16, cannot remember much relief. Yesterday I had TearCare. Very painful. After heating strips (which for the life of me I cannot understand the high cost) the dr has to manually squeeze eyelid margins with a tweezer and has to apply a lot of pressure. Hurts! I have to wait weeks before seeing results. Hope it was worth it.
