Does Amex card work well in Japan?

yes AMEX is accepted in most places but mastercard or VISA preferable

Shinjuku and Shibuya not connected to shinkansen trains directly and always crowded.

You can try to avoid pick our travelling for the local trains - for Shinkansens consider getting the green class tickets instead - they are expensive but ideal if you want to avoid crowds

it is not impossible but just very cumbersome as you have to purchase every time you board a train and also you have to work out how much to pay A IC card is definitely the way to go unless you plan on mostly walking everywhere
