For anyone wondering the relationship is sisters. Their parents are Izanami and Izanagi. Aka the creator gods of Japan.

Eika is based on Ebisu, their firstborn and a god of fortune who also happened to be born cursed. Keiki is based on the Haniyasu twins, a god and goddess of pottery born from Izanami's corpse after she burned alive giving birth to Kagutsuchi. Basically, Eika is Keiki's oldest sister while Keiki is Eika's youngest sister, which in addition to being ironic given their appearances, also fits the tradition of most Touhou sisters where the younger is stronger.

Fun fact: ZUN stated in an interview that Eika has ties with Keiki's idolatry theming, and since the gods they were based off of are siblings according to the Kojiki and Nihon Shoki genealogies, Keiki and Eika have been paired before in fanart. (From Strange Creators of Outer World)

Pretty much any god or godess or leader of a religion in Gensokyo is directly or i directly related to one another. With Okina Miko Keiki and Byakuren being some of the more connected of everyone. Kaguya, Mokou, and akyuu also being related or knowing others for a few 1000 years or so

Really because I thought that the relationship was that Eika was Keiki's daughter so when she died stillborn she made an idol of her that's actually how idolatry is explained in the Bible by the way.
