Tracheotomy for Intubated Covid-19+ Patient

This case was designed during the COVID-19 outbreak in order to assess and improve team preparedness for safely and effectively caring for a critically ill patient with COVID-19 who is already intubated and requires a tracheotomy.
Note: Expired or simulated/replica personal protective equipment was used in our simulation, in order to redirect precious PPE from the University of Ottawa Skills and Simulation Centre to the front-line workers at The Ottawa Hospital.

Case History:
You are the on-call surgeon and your next patient requires a tracheotomy. They are already intubated and have tested positive for COVID-19.
The patient is a 65-year-old male with COVID-19 positive status and bilateral pneumonia. He requires long term airway support. He is otherwise healthy, no routine medications, and no allergies. He is currently intubated and stable; however, requires conversion to tracheotomy for long term ventilatory support.
Learning Objectives:
1. Demonstrate knowledge of and effective management of a patient with COVID-19 to reduce the risk of contamination to oneself, OR staff, and OR environment;
2. Apply Best Practice Guidelines for tracheotomy in a COVID-19 positive patient;
3. Manage a high-risk aerosolization procedure of an infectious agent - tracheotomy

My mom lost her life from being trached and vented. She developed two fistulas which is supposedly rare and it went undiscovered until it was too late. Please educate dr.'s and nurses to look for signs of trouble. Air was going into my mom's abdomen and it was not discovered until she looked 9 months pregnant. Please please please watch for these signs. It could save someone else's life.

I love these! So important to work as a team. Plus so calm. I like the educational part. Yes please do one on how the entubated pt is to be assessed that air is not going in abdoman. I too know of a pt where it was found to late.. as a former nurse we followed several steps on our shift so it shocked me when I hear this goes Un noticed. Thank u.

@@518tttan Thank you for the response. I am sorry for your loss... My mother is currently on the ventilator for the last 18 hours or so. Hope she recovers. She took 2 shots and 1 booster and one flu shot and this was the result...
