This is a very nice explanation of the CLO technique. Thank you. I have been in practice for 15 years doing AP/LAT views for ESI but I think I may change my technique.

정말 감사드립니다. lateral view로 확인하는데 한계를 늘 느껴왔고 LOR의존하기에도 환자따라 variation이 많아 힘들었는데 큰 도움이 되었습니다. 혹시 contralateral view check할때 씨암을 대략 몇 도 기울여야 될까요?

환자에 따라 다르고 또 approach가 center에 따라서 많이 다릅니다. 최대한 oblique 를 돌려 lamina의 lateral profile이 잘 보이도록 하는 것이 가장 큰 성공 포인트 입니다

Thank you very much for the great video. In the video, you did not use a Tuohy needle while performing an interlaminar injection. You used a spinal needle. You did not use the LOR technique during the injection. So you performed the procedure completely with radiological image guided. Is there a risk of dura mater puncture with this method?

I have one case of dural puncture while I use a spinal needle. so, I do not use a spinal needle anymore. I always use a Tuohy needle.

In this method, you don't get any lateral view to determine the depth of the needle, right? You only get a contralateral oblique view instead of lateral imaging, right?

No, I do not use lateral images. it increases radiation exposure to the patient and operator.

How much steroid and local analgetics do you use in interlaminar lumbar block, is it depend to how much level you want to cover? In your experience untill what level drugs can spread if we do block fo example in level VL3-4?

I try to put the needle as close as possible to the pathologic area. I do not control the level by the volume. I have a rule, which do not exceed the triamcinolone 30mg in one person in one session.
