Spasticity can limit function and cause pain and deformity. Dr. Judy Gooch shows how botulinum toxin injections with Dysport or Botox help decrease spasticity.
I saw the MD last month and I am going January 14, 2021 for Botox. $1200 per session for the Botox. Have to go every 3 - 4 months. $1200 is a lot for many but for me it is nothing. This condition started when I was 55 years old. I am 155 lbs and always been an athlete and exercise daily. I use a cannabis cream daily and it helps as well. And compressions too.
The Baclofen I was given prior to the Botox did help but I got diarrhea and stopped them. The heart meds did the same so I monitored my BP for 1 month and reduced them so diarrhea went away. Under my heart surgeon's supervision.
Hello can i ask for u dr? My nape is contraction. I feel my nape is tight and if i walk my nape is sound. And my thorakal is tight and pain. If i squat down my thorakal is pain. Can use botox to this condition doc?
For botulinum toxin, I am injecting the muscle belly. For phenol, I inject the motor nerve or motor point. There is a video on phenol injections for spasticity on the Utah Neuro Rehabilitation channel.
Thank you for this video i have cervical dystonia have been getting xeomin the past year but it has not been working well for me so depressing cause i not inly have thie twisting of my head to the left but also adrenaline rushes and dizziness...do you think the dysport or botox would work better ?Have been doing xeomin because thats what insurance covers
I have a stretching regimen which I’ve be doing for years to help loosen my spastic quads and hip flexors, but I’ve plateaued as to their effectiveness. This sounds like a rather simple rather non-evasive procedure that’s worth trying. I’d be very interested in seeing before and after videos of the results of a treatment regime if you have any available.
Every person with spasticity is different. I usually do not inject the whole group of quads. They are an important muscle group for standing and walking. So you don't want to cause weakness. Sometimes it is helpful for tight quads and hip flexors to inject the rectus femoris muscle (one of the quadriceps that flexes the hip as well as extends the knee). Do you have access to a doctor that does these injections.
Utah Neuro Rehabilitation Thanks for your quick response. I plan to bring this up as a possible treatment avenue with my GP and see it makes sense to get a referral. My physiotherapist mentioned this treatment option, among others, so I’m assuming it is available locally.
I do to as well I must stretch before I move each morning. My issue is my median glutes. As I walk that contract until the point I can no longer walk. I've had 2 tomorgraphies. Dry needling ultrasound tens electromagnetic stimulation. Branch raidio frequently injections PT says I have gluteal amnesia and knots. Nothing has worked
I have glute medius pain also… there is increased signal At the tendon on the mri. My deep back muscles on that side are insanely stiff. Did you experience pain anywhere else?
@@yasminister2011 I do. I can't leave the house unless I stretch. I use a large heating pad to loosen the muscles and then stretch. Sometimes 3 times a day. My chiropractor said no more than that is recommended
Both neurologists and physiatrists (physical medicine and rehabilitation physicians) give Botox injections. Your best bet is to either look for a physiatrist at the website for the American Academy of Physical Medicine and Rehabilitation. You can also call one of the major hospitals and ask them for suggestions.
I need this procedure. My gluteus medius on both sides contract so bad as I walk I have to sit and stretch every 25 feet I've dealt with this nightmare for ten year. Orthopedic doctor has thrown the book at me. So has 3 physical therapist 😒😒some say I deferred pain Atrophy gluteal amnesia. But no procedure has helped. The only way I can get exercise is swimming. I can do that for hours with no pain.
Thank You very much for the knowledge. I friend is suffering for spasticity in her both legs, if we use this botox toxin while she is pregnant or without pregnancy, will it be effective? Is this true that if she is pregnant with a child, the child will also born with same problem of spasticity?
During pregnancy, you should be careful about all medications, even Botox and Dysport. Have her talk to her obstetrician about it. Spasticity is not passed onto a child unless the spasticity is caused by a hereditary condition, such as familial spastic paraparesis.
Hi mrs Utah can I have a question please... I looking for the spasticity treatment and I see they have Botox and dysport so what's different between Botox and dysport??? Thank you !!!
They are very similar. They are both botulinum toxin type A. I used to use mostly Botox, but switched to Dysport because in the US it is less expensive. I have found that it may last a little longer than Botox.
@@UtahNeuroRehabilitation oh really but they work a same treatment for spasticity??? Because I first time get a treatment on July 1 hopefully is work on me , I don't have a stroke, I have a hemispheric and spasticity thank you
Where or who can I get the injections? I have my hand balled up exactly how you demonstrate on the video, I was completely paralyzed on my right side and now have some movement although I deal with the spasticity. Thank you and God bless you 😊👍
I'm to see if it can reverse muscle fibrosis or at least relax it to a point. Example from an injury that's been scarred and in spasm for over a period of time?
@@UtahNeuroRehabilitation Thank you. I just received xeomin from by neurologist. All done by emg with utrasound guidance. I'm a week in and do not feel any different. Is this standard for the first time?
I have this. Dealing with it for over 10 years. Stretching daily, massage weekly, physio every 2 weeks, Acupuncture once a month, Reflexology every 2 weeks. Exercise daily. Going for Botox in a few months with a MD in Winnipeg. My acupuncture is GTT. The needles go into the muscle. I have about 50 needles per session. 1 in 100,000 people get this. MM
How much botox injections can be considered safe for our little ones? Is there a maximum number of treatment cycles that is considered safe levels? Doc, we have heard of myofascial therapy better than botox. What do you think about this? Really appreciate all these videos. These has helped us a lot. Thank you very much.
I believe that we still need more study on stem cells in cerebral palsy before it becomes standard treatment. But I think the research is encouraging and exciting.
Injections wear off after about three to four months. You can take advantage of the time they are working to make longer term gains with stretching or strengthening. In children muscles can appear more spastic as they grow because bones grow faster than muscles and then muscles seem tighter with growth.
I tried Botox for the first time. I got cp but active n golf. unfortunately now( Feb 23)shot. think it was a light amount. I can squeeze hold weights but unable to hold a golf club through out the swing. how long will this take. can a tens unit help.
We are looking for a provider that does Botox injections for children. Can you steer me in the right direction ? We live on the Alabama Gulf Coast - we are near Mobile,Alabama and Pensacola, Florida but have no problem driving further if needed. Thanks.
Try Children's Hospital of Alabama in Birmingham the Rehabilitation Medicine Department or Mobile Physical Medicine and Wellness. The group in Mobile sees adults. But they could also give you suggestions.
@@UtahNeuroRehabilitation yes I do have muscle tightness and contractions in my left and I wish to play the guitar and the my muscle tightness affects that
Hi Dr Gooch, my son is 8 yrs old, from India. He has hemiplegia, affected on right side and active kid. He is on physical therapy, but still he can't use his arms and has ankle spasticity. We planned to go for botox , but from last 4-5 months he is having siezure. My question is - 1 -can we go for botox for spasticity with his siezure condition ? 2 - somewhere in Google read, siezure can be treated with botox treatment - is that true ? Kindly help me to decide about botox or dysport treatment.
Hi Ma'am thanks for the knowledge...I had a stroke 1 year ago and have a footdrop probably due to calf spasticity. I'm thinking of this treatment...but before i make a decision I wanted to understand if there are any side effects of this now or in future.. could you please guide me on that..
In general it is a very safe treatment. It lasts a few months. We have not seen any long term negative side effects and have been using it for many years. See someone in your area who has a lot of experience in injecting and talk to them.
There are many therapy options depending on the person's needs. I usually recommend stretching and strengthening. A physical therapist can guide this. There are also some move videos on the Utah Neuro Rehabilitation RU-vid channel on stretching and strengthening. They can be found most easily in the cerebral palsy playlist.
Hi , can 10 year old kid for crouch in knees and ankle be given botox ? My son had botox injections at the age of 8 now he is 10 they say he is bit older for botox in India is it true.
That is not too old for Botox. Many adults receive Botox. It is more important to look at the particular person. Is the spasticity causing worsening contractures or limiting function? If so, then Botox may be indicated.
In general it is very safe. Make sure you see a provider with a lot of experience. You can get some weakness. It would be temporary. Very rarely, it can spread to other muscles and cause generalized weakness. But this is extremely rate.
You might want to look for phenol injections. They are much less expensive and might be available. We have a video on this on the Utah Neuro Rehabilitation RU-vid channel: phenol injections for spasticity
Mam thanks for the reply mam i wish you have a godday mam can i ask question again its the effect wwillbe permanent or twmporarary ?😬 Sorrry for the typo and bad grammar as a stroke surcicovery still suffering effect from stroke even iam instill recovery from stroke and do exercise everyday disclaimer iam not eglish speaker 😬✌️
hi mam iam an stroke survivour for last 8 years i had recoverd almost but my left hand fingers still strugles for its recovery how does botox helps in my case kindly help me on this mam iam in india
Botox or Dysport can help a hand after a stroke if the muscles are tight. Another video on my channel, Utah Neuro Rehabilitation, called Treating Spasticity after Stroke talks about this. Also, if Botox and Dypsort are too expensive, another substance called phenol can be injected to help tightness. Stretching is important and using the hand as much as possible can also help.
Botox and Dysport are both forms of botulinum toxin. They are injected directly into the tight muscle. They slow the signal going from the nerve to the muscle. This signal is hyperactive after an injury to the brain. The muscle is looser for about three to four months. Phenol is injected into the nerve that goes into the muscle. These injections are more difficult to give because you need to find the nerve with electrical stimulation. They last a little longer than the Dysport and Botox.
