In this video, I confront the reality of patient dishonesty, explore the reasons behind this behavior, and share strategies for fostering a more open and trustworthy relationship between doctors and patients.
It's no secret that the doctor-patient relationship is at the heart of effective healthcare. However, this relationship can be compromised when patients are not completely honest about their habits, conditions, and medication compliance. Why does this happen, and how can it impact health outcomes? We aim to answer these questions in our video, with a special focus on generational differences.
We often assume that patients are fully honest with their doctors. However, this is not always the case. From Generation Z and Millennials to Baby Boomers, each generation has its own patterns of concealment. It’s important to note that these untruths are often not malicious or intentionally deceptive. Instead, they are usually rooted in feelings of guilt, shame, or fear of judgment.
Gen Z and Millennials, for example, may not be forthcoming about mental health issues or substance use, including alcohol and marijuana. They might understate their reliance on these substances or downplay their mental health struggles. On the other hand, Baby Boomers might not be transparent about their actual diet and exercise habits or the extent of their tobacco or alcohol use.
One significant area where honesty becomes critical is medication compliance. Across generations, patients can be less than truthful about how consistently they take prescribed medications. This lack of transparency can be harmful, as it can prevent healthcare providers from making informed decisions and providing the best care possible.
In this video, we shed light on these issues, discuss the potential harm they can cause, and most importantly, suggest ways to improve openness and trust in the doctor-patient relationship. We highlight how understanding and addressing these tendencies to withhold the truth can lead to better health outcomes. We also underscore the importance of building a non-judgmental space where patients can be honest without fear.

I'm honest, I'm having a really hard time finding a good doctor who listens. Know a great MD in Cheyenne, Wyoming? I'm traveling once every three months to see my doctor in Jackson, Wyoming, and now I live in Cheyenne. It's a long trip for one doctor.

Doctors have a tendency to break your confidence when it suits them. That's why people lie to you. Because you can get any one of us in a lot of trouble with a phone call and you're given automatic credibility because of your status even if you could be lying or speaking in conjecture.

A little story about honesty towards doctors: Years ago, I had to to the neurosurgeon as an emergency, due to a liquorshunt malfunction. After 2 full days without eating or drinking anything and the nurses telling me, they didn’t know when my surgery was going to happen, my mood was, let’s say, not THAT good anymore. Because my mouth was dry like a desert, I asked the nurse, if at least I could have some water, since they didn’t even know if or when I was going to be operated. After a long deliberation with another nurse, she allowed me to have ONE sip of water. So I had one. Hours later, my surgeon showed up asking me how I felt and if I had eaten or drunk anything, since the surgery was going to happen in the evening. I was honest about that I had had a sip of water. Instantly, the eyes of the doctor darkened. He told me that I wasn’t being reasonable and that my irresponsible behavior could be delaying the surgery. I responded - I confess that I wasn’t very polite - that I had asked the nurse about it and instead of treating me like a naughty little child, he maybe better should talk to said nurse about the matter. So, after 2 days without anything to eat or drink, with him not caring how I was, with very little sleep- since a hydrocephalus can cause enormous headaches - and no exact plan for the foreseeable future, my honesty brought me this kind of treatment. I‘m sorry, but I understand people who lie, because if you’re not feeling well, that treatment is the last thing you need and want.

If pain was properly treated instead of punished as a character failure, patients would not feel the need to step outside of their Dr's advice and then feel ashamed for their actions. Self advocacy is exactly what leads to the little labels that forever follows the patient. Blaming the victim is never the answer and yet one is judged merely passing through the gatekeepers of a system meant to generate wealth while dispensing care, for a price. No other country creates a homeless problem by bankrupting the customers of Healthcare.

Many years ago my mother was supposed to be fasting before surgery (NPO since midnight but the procedure was scheduled in late afternoon). She ate one French fry in the early afternoon, against my advice (I was a hospital HIM coding specialist, also with experience in an ophthalmology practice where they had an OR, mostly cataract surgery.) When she was admitted and in bed, filling out forms, they asked her if she had been fasting. My sister and I gave her "the look" and she admitted to eating the French fry. Her procedure was rescheduled for morning. At least she got to eat supper *but* the anesthesiologist came while we were still there and they had a talk about that French fry. One French Fry. Important enough to reschedule a procedure, y'all!

Yet when I asked to reschedule my surgery because I had a productive chest infection and a un investigated heart problem, they were surprised and couldn't understand why I was hesitant.

To be fair sometimes, especially in elderly, one French fry is just enough for them to drown in their own vomit during surgery/immediate recover. One French fry probably wouldn't, but it's the kind of probably you have no real insight into. So no ability to differentiate between gonna drown and gonna be fine

Same. I'm mostly 100% honest with doctors. However, if I'm seeing a new dr. I withhold information about my anxiety. Because otherwise they will refuse to look into things that clearly are not anxiety-related. It took me more than 10 years, but I have a rare condition that most doctors wouldn't even investigate.

I suffer from fibromyalgia. Many doctors decide all your health problems are related to fibromyalgia. I rarely tell doctors and health professionals that I suffer from fibromyalgia for that reason. I also have IBS and acid reflux. I stopped taking iron supplements because of a massive flare of IBS. I did ring my doctor to tell him. I have increased my intake of red meat and eggs in the hopes it will heal my anaemia.

Gen X here. I gave my doctor, who was the Chief of Surgery, my daily food intake list for one week. He looked it over and asked if he could keep it because it was the most honest list he had EVER seen! I was obviously there seeking his help. I saw no benefit - to myself - of lying at all to my doctor!

But at the same time I have the gin X streak in me that says who cares what they think leave us alone. Really, though most of the time, I’m quite irritated by stuff like this to be perfectly honest.

I'm an RN and I am non-compliant. I don't lie about it though. I just don't take drugs unless I absolutely have to when nutrition and lifestyle changes don't help.

Here in the US, I have learned to never ever trust a health care provider inasmuch as they are duty bound to document in your permanent medical record everything you say and on every form you complete. Now, with the institution of the Nationwide Health Information Exchange, one's medical record can be accessed by any health care provider, insurance company, health care organization, etc., in every single state. This is being implemented without the patient's knowledge or consent. Medical records are often filled with errors that is then perpetuated for the lifetime of the patient. This is one of the many reasons that people need to lie.

As a patient I am very frustrated that my medical record has diagnoses that don't apply to me and it has resulted in being denied insurance. I do not have high cholesterol, diabetes, high blood pressure, or cancer yet you can find those diagnosis codes in my chart.

Are these in a hospital record or private office? For diagnoses such as these you have HIPAA laws that allow you to review your medical record. If you know there is an error you are legally able to request that your record be amended. In a hospital there is usually a patient advocate to guide you. I don't know how it would be done in a private office. Perhaps someone here who currently works in the medical field can advise or know a phone number or website. It's been 20 years since I worked in HIM coding. I had problems myself with insurance denying payment on two ER visits. I had been prescribed an antidepressant in 2000 and had an adverse reaction. Difficulty breathing, nearly fainted, then had a brief tremor after arriving in the ER. Instead of diagnosing an adverse reaction with the symptoms, the doctor assigned a primary diagnosis of Anxiety(!) Back then, insurances could deny payment for any psychiatric condition as a primary diagnosis. It happened again 4 days later with another medication, only this time I felt as if I was falling into a pit and was dying. Every 15 minutes. For this visit I was given a primary diagnosis of Depression(!) rather than Adverse reaction with symptoms. My primary doctor then prescribed a third medication. I filled that at the pharmacy (the first two meds were sample packs). The pharmacist took down the information I gave him regarding the previous meds and ER visits. He said they, as well as the third, were from the same drug family and they might want to try another. But he filled the Rx. I was sick as if I had the flu and missed work for 2 days. The 3rd day I returned to work. Morning was OK. In the afternoon, BAM. Trouble breathing and dizziness. I had to go home. Not the ER. I called the doctor's office. By then I felt as if burning hot liquid was under the skin on my arms as well. I got the doctor on call who said to stop the medication (ya think?). I told him what the pharmacist said. I went in to the office the next day and the doctor (thankfully) assigned a primary diagnosis of Adverse Reaction to prescribed medication. No more Rx. I was referred to a counselor. Back then I didn't know that I could request that my ER visit diagnoses be amended. HIPAA later spelled it out plainly. Too late.

I’m a boomer who’s had the same physicians for 20 years. All have access to MyChart, as does the hospital around the corner. I am excruciatingly honest about reporting. When I first saw my neurologist in 2002, I noted that I was an occasional drinker. (At that time I think my last drink had been in 1999.) Shortly thereafter, I hosted an outdoor party, opened a very expensive bottle of Chardonnay, and between the smell of the alcohol and the heat, began vomiting until I reached dry heaves. (Nice party, eh?) I haven’t touched alcohol since, but my chart notes still state I’m a social drinker. So I’m constantly admonished about my drinking 😂. I also admitted to experimenting with illicit drugs (mescaline, LSD) in the late 60s) for which I did not seek rehab. I’d laugh at this but “in recovery” appears on my ER records and is seen by resident hospitalists who don’t know a thing about me. Lastly, once a medication gets on your chart, it’s near impossible to remove it. Following an MI, Pravachol appeared on my discharge summary. I’ve never taken this medication lol. The cardiologist dictated over MyChart med list, substituting Pravachol for Provigil. Took about a year or 20 visits to get this corrected. And then there’s Zantac which my PCP prescribed to help with dysphagia when I reached the limit on baclofen. But ER doctors saw this and assumed I was taking it for GERD, and this diagnosis appeared on my records, prompting the alcohol questions. The PCP substituted olemprazolole (so?) but it doesn’t help with swallowing so I stopped taking it and changed my eating habits. Still it appears on my records. So, dear doctor, I’ve done what I can to ensure my medical records accurately reflect what’s going on and in me, down to the last 81 mg. aspirin, but it’s an uphill and at times absurd battle.

Ever since the implementation of EMGs (electronic medical records), the Bain of medicine, there are so many untruths on charts primarily because of too many wrong boxes checked

Why is it doctors lie! I’m honest and they lie to me. I got bit by a bug the night before my clearance before surgery. When I got bit by the bug I bleed for 30 mins. I couldn’t stop the bleeding. I put 2 elastics around my arm to stop the bleeding. When I seen my primary the next day I told her about the bleeding. I asked her to check my coagulation and she told me that the surgeon didn’t put that down. I asked her if she heard me say I bleed for 30 minutes and needed to use elastics to stop the bleeding! She finally agreed on checking my coagulation and it came back abnormal. She told me that it wouldn’t be a good idea to have surgery at this time. Gee imagine if I had the surgery and I bleed to death. I would have been signing my own death warrant. Speaking of a heart attack. The nurse ran out leaving me in the room having an anxiety cuz of the way she ran out. Next thing my doctor comes in and tells me oh your heart is moving more than normal. You think she would have explained it to me. I’m really upset! I have terrible doctors and I don’t trust any doctor. I am very honest with everything. Even about taking methadone. I have a lot of medical issues but I need to feel reassured.

Yes, absolutely. However: You being honest should not have any kind of negative aftermath for you. Patient/Physician talks are meant to stay in between said parties, right? If that’s not the case an the patient learns, that you can’t even fully trust a doctor, one might think twice about being fully honest.

When I was in 7th or 8th grade, one teacher told all the group, we were known as gen x. Since that all of us use the word "x" to answer any question about anything we didn't know how to answer. Ten years ago, I discovered I'm not gen X , I'm a millennial. Being honest, this ideas are really fun because this classifications can't be applied to everyone. I was born in 1982 in México City, my boyfriend was born in 1985, in rural Argentina. Sometimes I think he is more a boomer or gen x than millennial nor centennial.

Here in the UK it's a sad fact that Doctors have less time to engage with their patients. And phone consultations are no substitute for a face to face, patient concerns are missed or glossed over in the need to adhere to 'guidelines' on time allocated per patient. Market forces are increasingly reshaping the NHS in the image of American healthcare, and to be completely honest it's terrifying. I do appreciate this doctors efforts to address patient concerns and educate us about his. Thank you Doctor.

I never smoked or did drugs. I tasted alcohol (wine, beer, piña colada) and hated the taste of alcohol. It was like nasty cough syrup or something and a second sip was worse. This Boomer remembers when they showed films about drugs in junior high. They mostly focused on LSD, heroin, and marijuana (called "grass" back then, late 1960s). I remember them warning about residual effects of LSD, that it was possible to have "trips" later on even without taking the drug. Some of these drugs do cause damage that will manifest later.

It was good to hear you mention valerian and passionflower. In my experience , most allopathic doctors give me a blank stare when I tell them what supplements I'm taking. Since they are not schooled in such things, they are disinterested and distrustful of non-pharmaceuticals, which acts as a barrier to effective communication. It'd be nice if their schooling included an intro to holistic medicine so they would know more about potential interactions, and thereby be more trustable collaborators in the business of their patients' health.

Lol - I'll never forget the look on a new family doctor's face when I said I had begun to substitute a glass of water with a T or 2 of apple cider vinegar instead of the reflux Rx. 3 or 4 days of drinking the acv mixture takes care of my reflux for months. However, I can understand a doctor's doubt - they probably don't teach acv in med school. 😉

I came off omeprazole after years of taking it. ACV gummies were my best friend until things got under control again. I had reflux due to anxiety, and once that was treated I came off the reflux meds. It is NOT a fun process. Doctors will tell you the horrific acid you experience after stopping is due to the disease progressing. This is NOT true at all. Your body is calling for acid production and not getting it, so it produces more and more of the hormone that triggers acid production. When you stop the meds your body is pumped full of hormones calling for massive acid levels, and it takes months for your system to correct itself. They also don't teach Metamucil. A family friend was put on a statin for his high cholesterol. He never took it and started drinking a glass of Metamucil every day. When he had a followup test the doctor was amazed as he had never seen that drug work so well. He was in disbelief that Metamucil was the only thing the guy was taking. Word spread and my Dad and all his friends now take it and they're all off cholesterol meds.

Well as a boomer I always ask about generic brand medicines. They work as well and are cheaper. As a person living on a Native American boundary, I use some traditional medicines. I tell my doctor as I know medicines, traditional or modern, can interact. With my arthritis I would prefer marijuanna for my pain but it is still not legal in my state. As for my diet…it’s horrible. It has improved over the last year and I’ve lost almost 40 pounds but there is vast room for improvement( as I sit here eating cookies).

Doctors here in Germany always make a comment that Americans need more anesthesia, but I don’t think that’s true. I’ve been dealing with chronic pain for 13 years, and tolerance is a factor. But one thing I’ve noticed is that one one really listens. Regardless of the field of work. Again thanks! Now I know why I woke up during back surgery

⭐⭐⭐⭐Great topic!!! The truth or dare scenario occurs more often than people are willing to admit. For example: I may have a pt. that comes in, I introduce myself and I advice them that I'm about to get their vitals signs BP, P, R,T, then I usually ask what's your weight? and they answer "Mmm 145" when one could see that this pt. is no less than 175pds. Therefore, I follow with "Please step on the scale", and then they act totally surprised and say "Oh! something is wrong with your scale" next question, Do you use, or ever used any kind of tobacco, recreational drugs, alcohol? and they respond "No, I do not" mind you I can smell the tobacco on their clothing, and I can see the yellow finger stains. That's when I inform them, there are ways that we in the medical field can find out if you do. Furthermore, I tell them I'm not here to judge you, I need to know because any medication that we may administer, could hinder your medical treatment. At this moment they usually say "Well I stopped smoking a week ago, and I have 1 or 2 beers on the weekend, plus I smoke every now and then a little "weed" At this level, I don't act surprise, I just thank them for coming clean, and I continue with their medical history.

I have had many surgeries but, I think about those who are having their first surgery and how listening to you talk about it with your smile and calm demeanor would help them to understand that the doctors are there to help them. And, now through ketamine. I also went through a big phase of thinking everything was going to kill me. . sooo probably too honest. #ADHD #Depression #Anxiety. My Dad the Boomer ALWAYS asks for a discount. Either Senior or Military. Which that's totally cool but like, every single place we go. I got him a t-shirt that asks "Can I have a discount". I'm too mortified to ask. But I will ask about income based care along with my insurance. Being neurodivergent, I over think about all of my medical stuff. I asked my Doctor for a full body scan so I could see everything wrong and she refused and told me that was a bad idea. But like, I want to know. 🤷

Unless you have symptoms of something you will not get a scan. It’s unnecessary radiation( CAT scan) expensive and causes people to worry. Scans can show minor benign cysts which can worry people then they insist on a biopsy and/or surgery. In most places you can get a scan if you pay out of pocket. There are very long lists of people waiting for MRIs as well that are triaged by urgency. Suspected cancer and acute conditions go first so if you have no symptoms no physician will just send you for an MRI or any other scan.

How about keeping quiet versus speaking up when your doctor tells you something that you know is wrong? I spoke up about being prescribed too high a dose for my weight with a very potent cardiac medication but my cardiologist ignored it, saying that the dose was necessary even though I had a SNP which made me a poor metabolizer of that drug. I did, with reluctance, decide to be compliant and ended up with complete heart block. How could I have better handled this?

Oh, I missed the live stream. I'm currently on holiday in Arenal D'en Castell, Menorca (the Baleric Islands). Its been so hot here & having to be so careful & keep in the shade as much as possible as I have Polymorphic Light Eruptions & have a skin cancer on my leg. I’m facing my 5th (yes I said 5) surgery 4 hip dislocation & I am absolutely confused as to why my hip keeps coming out of socket after doing EVERYTHING Drs tell me 2 do! As a chronic pain patient for over 20 yrs I expect & trust that my Drs will give me honest info just like I know my Drs expect honesty from me as a patient. My original hip surgery (hip implant after fall resulting in a broken hip was from a diff Dr) was fantastic & lasted over 3 yrs but last 3 surgeries to fix a dislocation of same hip has been 1 disappointment after another & I’m STILL not walking & in a wheelchair! How many surgeries do I let my current Dr do on me b4 going to a diff Dr?? I don’t know who to trust! Your videos have empowered me & taught me how to question my Dr w/out feeling bad! Also you have helped me understand why I am expected to undergo surgery w/OUT pain meds! I discovered I am labeled a “drug addict” in my medical records therefore my Drs WONT give me pain meds even though I have been honest & TRUTHFUL w/ALL Drs about all meds I am taking from pain management Dr! I am a Boomer caught in that Opioid pendulum you talk about but I was forced to stop all prescribed pain meds (opioids) after my Dr retired during the opioid meltdown! After 22 yrs of taking pain meds (low dose hydrocodone) SUCCESSFULLY w/NO issues to treat my chronic pain issues I was forced to stop Cold turkey! I suffered in pain until I finally found another pain management Dr who would NOT over subscribe me opioid pain meds! Then came my hip issues! But waking up from a 5 hr hip surgery in horrible pain to hear nurse say “u can’t have any pain meds BECAUSE u r a pain pill addict” was the absolute worst experience I have EVER been through! Thank you for your insightful & informative videos! I now understand what is going on. I don’t know how to fix it but I’m getting there! You are a godsend!

OMG! You poor thing!! I wish you could get a doctor like this guy. I'm a boomer. I have chronic intractable pain. Fibromyalgia too. I have to have opioids or I'm bedridden. I've been insulted many times by doctors because I do take pain meds. You are a drug addict. Puhleeze!! I don't crave it or abuse it. I sure don't get high. I'm just able to have a life! I've been jerked off a strong opioid cold turkey and nearly died. I find myself praying every month "Is it going to happen again?" The DEA needs to back off!!! Become obsolete!!! They don't care whether people OD or not. It's just about control! You keep looking for a good doctor and DON'T let them keep cutting on you, Please! Everytime they cut it builds up scar tissue that can become problematic on its own. My fiancee has had 6 failed back surgeries!! The 2nd surgery he ended up nearly losing his life because he got a horrible staph infection in his spine. That was 40 yrs ago and the staph is still there. Slowly eating its way up his spine. His pain is horrid. He was on some heavy duty pain meds and the DEA messed with the pharmacies and he got jerked off just about everything. Cold turkey. It's been a nightmare!! He is non functional now and in tremendous pain to the point he can't sleep more than 3 hours. I will put you in my prayers 🙏✝️❤️ Please find you a good doctor. The medical professionals pretty much suck anymore. They need to clone this doctor. I just adore him!!❤

My surgery was twice in two days and they found the infection in the bone but it is finally healing, and I am finally home. I pray they will give me surgery.

As a retired pharmacist, I got lots of honesty. I wish the Drs could’ve heard. People would share all sorts of things. And in most cases it was good info to have. (Knowing that you are going through an extra hard time mentally is good to know. But we are not licensed therapists.) Overall I am glad people felt comfortable asking me some questions they were either too embarrassed or scared to ask their Drs. Lots of questions about their supplements. It was and still is a special interest of mine. Looked up potential drug interactions and did encourage them to tell their Dr. I would print out I formation and suggest reputable sites for their own research.

I had a lot of steroid injections in my back. At one appointment I told my doctor I forgot to fast. He went ahead with the procedure & told me to try not to forget next time. It was never explained to me that it was dangerous to eat before a surgery, so I ended up aspirating.

This may not be relevant, but i want to speak to your point about millenials being more comfortable with virtual visits. ( which they didn't have when i started out in clinical in the 80s,). I can see why that would be so; especially if your televisit is for depression or anxiety However , anesthesia aside , there are so many necessary things that can't be done virtually eg. your blood pressure, EKGs, blood work, physical exam, etc. I had virtual visits with my dr for 9 months, before i finally went to a neurologist after a fall and was diagnosed with Myasthenia Gravis When i finally saw my dr in person after 9 months of virtual visits, his first words were ," oh my God,!!:. Now drs are trained not to show shock and alarm, so this really scared me and i started to cry. He said " How did this happen?". I felt like saying, because I've been telling you about these symptoms during virtual visits and you've never addressed them. So, i was 126 lbs; now I'm 96 lbs and look like a praying mantis So what's bad about virtual visits? Unless they are for therapy, or as followups after surgery to go over meds, you're better off going in How can someone examine your body without seeing it or touching it

In Canada all your medications are on the data base & one can look back up to ten years. Just ask the pharmacy to print a list out anyways the medical professionals can access it on the computer. I’ve had to have four major surgeries in the last 3 years and I’m 100% honest because they’re long surgeries and I want to survive. Two weeks ago I saw my preop doctor he asked about alcohol and I said one glass at Christmas and one glass of wine three weeks ago, so rare😂he laughed and OK good to know. I think a lot of people under estimate how much bad stuff they consume and over estimate their good habits just like you expressed here. We all want to think we’re doing the best we can for our heath.

I don't lie to the doctor unless it is expected. E.g. when I'm asked whether I have picked up the clutter in my house, I say yes. I don't trust doctors very much, except for very few. They never give informed consent regarding how vaccines are made. My experience is that Medical Doctors are rarely interested in supplements and disapprove of them rather than looking into them, so I don't bother mentioning them. My current medical doctor is an exception.

Doctors have become nervous to prescribe pain meds because of the opioid epidemic and all the new guidelines where they can lose their licences or have them suspended. It’s sad because there’s a lot of chronic pain patients who aren’t being properly treated because of this.

My experience its hard to med rec from most ALL age ranges. The over a certain age can't remember all their meds (please bring bottles to next appointment), the middle age never seem to think when you ask I need all meds and OTC's you are taking...enter MD and they remember three more 🤔. Then you have those that you ask are you taking anything for the pain "no". Then assessing for pain you ask again the response "what is your pain on 0-10 scale" is a ten, are you taking anything for the pain. "OH yes I am taking 3 1000mg of acetaminophen 4 x a day" 🤔. And then you have multiple people of different ages (over 50) hot box before they walk in. Then you have that one patient that doesn't want to tell you about the THC gummies prescribed legally. Then my experience today tooth extraction, I went to pick up meds 2 days before 4.rx's ready. Ibuprofen 800mg, um no, did I not say I can't take Ibuprofen d/t gastropathy and BP elevation. I was told twice take whatever anti-inflammatory medication you normally take. Um I don't. I am assuming oral surgeons only seem to be rx ibuprofen. If I need it like I will take my DH tylenol 3 that is 6 years old.

@7:30 - That's interesting. For me, those conversations are most difficult in person. Easiest in text. And easier if the interaction is asynchronous. Doctors and the medical system as a whole don't seem to be very flexible on this, though. I don't get it. If both of us want the communication to happen, why not make it easier?

I always miss the live feeds, I live in England , my timings are off. Love the videos, wish Dr's were more outspoken to say these things. At least have a conversation with the patient, instead of a quick few questions then, they turn away. Go onto the next patient. Makes me annoyed. Having my gallbladder removed in February 23, I have had a few problem's. When my surgeon came to see me, his exact words were "did I operate on this lady", well yes you did, as I asked you to look at another problem while you are there, still associated with my gallbladder. To say I was upset , well I won't have him again, and yes he turned and walked away straight after he asked the question!!
