@ I am Sekou - Pilot should be wearing safety head gear. Boat should of cleared ship's side once safely on ladder. If he falls from height onto the boat, he's going to be more banged up than falling into the water. Deck hand should walk down outboard side on return, harness or not it's poor seamanship.

Pilots as with other Marine Professionals should have as PPE a safety line to prevent injury in event of any problem with Pilot ladder. In the event a suitable safety line does not exist it could be designed and provided as part of safety standards for Pilots.
