Thank you so much for this. My ophthalmologist did a thorough job of explaining my cataract options, but it was a lot to take in during one sitting. So it’s great to have a resource like this to more fully understand my choices.

Dr. Wong, Your videos are really comprehensive and lucid. They helped me to make the decision to have cataract surgery here in Vancouver (BC) 9 months ago, at age 75, because your explanations of all aspects of the process are so clear and so positive. I had no sedation of any kind. The first surgery-totally painless, as was the second-took fewer than 10 minutes; the second no longer than 15 minutes. The eye drops, which I was not thrilled about having to use, were just fine, and I drew up a timetable for application of the 3 kinds of drops in decreasing doses over a period of 4 weeks, which helped make it a very easy task. My nearsightedness had progressed to a visual acuity of worse than 20/100 in my non-dominant eye, and everything was getting blurrier as each month passed. So I knew it was time to act. After the surgeries (done three weeks apart), I ended up with 20/30 in my non-dominant eye (done first, and set slightly near-sighted on purpose so that I would be able to see well close up, as well as at a distance, which in fact I now can) and 20/20 in my dominant eye. At a family outing to a restaurant last Christmas season, there were 8 of us at one table, all but one younger than I, and I was the only one who didn't need glasses to read the menu. I haven't worn glasses even once since the surgery. In good lighting I can even read the tiniest print on a bottle of vitamins. I did have a PVD in my non-dominant eye 6 weeks after surgery (I had undergone a PVD in my dominant eye several years earlier), and I went to emergency and had it checked out thoroughly and then checked again by my ophthalmologist a few weeks later-all okay. So, as a result of my experience and the information you have kindly provided, I've been able to help others in a similar situation, giving them a bit of guidance and encouragement. As I type this, my laptop sits about 2 feet from my eyes and all the text on the screen, even the tiniest, is perfectly clear. Before the surgery I had to bring my laptop within 8 inches of my face in order to see the text reasonably clearly, and people's faces at even 20 feet were becoming progressively blurrier. Quite a dramatic change, with such excellent near and intermediate vision, not to mention the clear vision I now have at a distance (not perfect, but excellent enough for me). So thank you again, Dr. Wong, for providing such indispensable information in such an effective format.

@@pennyf9886 Yes, I was offered the standard monofocal lenses which are fully paid for by the public health care system in British Columbia (and Canada) and was told that their quality is good and that, given my profile (e.g., no astigmatism, etc.), I didn't need any special type of lens. I'm happy with the result. I haven't worn glasses of any kind since the surgeries.

@@pennyf9886 A good friend of mine had the same procedures as I did, same lenses, same surgeon, etc., but he has a significant astigmatism and therefore he uses glasses to correct it. He had his cataract surgeries in his mid-80s, and is now 94 years old and still enjoying good vision as a result of his lens replacements. Another friend of mine also had cataract surgery and enjoys good vision without glasses for most purposes, including computer work, although he does use glasses for driving at night or when it's cloudy and/or raining. I know about 10 other people who have had cataract surgery, most having monofocal lenses and a few having premium lenses (i.e., lenses for which they had to pay extra, such as toric lenses for astigmatism correction, etc.), but all have expressed satisfaction with their new vision. In any case, all best wishes for your own new vision!

Thank you so much. Just had cataract surgery and your video is so informative. Initially watched just to readjust my eye shield, and I learned so much more. My vision is great, btw, after four days. Never knew I could see this well, as I have needed corrective lenses since I was eight years. Developed astigmatism in both eyes in my late teens. So seeing as clearly as I am now seeing is a miracle

Had cataract surgery yesterday and it went well but hate the plastic covering you got to sleep with..It amazing the technology to protect a 3500 dollar surgery is a plastic eye piece that look like it was bought at a dollar store.

Thank you very informative! Had PRK surgery 25 years ago. After cataract surgery I couldn't see how many fingers he was holding 2 feet away from my face 2 hours after the surgery! The doctor told me he could see a swollen ring on my cornea from my prior PRK surgery. 24hours later the prescription anti inflammation drops began to clear up my vision and 4 weeks later I can see well! FYI ex PRK people.

Dr, Wong, thank you for your detailed explanation and caveats. My question: I was not happy with the outcome my left eye surgery in Feb 2020. COVID outbreak led to closure of clinic, so I had to wait till May for the right eye to be done. In late May, when I went to meet the surgeon after the second eye, I mentioned that my left eye didn't feel as good as the right. I was told that I might need to give it some time. Many months later, I still did not like the way the left eye felt. There seemed to be a very thin milky film through which I was seeing with the left eye from waking to sleeping, whereas the right eye was 20-20 and all clear all the time. There was also this sensation from after the Feb 2020 surgery of something holding, as it were, the left side of my left eye all the time. The surgeon spoke of my having scar tissue which could be zapped by laser. I decided to have it done. I called the clinic weeks later and said the "milkiness" and other "holding" sensation hadn't changed much. The clinic suggested that I give it some time. Earlier this year, I went in for the post 6-month visit to the surgeon. He looked and my eyes, said all was perfect--despite my repeating what I had been experiencing for a year now post surgery. He brushed it off with his hand, while exiting the office, and said once again, "It's all perfect." I called again a couple months later. The clinic heard me out and said, "Visit the surgeon again." I expressed my reluctance since he had seemed dismissive of my complaint. Last week, I went personally and said I have the same issue. They want me to meet the same surgeon. It seemed to me like a symptom of madness to go to the same person and say and do the same thing over and over and obtain the same comment as a result. And, worse than that, to have to pay $35 as copay for every single visit, including my laser zap session. I'm thinking of approaching a different eye specialist. This man seems to not be competent as he can't fix my problem without repeating (dilation of both eyes, and so on). How would you account for the "milkiness" and "holding" sensation? What should I do? Thank you for your patience and your time.

I had my left eye done 3 days ago. I was a 20/250 in that eye. Now I am a 20/20. I had my right eye done in June of this year. That eye is also a 20/20. Not bad, not bad at all. The Lord gave me the mental strength to do this. I also had NO sedation at all during both surgeries. I did perfect. I do not like drugs, at all.

Dear Dr. Shannon, I had Monofocal Lense Replacement Surgery here in the UK a year ago. I wanted to have the Multi-focal lense replacement, but was told I can't have it done because I have Rheumatoid Arthritis, hence I only had Monofocal to improve far and intermediate vision. Why is that? The Opthalmologist explained it to me, but I did not really understand it properly. Hope you can do a video about why someone with Rheumatoid Arthritis is not a good candidate for Multi-focal lense replacement nor laser treatment. Thank you very much. Take care.

I'm 65. I can't see the server clearly when playing Pickleball. Driving at night is a "nightmare" due to oncoming glare. Fortunately I don't have to drive often at night. Final straw was my right eye's vision has deteriorated between July 2020 (when I got my current glasses) and January 2021 when I had difficulty on a trip driving at night. Left eye cataract surgery scheduled for tomorrow with Dr. Paul Griffey of Griffey Eye & Laser Center Chesapeake, VA. Right eye is scheduled for April 7th. Can't wait to get this done. Thank you again Dr. Wong for the excellent information.

@@chrischamberlain7628 Both procedures went well but drastically different. For the left eye (done first) Dr said I was moving around and not following instructions (I was half out so I don’t remember any of that) so they put me out completely. Woke up, didn’t feel a thing vision next day was fine. Two weeks later for the right eye I was awake (and unfortunately following instructions) and I could feel him scraping the old lens out of my eye NOT FUN. It didn’t really hurt since the eye was numb to some degree but scared the -- out of me because since I was totally out for the left eye, I didn’t expect to feel anything. Dr kept telling me we’re almost done, so this is that but I made it through. Next day right eye wasn’t quite as good. Ended up with distance 25/50 so I really don’t need glasses for anything except reading.

Thank you I had panoptix 16 and 18 days ago right and left respective. I have ability to read and drive without glasses but very blurry otherwise known as ghosting. Today I can see a positive difference in clarity between right and left eye where the right eye was the first to be operated on. I hear a lot of people having clear vision after 2 days but I don’t think this is realistic for some. Hoping the ghosting will now start to improve from now in both eyes as everything else is great other than ghosting.

All eye surgeons should make a video like this for their patients to watch prior to surgery. That way, they don't have to hurry through any instructions, she/he/it just hands the patient a video and says Watch this.

I think CLEAR LENS (not cataract, good corrected vision with glasses) extraction and implantation of multifocal intraocular lens is not good. even if surgeon wipe the lens capsule bag clean, remaining cells prolferate. and then aftercataract(opacity of lens capsule ) must occur in the future. Most of young patient have to recieve laser capsulotomy(rupture of opaque posterior capsule) after surgery. Cells that continue to proliferate may influence macula and aqueous outflow through ruptured posterior capsule. what do you think about it, Dr. wong?

Dr. Wong, thank you for the detailed explanation. I am a RK patient, When you mention that I will need longer time to stablize the vision. Do you give RK patient the same lens as the other patients? I heard Light Adjustable Lens for RK patients. Can you feed me more on this kind of lens?

Usually how much is symfony lenses? What % is usually paid by Medical insurance in a cataract surgery?

I have a question about the Lens Replacement Surgery for a person who is 45 years old and has quite a significant Myopic prescription difference between the left and right eyes. She wears glasses now which bring both eyes somewhat near the perfect plano but she claimed that when they tried to fit her with glasses that was within 0.0 diopters for 20/20 in both eyes, she could not tolerate them. Contacts seemed to work for this problem. What I would like to know will Lens Replacement Surgery cause similar issues as the glasses or whether the brain will be able to resolve the issue of the wide diopter difference between the eyes. Thank you.

If there is a large disparity in the prescription (usually 3 diopters difference or greater) then vision through glasses can be problematic. Contact lenses work better in this scenario. Lens implants work as well as if not better than contacts for this scenario as well.

Dr. Wong, Your videos are absolutely first-class and obviously greatly appreciated by so many! I was pleased to hear you say that "A person could wait all their life and never have cataract surgery," mainly because I have been offered cataract surgery but do not feel bothered enough (yet) to opt for it, preferring to try a first ever pair of prescription lenses, at age 74. So, is what you say categorically true, or can a person by holding off on surgery either a) go blind in an eye or b) end up with cataracts that are problematic to remove and replace with a lens?

The scenario can arise where the cataract is allowed to progress to a point where it is more technically difficult to remove it. Still, once the cataract is removed at this late stage, the vision will in all likelihood be normal with cataract surgery.

Hi. Can u answer this question please? My story: Say 10 plus years a go I had my right eye cataract removed. After surgery, 3 days later surgeon removed bandages I was able to see clearly right away but double vision . Yes my left ieye had become lazy. Back than but after it corrected no more lazy eye. Luckily my vision corrected aon its own so both eyes worked very well no glases no glare no short Orr long distance issue. I went back to same surgeon last month for left eye. Yes I haD cataract in left eye even than was recommended to have it removed but I have waited 10 plus years yes my left I was very developed dense cataract. Removed bandaid 1 day latter absolutely no vision as if no surgery was performed I. My left eye. 5 days after surgery I have gained vision gradually has gotten better now 11 th day. However, left eye has the generic lense I chose, whatever my insurance paid. Since my right eye also has a simple lense from 10 years plus Now left eye yes was told by surgeon more astigmatism than right eye R is ..7 and l is 1.7 astigmatism. With normal lense I have 11 since my right eye has a near and far distance is vibrantly but no clarity in Either short or long distance. With both eyes I can see 20/15 to 20/20. But left eye I have only sharpness no details on short or long term. Can u help with this problem I need bifocals on left eye or need to put in multiform lense re cataract surgery?

Thank you Dr Wong for all your information it was very beneficial I just had both my eyes operated on at LBJ hospital in Houston Eye clinic with Dr Kim and His surgical team and interns he did an excellent job on my surgery and my vision is restored probably at 90 95% and I am more than Grateful for their work in their team effort but thank you for your information it was helpful and it made me feel comfortable in my surgery God bless you and may you be successful in your career!!!!

Dr. Wong: If I get Monofocal IOL for seeing up close, will there be a sudden visual “drop off” past that visual range. I understand I will be needing glasses for driving, etc. But what about the intermediate range? Will the TV be absolutely blurry (like it is now {presurgery})? THANK YOU!!!

Hi Dr. Wong. I’m 60 yrs old and will have cataract surgery in both eyes this month. I live in Chicago and the Dr recommended the Tecnis symfony ZXR00 IOL to both eyes but Im not sure if this Len can let me read up close to small fine print and a far distance for driving . Your opinion is very important and Thanks in advance for takeing your time and feedback

Very informative video in general but more emphasis should be made that not all people are candidates for multifocal or Symfony lenses for various reasons - characteristics of the eyes, personality type, etc. A video discussing all these factors would be helpful.

I will be 92 this November, male and I need a cataract surgery for my left eye. Right eye was done 5 yrs ago. Couldn't stand the 3 x a day chore of drops in the eye. Am I too old to go for it? Also AARP article suggest to ask doctor about if I am a candidate for the PanOptix lens.

In my opinion, if you are 92 years old, online and on youtube and not seeing well and have a cataract, then there is no better time to have cataract surgery to see your best than now. #QualityofLife

I need Cataract surgery and I was thinking about getting the Vivity lens because I do not want night glare or halos and trying to minimize the use of glasses. I can see small print without glasses when it is very close up and I am very nearsighted. If I can see well so close up, do I really have presbyopia? I am 56.

I am terrified of getting this surgery..... I'm only 34 and I was just diagnosed with cataracts, part of me wants to get a 2nd opinion because I was at a walmart for my glasses appointment and i'm not even sure I've got the correct perscription...... I'm just horrified... I don't know what to do.

Hi Dr Wong, i have some question if dr can advice. My wife age around 40 with eye very very high power and ophthalmologist suggest to do either ICL or Cataract surgery She interest to go with ICL which the original lens still preserve , the brand use is oculentis lense for ICL , do this consider premium lense grade. After this ICL she will no need to wear any glasses and depend. However there will be others halo, galo that could arise. That will be just stay with it . As my understand your video premium lense replacement will straight replace our original lense and suitable for Cataract. ICL should be different technology that i understand to put another lense. Varies greatly depending on multiple variables: 1) the eye care practice, 2) the surgical center, 3) the anesthesia used, 4) whether you have a visually significant cataract, 5) your medical insurance, 6) your deductible. Big picture - between $1000-6500/eye.

Cataract or lens replacement surgery would in all likelihood HELP a patient with narrow angles. The natural lens at age 49 is much thicker than an intraocular lens.

It was indicated on 10/9/18, by Dr. Wood at the Department of Veterans Affairs (DVA), I have -2.00 diopters in both eyes. A Corneal Transplant was also successfully performed by Dr. Nieman at Texan Eye in my right eye on 9/4/18 and my left eye on 9/29/20. I am looking forward to cataracts surgery that was indicated also will be needed. I am currently wearing a Scleral Lens in my right eye and able to see letters at 20/20 but prefer not wearing contacts at all if possible with Symphony or Panoptix Lens. I know will need to wait until left eye completely healed, but needed to know at least the cost for lens, and my understanding the cost of surgery paid by the DVA as a Disabled Veteran. Please let me know any other information important regarding my eyes to know to help ensure a good candidate for surgical vision correction with lens replacement. I was unsure if lens for external depth of focus, trifocal lens, or multifocal lenses recommended for my condition. It was also indicated many classes of Extended depth-of-focus (EDOF) Lenses available and you can choose lens for far vision or Toric Lens for astigmatism.

Dr. Wong, thank you so much for your informative videos. Are IOL lens prescription the same as contact lens? I had lens replacement surgery on 7/5, 12 days later, my vision is still blurry. My prescription after surgery is 20/40 (surgery eye). How long can I expect my vision to improve after surgery? You mentioned a fine tune surgery but is this done only for patients who had PRK or Lasik?

If I understood this correctly why would anyone want Lasik, why not just jump to Len replacement?

LASIK is preferable for people 18-40 years of age. Lens replacement is preferable for patients 45 and older in my opinion. The lens of a 30 year old for example is still optically superior to a lens implant. Therefore, between 18-40 years of age, LASIK is preferable to lens replacement.
