Understanding Ventilator Settings

This is an introduction to ventilator settings like FIO2, PEEP, Flow rate,trigger,TV, and RR. I also discuss how these settings relate to CO2 and O2 control and to complications like oxygen toxicity and barotrauma with an emphasis on physiology. I targeted this presentation to fellow medical students, nursing students and respiratory therapy students. It is admittedly over simplified in places, but I hope you find it useful nonetheless.

I'm a respiratory therapy student and I've always struggled with school work since the beginning of highschool due to needing a more detailed explanation that I felt was hard to get. This video is absolutely wonderful!! Ventilators are so complicated and I just started learning about it in school so this video is helping me a lot!! Thank you!!

sincity RRT home care nurses are more trained with ventilators and other respiratory equipments. Nurses in hospitals get spoiled because they have RTs

I think that ALL respiratory therapy staff members that work in ICU, should be made to watch this video at LEAST twice a year and pass a quiz following the video. Should be MANDATORY. This guy is GOOD!

how were you able to give this lecture as just a fourth year medical student? i mean nothing derogatory, only to state the obvious- 4th year med students have almost no experience with ventilators. were you a CRNA or ventilator rep in a previous job? are you speaking from experience? you seem to be very good at teaching, regardless. thnx.

As a resident, I would like to have seen how we detect and how is the management of each of the 6 complicaions, for example- how do we detect that indeed we have issue with auto PEEP AND how do we manage auto PEEP etc.

This has been so helpful! I am on an ICU placement at the minute and I am finding all of the ventilators and other machines very difficult to understand. You're explanations were very clear and thorough - many thanks for sharing!

When I refer to "setting the sensitivity too low", I mean making the the trigger too INsensitive (i.e. high negative pressure required to trigger a breath). And when I refer to "setting the sensitivity too high", I mean making the trigger too SENsitive (i.e. only a very low pressure required to trigger a breath). I apologize if this was unclear.

Pharmacy resident here, this was by far the best intro to vent setting and complications i seen, better than the RT that taught us. From a pharmacological standpoint, factors that decrease Cardiac Output such as tidal volume, RR, and peep at the higher end can result in altered pharmacological effect by high E drug that are flow dependent, including phenytoin.

Very good info. Thanks sincerely. Any want to answer how to prevent stacking when dealing with respiratory overlap syndrome AKA COPD with complex apnea (heavy emphasis on central apnea events)? Would AVAPS have time controls to address minimizing stacking?

This is an excellent lecture. It should be re-made with minor revisions due to progress in ventilator technology. I feel sorry for the speaker who had to constantly face the bright lights. This needs correction for the remake of the master piece.

Excellent video but the only caveat is the FIO2 setting. You would only set the FIO2 at 100% if the patient's status is unknown such as codes or trauma. Other wise you would set it under 60% FIO2 normally 40-60. In cases of COPD patients you would set it around 28 to 30%.

Thanks! Some of us in IT need to know the basics to understand how to help our clinicians analyze the data and build algorithms. For example, patient stable and then gets worse by x%, but if on APRV setting ignore PEEP values.

for a nursing student like myself who has zero experience in the field, besides clinical, this was extremely helpful. You didn't assume we would know the basics already and explain things too fast, and you were extremely clear with your explanations. Your slides were easy to follow also. Thanks Bill,Your knowledge of the subject matter is excellent. I don't know which I enjoyed best your teaching method or the way you explained "understanding ventilator settings".. No, really your ability to help me understand what was previously taught to me has increased my understanding of ventilator settings. I assume you are a really good Dr. I am a nurse now starting a job in home care with ventilator dependant patients. I will always consult utube for your educational videos. Keep them coming, brilliant!

I have a issue with mine. It is set on IVAPS mode. When I let a breath out, it will stop me from taking one in. Today, my heart started acting up. When I switched back to my old bi pap, my heart issues quit. I don't think this lady knows what the hell she is doing. She can't figure out what to do and in the mean time , I'm chancing my life. When I come off of it, I have more trouble breathing. Can you explain? I wear a mask. Show less

My daughter 2 months 1 day has been diagnosed with Barotrauma only on ventilator less than a month. I kept asking to lower the pressure she has been on 7 Peep majority of the time with rates between 45-50 now they want to up the peep to 8. I disagree please call if you can advise need 2nd option

Hey man great lecture, you really know your stuff! Quick question: When your talking about flow rate and you mention that if the inhalation takes too long, the machine might deliver a breath during exhalation because they "run out of time" since it is a set RR. My question is how does the trigger play into this? wouldn't it prevent the firing during exhalation since there is no trigger during exhalation.

He mentioned that PiO2 is to adjust our paO2 but also the PEEP we use to adjust the paO2.
