I have small black spots, does this equate to arrested carries as they have stayed like that for years without change and there is no pain whatsoever

Yes it could be arrested caries, but it's always better to get it checked by your dentist to avoid any doubts.

Hi doctor first of all thank you so much for your explanation I am a dentistry student and i have a little bit difficulties to classify caries according to surfaces being involved (complexity) also my doctor is saying how can we classify dental caries according to G.V black and complexity together for example he says class 5 complex and also the same for other classes so would you please explain that point ❣️❣️

We are covering Dental Materials Sciences from McCabe. And Yes, all the important stuff that the students need to know is being covered. Currently, we are on Chapter 2 of Dental Materials Sciences from McCabe, which is about The Properties used to Characterize the Dental Materials.

Can arrested Caries become active again ? And are you sure that it cannot be grinded off ???

Arrested caries can/should be grinded depending on the clinical scenario, because there's always a slight chance of it becoming active again.

@@TheDentalistt I have been using GC Flouride Varnish every 6 months as recommended by my dentist and follow a very disciplined dental hygiene and the arrested carries are in the way they are for some years now. Thanks for the information doc 👍

Progression or pathway of caries can be understood by the help of a triangle▼a triangle has a pointed side, which we can call as "Cone" and then the opposite of cone has a "Base" Now when caries occurs on concave surfaces like Pits and fissures, the progression of caries is in this direction ▼and that is why its called Forward caries. This means that the top of enamel surface will have a broader base and as it progresses further into the tooth, it will have a cone shape progression. When caries occurs on smooth surfaces or convex surfaces like Interproximal areas, the progression pathway of caries is in this direction ◄ or ► depending whether its effecting right or left proximal side. The cone is on the outer side and as the caries progresses into the tooth it has base shape progression. This happens when caries progresses in the DEJ area. Caries does not originate from the DEJ area, rather it goes from outside the enamel to inside but it has this shape of progression that makes it look like its progressing from inside to outside, because the normal pattern of progression of caries is broader base outside, and narrow tip or cone inside. But in this case, its opposite, Narrow tip or cone outside, and as it progresses towards DEJ it has broader base inside. Now also note that it is not necessary to strictly have ▼ this appearance of caries progression at the pits and fissure region and it is not important to strictly have ◄ this appearance at convex surfaces/interproximal areas. The point to understand here is that caries progresses in greater speed and in more lateral direction, when it reaches the dentin, than when it is in enamel. And that is the reason that it gives this appearance that caries is extending into the enamel from DEJ, and is then called Backward caries. A lot of people believe in this hypothesis that the dentinal tubules can get infected from pulpal origin and can cause caries originate from the DEJ, but this is just hypothesis and the authenticity is to be proven yet. I hope it has cleared your confusion. And how caries originates in this area, is through plaque accumulation or presence of food debris in the interproximal areas, etc. I believe you know these reasons. Dr does occult caries reach the pulp. I have a slight grey color on my tooth but no cavity pain. It is between 2 teeth precisely on first upper molar. Thanks for the informative video.
