Mild vertigo on turning to one side may be due to a disease called BPPV. If it is really there then there is NO medical treatment for it and it has to be treated by some manoeuvrers in case it is really there. But to know if it is there or not, certain clinical tests like DixHallpike test, Supine Roll test, Head hanging test etc will have to be done by a neurotologist. Otherwise it is NOT possible to know whether it is there at all. However BPPV is a self limiting disease and if really these it usually regresses in about 7-10 days time.

NO these are NOT the symptoms of any vestibular problem. These are cognitive and /or psychiatric problems. Moreover, vertigo is NOT the name of any disease. It is the English translation of 'matha-ghora' and is the mere SYMPTOM of some underlying disease

Phenytoin induces may types of abnormalities but the cerebellum is the most common site which is adversely affected by Phenytoin; this cerebellar damage causes dizziness and ataxia. Unsteadiness dizziness, head spinning, drowsiness, nausea/vomiting, general weakness, are the most common adverse effects of Phenytoin. The serum Phenytoin level in the blood should be evaluated at regular intervals in all patients taking Phenytoin. if blood level exceeds the accepted levels then the dose of Phenytoin must be reduced and /or some other antiepileptic drug should be used. The treating neurologist is the best person to decide on that. Coming to Oxcarbazepine, the most common adverse effects of the drug are dizziness, unsteadiness, drowsiness, headache, nausea, vomiting, vision problems. You must discuss this with the doctor who is treating you for epilepsy. There are may anti-epileptics available and so if your doctor thinks it prudent he may change the drugs without compromising on the treatment of epilepsy.

I am NOT the right person to fix up your dosage of the antiepileptic drugs .A neurotologist like me only deals with patients of VERTIGO-INSTABILITY-DEAFNESS not with epilepsy. Please go back to your original neurologist who has been treating you for epilepsy over the years. He should be the best person to manage your epilepsy with the drug combination that suits you best. But before going to him have your blood levels of oxcarbazepine and Phenytoin measured from a very good and reliable laboratory. If YOU think it prudent you may seek the concurrence from another neurologist later also but ONLY after going to your original neurologist with the blood reports. Wish you a vertigo-free and epilepsy-free existence

@@ChandanaBasak-fp3vy The symptom of vertigo suggests that there is some underlying disease that has involved the vestibular system. To evaluate the vestibular system there are tests called the VESTIBULAR FUNCTION TESTS that need to be CORRECTLY done. Having the tests done by any Tom Dick and Harry (Horidas Pal in Bengali) may be misleading. This is not to demean anybody but to ensure that you are not mislead. However the patient first needs to be evaluated CLINICALLY and a detailed history taken by a person specialised in the treatment of neurotological disorders (specialist in balance disorders) and then that doctor will decide which specific tests (vestibular / imaging/ blood/ psychological / neurological) are most ideal for that particular patient. From the history it appears that the patient you are mentioning possibly had a disease called Benign Paroxysmal Positional Vertigo followed by a Phobic Postural Vertigo. But without seeing the patient and clinically examining the patient the diagnosis will remain conjectural and NO definite diagnosis can be made on the basis of a two line history. You need to have a very very thorough history taken and thorough assessment of the vestibular function done so that the underlying disease is first diagnosed. Logical treatment can happen only after that.

@@AnirbanBiswasneurotologist ধন্যবাদ sir.reply জন্য ।। Sir.. আমার মা এর ছয় মাস আগে হঠাত্ করে সকালে বিছানায় বিশাল বিশাল মাথা ঘোরা শুরু হয় তারপর বমি ও করে।পরের দিন হসপিটালে নিয়ে গেলাম ঘাড়ে x-ray r matha ct scan করলাম ডাক্তার বললেন ঘাড়ে সপনডো লাইসিস হয়েছে এর জন্য মাথা ঘোরা । কিন্তু ঘাড়ে কোন ব্যথা নেই । এটা কি সম্ভব । আর মাথা ঝাকুনিতেও মাথা ঘোরে বা মাথা ঘোরার ভয় পায়।আর মাথাটা সবসময়ই একটু বা দিকে কাত করে রাখে সোজা করতে বললে সোজা করে কিন্তু ভয় পায় যদি বেশি মাথা ঘোরা শুরু হয় । আর ঘাড় কাঁপে ।। আর মাথা ঘোরাটা 1-3-4 সেকেন্ড থাকে। but এখন বমি করে না বমি ভাব টা হয়।আর হাটাচলাও আসতে করে যেন মাথায় ঝাকুকি না হয়।আর চিৎ হয়ে শুলে মাথা ঘোরে বা একাত ও কাত হলেও সম্ভবনা থাকে ।। এই লক্ষণ গুলি দেখে প্রাথমিকভাবে আপনি কি বলবেন । ঘাড়ে সপনডো লাইসিস হয়েছে নাকি ভারটিগো ।। আপনার reply ar অপেক্ষাই রইলাম । ধন্যবাদ sir..।।

No Madam, cervical spondylitis is not usually rather never the caise of vertigo. At the worst case cervical spondylitis may cause a sense of HEAD ATAXIA which in layman's terms means a feeling of the head not being stable on the shoulders. But that too is very RARE. Typical head spinning practically NEVER occurs due to cervical spondylitis. Moreover, cervical spondylitis will cause remarkable shoulder pain and stiffness of the shoulders. In aged patients each and everyone WILL DEFINITELY SOME RADIOLOGICAL EVIDENCE OF CERVICAL SPONDYLISIS as cervical spondylitis is a natural age related change like graying of the hairs. The cause of HEAD SPINNING is something else. Seek opinion of a neurotologist if available in your place. Some tests may be required to ascertain the underlying cause.
