Just outside the Royal London Hospital entrance, there should be a Spy. I found one there around the evening at 14:40. If you don't know where the Royal London Hospital is, open up your map and go the Towlet Hamlet Borough, it's North East on the map. Zoom in and you should see a large building, that is the Hospital.
