I had a labrum repair when I was 17, and it was absolutely the right decision. I dislocated my shoulder in a bicycle accident, and after I popped it back in, it would dislocate again almost daily when putting on a shirt. It's been 25 years now, and my shoulder is rock solid and hasn't hurt since the surgery.

@Abhinav Nirvan mine did. Fortunately it happened at a young age and my muscles hadn't fully developed yet. The only long term side effect I have is a slight decrease in the range of motion in that arm. With my arm bent at 90 degrees, I can put my hand behind my head, but not much further back.

Excellent video. I've watched 15 other videos without finding one that explained the labrum structure and associated tear categorisation in a way that made sense. Advice on appropriateness of surgery was really helpful.

Thank you! I've had tears on both anterior and posterior labrum on my left shoulder in 2018. It gets better but heels very slowly. My Ortho asked me to leave it alone. Ive learned to live with it.

@@PERUANO31 mine was super painful for 2-3 weeks. Sharp pain. eg reaching for the seatbelt.. but is decreased a lot within 4 weeks to the point where i didn't go back to the surgeon.. in the end 6 yrs later i have trouble putting my shoulder in a certain position as it would set off muscle spasms that would go up my neck and clamp pretty good. chip ups or monkey bar type of position while holding body weight would also set it off. but i was back surfing within 2 mths. still feels like there is something wrong but no pain.

@@abhijeetsingh2361 My shoulder is fine. I only have pain when holding my arms up for longer than 60 seconds but it's nothing unbearable. I stopped weight lifting and focused on calisthenics, added light lifting after a while. No surgery needed for me. But i do have dead tissue that healed incorrectly.

Going on 3 yrs now gor me with a labral tear. Osteoarthritis and complete rotator cuff tear. Mosy orthopedic surgeons wont touch soneone with ehlers danlos. My shoulder now just hangs outside the socket. Very painful and good luck sleeping. Finally found a dr who is doing something about it.

Just found your video and it is well done. I injured my shoulder playing high school football, arm tackle and then another player was behind my should creating a fulcrum- remember it like it was yesterday. Don't recall what I did to get it better but wasn't really an issue for a long time except for range of motion- example, it was difficult to lay on my stomach with my hands under my head- caused pain in the left shoulder joint. Well as I've gotten older, almost 54, for about maybe the last 5-10 years, I would get sharp pain in weakness when doing something while the should is in a certain position- like pushing on a door, opening car door, lifting something. Ironically, I have been going to a trainer and exercising more the past 16 years. A couple years ago my wife and I started taking the BiFlex for joints and it seems to help. But my shoulder will be fine and then I'll do something to tweak it and then it's touchy for a few weeks or so. The pain and weakness are only momentary until my should drops- no pain otherwise at any time. The other week I reached out to pick up a glass of water and the weight of my arm was too much, my arm dropped, and I had to use my other arm. Any thoughts- sounds from what I have seen on RU-vid- it may be a rear or top labrum tear. But again, I've been living with it- and like the old joke about the guy who went to the doctor for pain when he moved his arm a certain way- the doctor said "don't move your arm that way" lol, and that's what I've been doing. Sorry so long- thank you!

Ah man i had that extreme weakness when i tore mine too. Went from lifting heavy in the gym, like 110 lb dumbells for overhead presses for 7, 8 reps to super difficult to do 25 lbs for 5.

Great video! Could a labral tear cause the shoulder to sit more rotated and forward? My brother injured his shoulder a few years ago(anterior dislocation, popped right back in but he heard a tear like velcro) but in the past year he has noticed progressive weakness, swelling and numbness in his hand. His shoulder still sits slightly forward and scrunched in and he's starting to having trouble using his hand to write, etc. He also hears a kind of popping and grinding when he moves his arm in certain ways. Any thoughts?

When you say you can live with it for a long period of time does that mean it will eventually cause osteoarthritis and potentially need to be replaced? I already have some osteoarthritis due to bot getting my bankart lesion fixed with the biceps tenodesis. If I get those repaired now will I prevent the osteoarthritis from progressing? Of If i ignore it will the osteoarthritis get overwhelming to the point of a replacement? Thank you Dr.

For me, the problem with my SLAP TEAR is that the shoulder pain keeps me awake at night when I lay down and turn in my left shoulder. It's unstable and I wake up in pain. No one should live with chronic shoulder pain when surgery can correct it.

Same here! Left shoulder SLAP tear, left side sleeper! I had to switch to the right side now and use more pillows. I put a bed pillow behind my back so I don't roll completely onto my back, and I tuck a couch pillow underneath my Left armpit so that my arm will lay flat on my left hip and not roll forward. As long as my shoulder doesn't fall forward the pain isn't so bad. If it gets to be too much I have to take a sleeping pill or I'll be in immense pain all night! I'm sorry we have to go through this!!😢😢

@@juliannewman8720 wassup bros jus seeing this message!. playfighting with my big brother (6’3 280 pounds) bear hugged me n literally heard a loud tear. It sucks man I had to be in a brace for the first 6 months of the year. I couldn’t even brush my teeth or take a shower on my own cuz it was dominant arm (left shoulder). I’m now recovering from surgery it’s been 2 months now after recovery n I am in way better shape than how I was trying to heal without surgery! I use to wake up in middle of the night crying actual tears cuz jus simply me laying my head on my shoulder would cause so much pain. Now that I’m in recovery I finished two months of therapy motion is coming back I still be sore if I pick up something too heavy or put too much pressure on my shoulder I will feel it in the morning but man I tell you i am way better and happier after surgery vs jus trying to make it heal on its own . U will suffer badly trying to thug it out go n get help!!. Also get a tennis ball put it on wall n press the back of your shoulder on the tennis ball n rub it hardd. That will release some tension in your shoulder.

Thanks very much. I had an inferior shoulder dislocation and there isn’t as much info on it as the other shoulder dislocations. As a result after seeing an orthopedic it was determined that my rotator cuff and my nerves were unharmed. However it was confirmed that i injured my labrum (likely a slap tare). I did not get an MRI.. I’m amazed that i honestly have very little pain (it’s been three days since the dislocation). I feel soreness when reaching above my head. I’m hoping for a full recovery with shoulder stabilization exercises and strengthening my rotator cuff muscles. For not I’m just resting and icing the shoulder.

i have issues in my right shoulder starting a year ago, due to some reasons I have not had any treatment on it... altough it's a bit better but it is still bothering me.... (i'm 47 years old) symptoms: At the beginning (a year ago), 1: i felt weakness on my right shoulder, i used to bench 145kg but I could only bench 120kg then and during the movement I felt like my shoulder was about to come off... i could finish couple reps but my shoulder would be so sore for days.... then i had to down it to 100kg, i could do it with better after condition, but my right shoulder would feel tired in the middle of the round. 2: when i tried to reach my left shoulder with my right hand, my right shoulder would feel so much pain 3: i just ignored the pain and did regular work-out, then my bicep, trapezius, tricep, infraspinatus were alll very sore 4: i tried to do Y raise of arms with 6kg dumbells, clearly i felt weakness on right side (my left could not 10 reps but my right could do only 4 reps) now (a year later): 1: i can now bench 120kg~125kg, but somehow I still feel some weakness on my right 2: my bicep, trapezius, tricep, infraspinatus are constantly sore... and very tight.... this has been giving me hard times.....so annoying 3: for couple of times i had to raise my hand to put something at high places... at some angle, a cramp would suddenly appear on right shoulder....now I am kinda cautious everytime I raise my right arm... 4: when I do the chest-fly machine in heavier weight...i feel my right shoulder is like a bit coming off.... sorry for long words.... would like to ask if i was like in need of a surgery.... really thanks...

I tore my right when I was 51 had it repaired and after a year it was pain free. Now 10 years later I’ve torn my left one and it feels like a hot ice pick in my shoulder when I rotate that arm behind me like when you put on or take off a jacket. I don’t see living with this the rest of my life.

I'm recovering from surgery right now (typing with 1 hand even). Was heavy lifting upwards at an inline and my shoulder dislocated backwards. Awful initially but felt fine back in the socket after a few weeks. It could easily fall out of the socket when my arm was up for a moment. 2 years later, my shoulder kept aching (regular gym usage in that time). Got it looked at and opted for surgery. Pain when healing isn't nice but it's bearable 3 days later. Feels like my arm is held together with paperclips at the moment. I don't know how long it'll take for me to go to the gym again but very single day feels a lot better. I'm 28 yrs old.

Hey there, just read your comment and wanted to ask how is it going now since it's been 3 months since your comment? I am about to undergo the same surgery next week, I didn't dislocate my shoulder but I must have hurt it somehow (during lifts or playing football) but I kept doing regular exercises for months (it pained very less at the beginning so don't even know when it started). I just want to know how recovery looks like in 3-6months period, are you going to gym and stuff? I am same age as you as well.

I have a shoulder labral tear and a cyst on spinoglenoid notch and measures approximately 1.5 X 1.4 x 1.3 Cm. I was given a cortisone shot for pain and it has relived 95% of the pain, but the cyst is definitely pressing up on my nerve cashing severe pain when i didn't have cortisone. Doc suggested shoulder surgery but i cant do that RIGHT now. seeking to try Ultrasound-guided cyst aspiration... then we can talk surgery but i'm cannot do surgery right now.

I just found out that I have a slap tear when they are checking out my proximal bicep tendon tear. I do kettlebell sport which is all overhead movements. Would leaving the labrum torn interfere with doing overhead exercises? It sounds like it would. I'm almost 50 but still workout a lot and want to continue doing so. Thanks for the info. I'm slightly less freaked out about it now lol

I am 53 and have had pain in my right shoulder blade going on 3 years. No known injury, no accident, no sports, no falls. Repetitive motion and postural issues taking Blood pressures as a stress tech. Finally got someone to evaluate my shoulder. I have a SLAP tear and tendinopathy of the supraspinatus tendon. My trap is like a rock and sooo painful. Can this just happen without injury?

I had a dislocation with labral tear and Hill-Sachs 10 months ago from an accident with a mountain bike, took me few months to get back into lifting without surgery but this week I got a dislocation again from playing basketball. I am now considering surgery but can I skip it until next year? Thank you.

Hello Doc, big fan, I have hypermobility joints and 3 dislocation(Bankart Lesion) on right and 2 on left mainly due to rugby and my just profound condition , I am now 21 and have decided to retire from rugby, I am not fond of surgery, can I know whether the shoulder labrum tear can heal on its own by time? I know that the Hip does not heal, how about the shoulders?

I'm scheduled for surgery because the pain is just not stopping. I guess bowling did me in. Its a bicep and labrum slight tear but interior to posterior cysts and arthritis i think is where the pain is coming from. I'm hoping they just can clean that out and not reattach my tendon.

Thanks for the video! Great info. Is there any advice you can give on the anterior labral healing naturally? Maybe with a sling? How long would you have it time to heal before it has passed the healing window where someone may need surgery? Any advice would help. Thank you

Fort someone who did tear their anterior labrum, many live with it torn as long as their shoulder feels stable and does not hurt. If it hurts or keeps dislocating then surgery is needed.

About 2 yeas ago I was standing on a hill and my dog knocked me down, I landed on my right shoulder. My pain is in an arc from mid to the back of the shoulder. After that the back of my shoulder and down my shoulder blade started hurting. It pops out sometimes a little and I have to put my arm up then jerk it backwards real fast. Then my bicep started cramping up and around my elbow. That took about a year to go away but now I can't use my hand for fine motor skills. My writing looks like a kid did it cause I can't grip it. It always starts with a numbness in shoulder, a slight ache then good bye use of my hand. My doctor said, "probably a bone spur" and that was that. So 2 years later here I am not able to turn, twist my wrist, grib pencils etc... it is just going down hill slowly. I figure soon I wont feel my arm at all or be able to use it.

I had a slap tear 10 years ago from heavy benching , I wasn’t able to any overhead or pressing work for a couple months after the injury then slowly over time I’ve been able to add it back in . I never got the surgery because it’s not the end of the world to me if I can’t press heavy weights any more which is the only time it bothers me these days . I can still press moderately heavy 315 range for reps but I can’t do it for consecutive weeks without my shoulder feeling like it’s gonna come out of the Socket . There’s some clicking and grinding that wasn’t there before the injury but all in all it only bothers me when I’m wreckless with it . Also I’ve had to use a much closer grip on everything since the injury as well . A wide grip would cause a lot of pain

@@IldanMagadiev sorry for late reply . I do hv a channel showing what I’m doing . I basically hv to take most of the pressing movements in pretty close grip . I try and transfer a lot of the stress into the lats and triceps . Hope your shoulder issues improve. I know that’s a tough one . I hv my good and bad days with mine

I was working with about the same weight 325 for reps and it happened on 2nd rep ..I'm 55 & it seems I'm getting a different injury every 8 months but only if my bench numbers start to climb ..As your injury is,I'm getting better week after week but wonder what's gonna be next injury

I was benching 315 for reps on the third rep my right shoulder just instant jelly. Bar dropped on my rib. Right side . 3 days ago. Now not really painful. I have muscle. But I cannot lift arm . Loose shoulder. Not painful. Just can't move it up or out. Not sure what exactly I'd going on. Getting mri. But it takes weeks to get. My shoulder is unstable. I was able to hold 5 minute handstand and monkey bars traverse rapidly. All that stuff out the .windows. I need surgery to get the thing fixed. No way rehab gunna get me walking in hands again.

Suspected Circumferential labral tear, small volume Hills sacks deformity, small volume superior sub scapular is tear., adhesive capsulises Shit happens. Fell 30 m through trees. Injuries from trying to grab tree branches on the way down. 60% compression fracture of T 9 and shattered talas. Ankle is good. T9 healing pretty well. Shoulder is painful mostly when I’m lying on my back. 12 weeks since the fall. Trying to do this without surgery. So far so good. But circumferential labral tear sounds pretty significant. I’m 60 in good shape. Was doing Bulgarian ring dips for reps before accident. Physio says no to surgery. Surgeon says yes. I’d be grateful for your thoughts 🤩

The shoulder tear itself shouldn't affect the nerves, but what can happen sometimes is that the trap muscles get really tight in compensating and can affect the nerves. Therapy can focus on thoracic outlet syndrome

Hey doc- i have a small posterior tear. Starting PT this week. Not sure how it happened. One day i woke up and had mild pain. Is that common for these (where it seems to come out of nowhere)? I lift weights once and a while and do boxing a few days a week. Is boxing (hitting pads) putting me at further risk?

Hello Dr. Paull I have a tear on the large superior labral and also tear of the superior aspect of the posterior labral. Partial tear of the tendon for the long head of biceps. I'm having pain for about eight months and finally I got the MRI report. I will receive cortisone shot to help with inflammation if that doesn't help my doctor told me that I might need surgery. My question is all these tears can heal or these tendons and ligaments are not able to heal since they don't have much blood supplies in them. Thank you.

Mine is cussing severe pain. Dislocations (i have ehlers danlos to begin with) and. Severe shoulder weakness praying for surgery. My dr thinks i also have a full rotator cuff tear.

PRP and Stem cells are likely just very strong anti-inflamatories. They may make it feel better but I don't know that there is data to show that it will get it to heal back down

Liked and subscribed. Not what I wanted to hear (on labrum tear number 5) Here’s a question. With should immobilization, is it possible for a labrum to reattach itself to the bone? Specifically posterior tears? What about going through the motions post surgery, without actually having the surgery? Planning on taking to my ortho next Friday about options.

Labrum tears normally don't heal, but they can become asymptomatic, meaning that they are still there, just don't hurt

35 yr old male here....i have a torn labrum and supraspinatus in my RT shoulder, as a result of severe deformity of the posterior glenoid, which is severely deficient. There is advanced glenohumeral degenerative changes with diffuse joint space loss and moderate humeral head osteophytosis. I've had 3 osteo surgeons review my case including a specialist/fellowship. Everyone is scratching their heads with no idea how to fix the shoulder. Have had MRI imaging done, now waiting on another and a CT scan(healthcare here in Ontario is a terrible joke) hard for you to say without seeing imaging i know, but any ideas of what to do. Been a year now since ive been able to get my arm above 60 degrees. have been doing PT for it but with no results

I believe the surgery is the last thing you should do because I personally had one before and the shoulder still partially dislocated several years later. Training and stabilizing is the answer in my opinion.

Hi, question, what happens if after bicep tenodesis, an MRI shows truncated appearance of the superior labrum? could I relate this to the continues and extreme pain, weakness and being unable to move my arm up, sideways or back since surgery?

I have a SLAP tear and I have issues with subluxation when I reach across my body and above my head in certain spots. Apparently there are also cysts present inside the shoulder. Contemplating surgery

Hello everyone! So I elected to get surgery back on September 28th. The plan was to just give me a bicep tenodesis in hopes that the bicep tendon being relocated would help put less stress on my labrum. Unfortunately while they were in there they noticed my labral tear was much worse then they anticipated, so they had to repair the labrum as well. Recovery was OK, if you stay on top of the pain meds the first 3 or 4 days you will be good. Sleeping at night is by far the toughest part, just so darn hard to get comfortable. I have 2 more rehab appointments this week before being released, I'm probably at about 75% back right now. Still have some weakness in the back of the shoulder but with continued exercises I should be good. Hope this helps

Hi doc I'm 23, i had a shoulder pain back in 2months i had a MRI and the MRI shows a possible hill Sach's lesion and antero inferior labral tear - possible soft tissue bankarts Do i need a surgery? I've connected with nearly 6 doctors and the doctors are like 50-50 some suggest surgery and some not. What to do in this situation, can you please assist me here!

So I tore my pec and in the MRI a Posterior labral tear and was shown. The doctor believes I had that tear already and was asymptomatic since I did jiu jitsu. Now after the pec surgery I feel the posterior tear when doing push ups or bench pressing. If I want to return to combat sports is it necessary to repair the tear ?

@@TechnlcalTechnlques I’m holding off on surgery because I need to go back to the police academy. I rehabbed and did stabilizing exercises for the shoulder but do not plan on going back to combat sports until I get it fixed. For me it’s too risky. I would say just get the surgery if you can. The pain is not the worst but I definitely feel it and it’s not going to get better without surgery.

@@rubenperez5895hey that’s what exactly happened to me. I torn the tendon of my right pec when I was doing dumbbell flys on the bench. I got surgery to reattach my pec tendon and now I’m in PT. My chest is ok but my shoulder is weak because aside from the chest tear the was a tear on the labrum as well. I think I need surgery because the PT exercises are not working and I’m afraid that instead can worsen my shoulder.

Stem cells are experimental at this point so there is no good data. Reputable injections of stem cells will take the cells from your own iliac crest

Mine went untreated for a year, having general practitioner telling me my pain was from my bra. It got so much worse as time went on where I cant even hold a remote control. I was diagnosed with slap tear, age 54, but when they did the surgery found out my rotator cuff torn along with bicep was detached. My shoulder pops out of socket just picking up my phone. I had one repair, I am almost a year out but back to the same pain! I was told at the surgery it would probably be a temporary solution…PT hasnt done much and I am allergic to NSAID…have type 2 diabetes unstable and steroids are a no no at this point...along with a meniscus tear in my opposite knee. I have osteoarthritis and random bone spurs that are breaking off in all my joints. I just cant take the pain anymore. I do also have Alkaptunria which led me to having a THR at age 40. I haven’t gone back since re-injurying it opening the curtains a few days ago. I guess its Time to go back.

Hi doctor Great video. If a 58yr old has a posterior tear due to sleeping on stomach and has normal strength exam with some pain. Is it right to assume that it should be managed conservatively with nsaids shots and physical therapy to control the pain with surgery being a last resort if pain is unbearable? Thanks

I started back up twice because of my surgeon and my physical therapist had no idea why my surgeon had me still going when I wasn't making any progress. I was actually told by my surgeon to take prescription pain medication before PT which is ridiculous

I have these same symptoms right shoulder it's been a little over a year it has gotten so bad I can't use my arm at work I have not seen a doctor about it self diagnosis no insurance I'm gonna have to go to cnty.
