I just got back from my first Mediterranean cruise on Royal. I have to agree with you the vibe was very different. I had never met/seen so many rude guest ever. They pushed passed you to get on the elevator, stand to the side to get in front of you at the buffet, move you stuff to sit at a table even if the seats are taken. I was so taken aback.

We’ve never booked with MSC Lines and to be honest,I’ve have never had any desire to book with them. Yes it seemed to be the safest option. Now for certain ports where there is something nearby (like Ballaro Market in Palermo), I’d definitely do that again without an excursion. But those destinations 1-2 hours away, I’d do a ship excursion.
You made some really great points, and things we didn't even think about! When it's time to take our first Mediterranean cruise we'll be prepared with all the tips and advice you shared in this one. I agree with you for the most part but your "minority" issues only apply to specific European cruise lines as MSC. If you go with NCL, Royal, Princess, or HAL you will be fine.
Unfortunately they all weren’t. We only had one bus that had a lift. I’m not sure if they had separate buses for those needing a wheelchair/scooter lift but our buses were not all accessible. For those who can climb a few stairs, there was storage under the bus that a scooter can be stowed. It sounds like ADA laws are in the works over there and should go in to effect in 2025. Not sure if it will look like the US or not.
