There is a lot of new-ish information about endometriosis over the last few years - including that it’s not the endometrial tissue that is displaced throughout the body, but tissue similar to the endometrium. And that hormone suppression merely masks the symptoms and surgical excision with a highly skilled practitioner is the gold standard of treatment to get rid of the endometriosis lesions and have better outcomes. For anyone suffering with this debilitating disease, Nancy’s Nook is a great place to get accurate and up-to-date information on endometriosis treatment and management.

Yes! I had several laparoscopies with ablation of endometriosis and also a partial hysterectomy, all of which failed to stop my pain. I found a doctor who does excision surgery in 2015 and haven’t had any issues since. Endometriosis is similar to a weed in that, if you merely cut it down (burn it) it will probably grow back. You have to excise (pull the root) for it to truly be eradicated. Anyone living in TN or close to Chattanooga, please check out Dr. Furr.

Your comment re: treating women without a uterus experiencing premenopausal symptoms with an estrogen/progesterone combo is the question I’m after! I am currently 35 and had a partial hysterectomy almost 10 years ago for endometriosis. I still had pain afterwards and in 2015 I had excision surgery (Dr. Found several lesions remaining even after my hysterectomy!) and haven’t had endometriosis issues since. I went off of birth control a few years ago thinking I didn’t have a need for it and three years later, I am experiencing low estrogen and premenopausal symptoms, including hot flashes, joint pain, fatigue, low libido, and vaginal dryness. I recently obtained HRT from Alloy, which is estrogen only (because Drs around here won’t prescribe HRT to women under 40, and my Gyn surgeon who did my last endo surgery and would likely provide HRT is too far for regular treatment); however I feel that I am still lacking a libido and have very low energy. I also noticed that whenever I have been on oral birth control in the past I have been able to maintain a healthier weight, which seems impossible now and I am the heaviest I’ve ever been. I hoped HRT would help with ALL of that, but it hasn’t. I’m trying to research if the combination of estrogen and progesterone I was taking previously with birth control is a better option for me at my age and stage of pre menopause than the HRT with estrogen only I am currently on. I’ve been on HRT about 9 months now and am contemplating returning to oral birth control to see if it helps more (and because HRT is too expensive to not fix all of my issues).

I had endometriosis and had really bad periods and was extatic when I finally stopped them, I haven't had one for over 5 years but my menopause became so bad that I started hrt and now I keep spotting. I have tried different ones but it always happens any ideas would be appreciated

Had a hysterectomy 10 years ago for severe endometriosis, still have ovaries. Now perimenopausal. I’m scared to take estrogen alone. But, oral progesterone causes much GI/digestive issues. If I use progesterone vaginally, will it be as effective to balance out estrogen and avoid triggering endo pain?

Thanks for the great education Dr Hirsch. Wondering if you can do a video on endometrial hyperplasia in perimenopause/menopause please? If one still has a uterus are an IUD or hysterectomy the only options, or can the progesterone pill be enough to offset estrogen?

My was so bad my appendix was covered and bladder and kidneys and I had a appendix rupture 😢 my My tubal clips were covered also and one clip came undone ! I got a total Hysterectomy ! I was told I couldn’t have hormones

I also had severe stage 4 endometriosis . I had a hysterectomy and currently have been on HRT ( hormonal patch ) . I have been doing great . Good luck !

If you can, try to find a better gynecologist! Their info is outdated at best if they’re denying you hormones due to a history of endometriosis. My surgeon kept me on oral birth control after my endo excision and it worked great for me and I was in it for several years after and never had any further issues. I also had a hysterectomy for endometriosis and was told by multiple doctors there was “no way” I could still have endometriosis, even though I was still having pain. If you’re still having endometriosis issues, definitely look into Doctors that perform “excision” surgery. It’s the only way to truly get rid of lesions. Anyways…I stopped birth control about three years ago and started having premenopausal issues within the last two years. I signed up for HRT on MyAlloy because I wanted to try actual hormone therapy and didn’t think I needed progesterone like with birth control. However, now I’m questioning if the progesterone also helped my premenopausal symptoms. I’m considering switching back to birth control. BUT, if you want to try HRT and don’t have any progressive Doctors in your area who will treat you with hormones, MyAlloy is an option. Its all done via telemedicine without having to see a doctor. There are also online apps that prescribe birth control the same way and are usually at no cost with insurance aside from the initial $20-30 online assessment.

⚠️ if someone has endometriosis do they hurt in pain all the time like everyday or is it only during a period or during fertile week including ovulation day❓

Everyone is different. Some women suffer everyday and some like myself are cyclic flares. I’ve had flares since my 1st period at the age of 12. It took doctors over 30 yrs to diagnose me with stage 4 endometriosis. I’ve lived a life of horror as the pain has been debilitating for 2-3 days every single month for the past 35 yrs. I’m in peri menopause and wished I could take HRT but like she mentioned, many of us are just traumatized and horrified with the pain we’ve had to endure. I wouldn’t wish that type of pain in my worst enemy, ever.

I have had pain everyday for 39 years with endometriosis it all depends where it is found and how close to nerves and if adhesions have formed also scar tissue from surgery

Sure! It is also not a contraindication to HT. Most of the time, cysts are benign and resolve (go away) at menopause

Heather, can I use 100 mg of micronized progesteron pill vaginally? Will it protect me from endometriosis ? I am also on estrogen patch 0 .75 mg but recently my breast became very sore! Should I decrease my estrogen and how low should I go? Thank you!

Yes, breast tenderness is a sign that the estrogen dose is too high. Progesterone can be used vaginally in this way!

The incorrect definition of endometriosis is given, it is not a uterine disease and the tissue found within the lining of the uterus isnt't the same. Patients should be referred to an endometriosis excison specialist for proper treatment. The gold standard of treatment is excison surgery. Recommending Lupron is negligent, and I highly recommend you do more research and look up Dr. David Redwine, and Nancy's Nook for Endometriosis Education. Medical therapy does not treat endometriosis. The disease will continue to progress even when using the medical therapy.
