What Is Metabolic Syndrome? How To Check For It.

Have you ever heard of metabolic syndrome? It’s something you should know about if you take antipsychotic medications. All of the antipsychotic medications increase the risk of developing metabolic syndrome. Some do it more than others.
Metabolic syndrome is not a disease, but it’s a set of risk factors that increase your risk of heart disease, diabetes and stroke.
Here are the five risk factors that constitute metabolic syndrome.
1. Increase waist circumference Men greater than 40 inches
Women greater than 35 inches
You get this number by using a tape measure. You start at your hip bone and wrap it around your waist at the level of your navel.
2. Increased blood pressure of greater than 135/85
3. Triglyceride level greater than 150 mg/dL
4. HDL cholesterol less than 40 mg/dL in men
less than 50 mg/dL in women
HDL is considered good because it absorbs excess cholesterol and carries it back to the liver for processing. So high levels of HDL actually lower your risk of heart attack and stroke. This is why having a high total cholesterol isn’t necessarily bad. It depends on how high it is. But if you have a high HDL, it’s going to raise your total cholesterol. LDL cholesterol is considered bad because when you have too much of it, it can deposit itself on your artery walls and create plaques that narrow your arteries. This contributes to heart disease and risk of stroke. So you want to keep you LDL as low as possible.
5. Fasting blood glucose equal or greater than 100mg/dL
This is a measure of how much glucose or sugar you have in your blood when you haven’t eaten anything for at least 8 hours. Insulin is the hormone produced by the pancreas that processes the glucose from the food you eat.
Glucose levels
Normal = Blood glucose under 100mg.
Prediabetes = Blood glucose of 100-125
Diabetes = Blood glucose greater than 126mg/dl
According the American Heart Association, you need 3 out of five of these to be diagnosed with metabolic syndrome.
What makes this happen?
There are medical conditions that can cause metabolic syndrome, and obesity is a common one.
What does it mean to be obese?
Obesity is defined by your body mass index (BMI).
BMI = weight in kilograms/height in meters2
There are handy calculators online that make it easy for you to input your height in feet and inches and your weight in pounds without having to convert to metric units.
BMI Ranges
Normal - 18.5 - 24. 9
Overweight - 25 - 29.9
Obese - 30 or greater
There are many things that can lead to obesity. But in psychiatry, the antipsychotic medications we use can cause a lot of weight gain and lead to metabolic syndrome.
A 2017 article ranked the antipsychotic medications in order of weight gaining tendency.
The three causing the greatest weight gain were:
Clozapine
Olanzapine
Chlorpromazine

I've gained 80lbs in a year, since starting antidepressant/anti-anxiety meds. Being overweight makes my depression and anxiety worse.

Ive gained nearly 50 pounds while being on Zyprexa. My weight gain has certainly put a damper on my self-esteem. There really isn't anything I like about myself anymore

You need to be very aware of gaining the weight at the beginning of taking the meds. I knew my meds would cause weight gain so I weighted myself everyday. I gained about 10 pounds so I’m OK with them because the meds helped me.

I gained fifty pounds on Latuda. When I realized it was the medication I asked to go off. My psychiatrist said no and blamed me for getting fat. I switched doctors and went off it and lost twenty pounds right away.

A LOT of medications are to blame for weight gain. Doctors are ignorant or irresponsible to not inform their patients. Which is why WE need to keep ourselves informed and when a Doctor doesn't care about us..........get another one.

Thank you so much Dr. Tracey. While on Olanzapine, my triglycerides increased, my cholesterol, my weight and I developed Insulin resistance, pre Diabetes and also suffered from anxiety binge eating. That was years ago, but I still suffer of all the above, I'm on Metformin and Rosuvastatin, my cholesterol is controlled but since I haven't been able to recover my weight, even when I consume around 1200-1400 calories per day. My blood pressure is the only thing that is ok. I wasn't overweight before I was prescribed Olanzapine. After all these years I was able to lose only 8 kg of the 15 kg I increased while on Olanzapine. I don't know what else to do, I eat super healthy, I cannot reduce the calories any lower. I hate that my doctor never told me the risks and never took any responsibility. This has worsened my treatment resistant depression.

@@chillie000 Besides not really having the energy to do it because of all the mental health issues, I have developed a condition called lipoedema in my upper arms and I realised I have had it since puberty on my thighs, but since I was thin it was assumed to be "pear shaped body" by my own family (it runs on families). That fat tissue cannot be burn, not even with exercise. Fortunately I have type I and IV only, I'm used to type I and it doesn't hurt yet, it only makes me look weirdly shaped and more overweight, but the type IV, upper arms, it's really affecting me. Unfortunately the only real solution it's surgery and it's very expensive since it's considered cosmetic. So I just eat anti-inflammatory food, wear certain clothes that helps and hide myself from people I used to know. Thank you anyways.

I just had an annual exam and my primary care physician said my blood work show I’m considered prediabetic now and I’m on Seroquel. I gave my blood work results to my psychiatrist and she may lower my dose soon. First I’m going to try changing my eating habits and exercise. I have massive cravings of junk food, especially at work. But Seroquel works well for my insomnia and auditory hallucinations. My mood stabilizers are Lamictal and Lithium and for anxiety I take Gabapentin.

Good video. Most psychiatrist write the script and drop you off at the bus station. Gaining weight just makes your situation worse. Pretty much offsets the benefits of the psychotropic meds. I'm not saying meds are useless but therapy absolutely needs to be used if for nothing else than to get you out of the house once a week.

Thanks to you, I found out I was gaining weight because of Quetiapine. I am bipolar and I thought I was driving crazier when I ate less but still gained weight. Now that I saw about this metabolic syndrome, I will have my blood checked as well

I love that you talk about the risk/benefit ratio of these really effective drugs. I’m wondering if you could do a video on tardive dyskinesia. Fear of TD is the main reason have avoided taking APs, even though my psychiatrist thinks they could help my bipolar spectrum depression.

The five signs👍 A large waist. Carrying excess fat around your waist, in particular, is a large risk factor. A high triglyceride level. Reduced HDL or “good” cholesterol. Increased blood pressure. Elevated fasting blood sugar.

Thank so much for this video Dr. Marks, I learned so much from it! I'm a male bipolar I patient that's on zyprexa for 15 years now and unfortunately I have all symptoms of metabolic syndrome you mention in this video. Zyprexa worked wonders for me when it comes to positive and negative bipolar symptoms but I gained about 120 pounds over those 15 years and now that I'm almost 40 I feel like I'm getting first diabetes symptoms too. Started taking Mirtazapine and Quetiapine 1 year ago and promptly gained 30lbs. I’m only 5 feet tall so it’s a huge amount for me. I recently quit the Mirtazapine and started rigorous Intermittent Fasting along with a strict Keto diet. No exceptions, no giving in to the ravenous food cravings caused by the Quetiapine, but it’s been 12 weeks now and I’ve barely lost 5lbs! 😖 *All* my blood work, including Thyroid and fasting glucose rate are totally normal. This is SO frustrating and depressing because Quetiapine has really helped my insomnia. My psychiatrist is now suggesting Trazadone instead, does this sound like a good option? Desperate to lose the weight!

I was on quetiapine also had weight issues but also extreme joint pain. I found out about trazodone and was so happy I changed over. My sleep is so much easier than before. Granted everybody is different.

@@Contessa998 it’s the only atypical antipsychotic I’ve used in the last 11 years that has worked for me but keep in mind that every one and their chemistry is different! Also, keep in mind it took me 11 years to find a good match. Find a doctor that listens to you and keep a daily journal of your moods and symptoms etc to help find the right match and pick up on Med related patterns.

Great info, came across your channel recently as part of me looking for answers. I have been struggling with ADHD for so long and In the last 10 years, I have been exposed to traumatic experiences and slowly started noticing a shift in symptoms like episodes of depression and some others all of which became more of a concern after learning about bipolar disorder and ADHD. added to that aftermath after the losing of a very beloved one which I believe added a layer of anxiety attacks. I’m on mood stabilizers and a stimulant for few years. It looks like meds aren’t working effectively. I really appreciate it if you could address and guide me on how to approach my therapist and doctor with this because I have this fear of being hospitalized (not that I believe I need it from what I’ve understood watching your videos) but I’m not sure how doctors approach treatment options. Especially that passive dark thoughts have been surfacing during panic attacks episodes. I do see the need to talk about it but how can I make sure that a doctor will exhaust everything possible before making that decision. Maybe tell us about that procedure and what hospitalization looks like? What are patients rights and is it an environment that we see in movies? Thanks a billion

I was out on 2.5 mg of Zyprexa for my treatment-resistant depression just over a year ago. At the beginning of 2020, I weighed a fairly healthy 165 pounds. As of February 7th, 2021, I weigh 215 pounds! Sometimes I'm not even hungry, I just want to keep eating until my cravings are satisfied because eating is all I can think about most nights. Even after eating dinner, I'll eat granola bars, cereal, ice cream, popcorn, and drink lots of diet coke. I know my eating is a result of the Zyprexa and I'm currently trying to get off it. I'm just so irritated that I've gained this much weight because I feel like I have no control over my eating habits while on this medication. I just got a new psychiatrist and he ordered a blood test to check my triglycerides and some other stuff so I'll be interested to see what they say.

I was on olanzpine for a little over a year and gained over 50lbs. I’m on latuda now and it doesn’t make me gain weight, but it feels nearly impossible to lose the weight I gained. Constantly going to the gym and eating healthy (for months at a time with little progress) and eventually my negative symptoms and lack of motivation outweigh the motivation to lose weight in the first place

Thank you for this video, I've been wondering about this topic. I was on Quetiapine probably 9 years and I've been off of it for 3 years. I also took abilify for a few years and have been off of it for several years. I currently take other medications but I'm not taking any antipsychotics, however my belly size has gotten huge and was also large before. Almost all my extra weight is in my belly. Can you still have metabolic syndrome even years after you stop taking these medications? Thank you!

You can if you keep the weight on. But remember even if you are overweight, if your blood pressure is normal and lipids are normal it’s not metabolic syndrome

I was just put on Serotonine (fluoexetine) to help with depression, since I am on it (2 months ) I have gained a lot of weight which I have never struggled with. The leaflet said it should cause weight loss, not weight gain. Do you think this sort of medication can also cause metabolic issues or cause my carbohydrate cravings? Ps. She mentions fasting glucose levels but not "fasting insulin" which is very important. Mark Hyman MD (functional medicine doctor) calls the "Fasting Insulin Test" the most important test your doctor doesn't order. The body could be working overtime pumping out insulin to keep your blood sugar in an acceptable range. You might be on your way to diabetes for 10-15 years before a problem is detected by the fasting glucose test.

Hi Steph. I have a video coming up updating people on where we are with psilocybin. It’s still a schedule one drug which means doctors can’t prescribe it. Therefore we don’t have a treatment protocol for it yet.

If people think they have this condition, it’s important that they have a blood test called fasting insulin. Some of my symptoms were dismissed until I had a blood sugar of 2.6 come back and a fasting insulin of almost 400. (This was after 5 months of heavy Prednisone use.)

Very informative. I would add, along with the qualifier that psychiatric care was defunded after most of these asylums were closed, that although many patients "regained their sanity," treatment strategies that followed patients' progress out in the world and additional options like CBT were not pursued. Perhaps because CBT had not become an accepted part of psychiatric training until some time afterward. I think it was a little misleading to laud the advent of an antipsychotic, without also pointing out that people are still ill even while taking anti-psychotics and were in many cases simply turned out onto the street at the time.

True. People don’t respond 100% to antipsychotics. And there is still a lot of morbidity despite having antipsychotics. But they did make a huge difference in the care of people who were psychotic.

@@DrTraceyMarks Agreed. My point is not so much about the drug, but about the closing down of asylums without any adequate replacement at the time. Many mentally ill people were just "set free" without support (not that they were getting much support INSIDE the asylums before the anti-psychotic appeared, as you pointed out). The situation I am drawing attention to was not the fault of anti-psychotics, I don't mean to imply that. These drugs did and do make a huge difference for the better. It is the shortcoming of our government's mental health management at the time, do defund mental health and close asylums down with no plan to replace them with something better. At the time.

I was on Olanzapine and gained 30 kg. A few years ago we changed it to Invega. A year ago I learned that I have IGT, got Metformin, and was put on a low carb diet. My blood sugar is getting better, but my weight stays the same. It’s very hard to accept. :(

Thank u for this video, Dr. I just got started on Zyprexa for sleep. But ive already stayed up for 2 days and the have energy but then boom, i couldnt close my eyes to pray or i will fall asleep. Im already chunky because of the fibromyalgia meds. Will zyprexa cause me to get bigger? Also, does it affect the memory? I can't remember what is being said to me within 5 minutes. Is it due to meds or lack of sleep?

Sometimes people can feel like they’re thinking it’s cloudy with that medication and that could affect your memory. And yes zyprexa can cause a lot of weight gain

LDL is harmless in and of itself. The body needs cholesterol, or else it wouldn't go to the trouble of making it. High LDL in the context of low HDL and high triglycerides indicates insulin resistance, which will kill you, but high LDL in the context of high HDL and low triglycerides is associated with improved health, compared to low LDL in the same context. The really scary lipid, which blood tests typically don't directly report, is remnant cholesterol (the part of total cholesterol not accounted for by HDL and LDL).

I take Quetiapine for my insomnia and never knew they would result in weight gain. 😮 I started out thin, then I was put on anti depressive medication for around two years and gained 40 lbs during that time. I stopped the anti depressive medication but still took Quetiapine and have a hard time losing it on both diet and exercise. From now on I will no longer take it. No way will I stay this weight if I can lose it if I stop taking it.

DrMarks I stopped olanzepine 2 years ago but cannot lose weight. BMI is 30 but my waist measurement is v high. Is it possible that the olanzepine has permanently affected my insulin resistance?

It may have and that is a good reason to have your fasting blood glucose checked. But even with a high blood glucose you can still lose weight with a radical change in diet and exercise

I have been taking Olanzapine since 2004 and only since April 2019 that started to put on weight gain. My public psychiatrist checks 2 times a year for my sugar levels and I had 2 public psychiatrist since I came from Chile to Western Australia for the second time in late June 2005 for my bipolar Disorder and I have a private one for my ADHD. I can't take Ritalin like I did when I was diagnosed in Chile in 1990 because I have Bipolar Disorder now since June 2004 and I take now for my ADHD the Strattera (Atomoxetine). I have tried in Chile when I went back to see other Neurologist for my ADHD but he said that they need to treat my Bipolar Disorder first. So Since October 2004 when I went back to Chile until 3 Days before coming to Western Australia (WA) in late June 2005 I got given a box of Strattera by a private psychiatrist and I when I came to WA I got given the Strattera on an off until a Private psychiatrists prescribed it for me since August 2013. However going back to the weight gain I have taken Risperdal (Respiredon), Invega (Paliperidon)and Solian (Amisulpride), but because it increases the prolactin levels I can't take them. I have also taken Abilify (Aripripazole), Seroquel (Quetiapine), Lithium, Sodium Valproate and Tonaril (a drug I took in Chile for my restless legs). However the only 2 drugs that have ever worked were the Olanzapine and now the Sodium Valproate because on the last 2 occasions I could not take it because it affected my red and white cells and since the new public psychiatrist that I have since November 2007 and she tested me again late 2020 and I can now take it for good and it makes me feel a lot better than the lithium that I was taking. So Thank you Tracy Marks for this informative Video. Just 2 more. I have also taking the Cylert (Premonile) medication also for my ADHD and taken also Zoloft (Sertraline) for a panic attack that I had in 2003. Second Check the work of Susan Pierce Thomson from Bright Line Eating (google her name) because she explains that the reason why people put on weight is because people eat those refined foods that are not available in the wild like sugars, flour, pasta, pizza because they make you want to eat more and more food. That is why I am trying to explain why only since April 2019 I started to put on weight on the Olanzapine and not since 2004 when I first started taking it. I was born in 1984. Thanks You.

BTW I forgot to mentions that I do not have diabetes and my glucose levels are always fine. The public psychiatrist checks many things like lithium, olanzapine, liver, thyroid (I was operated in Chile for it), Kidneys, liver and Prolactin levels. She checks for about 8 different things 4 times a year but not all of them 4 times a year with a blood test and she has the results from the old test too. Thanks you.

I used to risperdal in my teens and I became allergic to it. I took abilify for 6 years. It stop working for me and I took geodon, I just had gave birth and had a baby and I was put on geodon., I was not able to function it made me too drowsy and sleepy. I'm on latuda and Zoloft. I believe the Zoloft increase weight gain. I just started taking it so I'm not sure right now.

I was put on rexulti 2 years ago, and I would literally black out and come back after destroying the party sized pack of oreos. Every night. There was no resisting bc I didn't even know what I was doing until I woke uo the next day with an empty package. It was so scary, and my metabolism is just starting to catch back up to where it was before I started the med.

I thought BMI only told so much of the story re: obese or not. Especially if you are very active, because muscle has more mass (weighs more?) than fat. Is that why you mention using the other methods to help determine if you have metabolic syndrome?

Yes if you notice BMI is not one of the risk factors for metabolic syndrome. I mention obesity as one cause of metabolic syndrome but there are other causes

Thank you Dr. Tracey. I am on respirodon and I did gain weight. Like you said the cravens for more food are real. My solution is to have a water bottle in the fridge. I haven’t lost any of the weight but it starting to stop the gaining. My last blood work showed higher levels as well. I have talked to my doctors about it. I am hoping one day to be off them but I know for now they help.

Is Trazadone included? I used to take Seroquel and my weight went off the charts. I switched to Trazadone (bipolar 2). It's listed as an antidepressant, but works like an antipsychotic. I CANNOT lose weight!

I was hoping to get a question answered not related to the video I’m very paranoid and it can be very extreme and I notice that it can be extreme but then I think to myself if I doubt myself I may get hurt I have generalized anxiety disorder and always been paranoid any time someone stares I think they are gonna hurt me if a car is behind me for to long I think they are following me my main thought is that i will be murdered kidnapped or hurt by someone could this be something I should address to a professional

Ive seen it used very off label with mixed results. Some do the behaviors less while others get a lot of side effects without much benefit to the compulsions.
