What is Mixed Mania and How Do We Treat It? The official term for mixed mania is bipolar disorder either 1 or 2 with mixed features. With bipolar 1 disorder, you have episodes of depression and episodes of mania. With bipolar 2, you have depression and episodes of hypomania.
For more information on Bipolar Disorder, see my Bipolar Disorder Playlist here: goo.gl/1hKamL
Mixed features means you have a combination of depression symptoms and hypomanic symptoms occurring at the same time. Mixed mania is harder to treat than bipolar disorder without mixed features. In this video I discuss some of the treatment options including electroconvulsive therapy (ECT) and transcranial magnetic stimulation (TMS).

Mixed episodes are hell. A mixed state is what sent me to the hospital. Before that I was manic for about a month towards the end I went into psychosis. I thought I was crazy and then I went into a mixed episode. I have never feared myself as much as I scared myself during a mixed episode. My mind raced with horrible thoughts, depersonalized, saw demons, and in the end I tried to commit suicide because I felt the shell that I was living in didn't matter to me and was foreign. I wouldn't wish mixed episodes on anyone. Mania; I like, Depression; I can live with, but Mixed? Pure hell.

Hey, I saw your comment in here and hope you're doing better now! I currently experience a mixed episode, too, and feel like I'm literally going crazy because I can't contain the massive energy I have and at the same time it feels like I'm drowning and everything is just too much. How would you answer the question "What's bad about mania?" to someone who doesn't know anything about it? I would like to explain it to my family, because most of the time they see my manic episodes by seeing I'm really creative and productive solely which is far away from the truth as well as doesn't reflect the consequences/effects of a manic episode to me.

Mixed episodes are the WORST thing I have experiencing. You have strong anger, sudden anxiety and sadness. When you are depressed, all you can do is lay down but in mixed episodes, you have the ability to take your own life. I rather be depressed than being in a mixed episode like right now.
@crayzmosher

I agree, I’m in a mixed episode rn and it’s so miserable. I have harmful ideas and my depression like normal says “nothing I do matters” but with mania added to it I think “nothing matters so I might as well do it” instead. Like it’s depressive ideas that I carry out and it’s been getting kinda scary lately. like I’ll go out at night alone when where I live isn’t safe for me to do so, and when I’m stable I’d never ever do that. I say weird shit that pops in my head without thinking and it’s harming my relationships. I’m either completely isolating myself from people I love or talking too much and making them uncomfortable and worried. I miss my friends so much, and them being distanced from me only makes me feel so much worse

I hope you're doing better by now. My only suggestion is to find spiritual peace. Practice detachment in meditation until you realize that all those symptoms are not you. Growth and strength should follow adversity. I know my depression wasn't helped much by 3 or 4 different meds. What helped me the most was getting the message from the suffering. Nowadays, when it rears it's ugly head around, i might just think: huh you again?

This description brings back memories. I had a very severe mixed episode with mania and depression in 2005, and continued to struggle with my bipolar disorder for years afterward. I was hospitalized three times in three years for it. But there truly is hope for anyone dealing with this illness. Because I've not had a manic or severe depressed episode in a very long time. With faith in God, solid medical treatment, supportive people in my life, a good licensed counselor... Bipolar disorder doesn't have to ruin your life, although it is a force to be reckoned with.

I am not a mental health professional/dietician/personal trainer. I cannot advise you on what is best for YOU. This is just what has worked effectively and in a lasting way for me. Things that have helped me cope with hypomania: 1. Drinking plenty of water to the point where I generally dont experience extreme thirst or dehydration at all anymore. 2. Preventing hypomanic episodes in the first place by having a plan in place for when I recognize an episode building. 3. Neutral affirmations. Example: "I feel very up and high right now, and that is caused by a chemical process in my brain." Or, "It feels like a good idea to move to Virginia with no plan, but what if I, instead, planned a camping trip there? It's not the immediate adrenaline hit my brain is craving, but it's an outlet for it." (I have to be in a stable or baseline base to be able to come up with an array of mental phrases I use to slow down the mania/hypomania/mixed mania.) 4. Yoga, slow stretching for at least 15 minutes, and walking slowly. These are methodical and flowing exercises. They are also not fast-paced, which for me can really ramp up my hypomania/mania/mixed mania. 5. Recognizing what "parts" of me I choose to define myself with. Figuring out what I like and not just what mania/hypomania/mixed mania "tells" me I like. 6. Eating a healthy diet. With lots of fruits and vegetables. And probiotics to help with stomach issues caused by the anxiety/mania. And fish oil with omega-3's to help with the rage. Not eating processed sugar (this is a really hard one for me but an enormous help with the insomnia.) I drink tea instead of coffee and then switch to herbal tea/water/hot cocoa/homemade lemonade after 3:30PM. 7. Routine, Routine, Routine. I'm 28 and just now accepting this one. But it's been one of the most important parts about keeping myself feeling balanced. Things That Have Helped Me with Depressive Epsiodes: 1. Setting 5 alarms with very gentle, nice music to wake me up at the same time every day. 2. Starting my day with tea (no enormous, debilitating crash like after coffee.) I still like coffee every once a blue moon. It just hurts my stomach. 3. Letting and allowing myself to be slow-moving and slower to wake up than when I was manic. Reminding myself that it feels even slower than what is actually reality, because of the contrast of the two episodes. And that's okay. 4. Singing my favorite songs even when my voice comes out in a monotone and the colors look grey. I'll cry-sing in the shower, in my room, in the kitchen. Anything to keep my voice going. 5. Medication. Talk to your doctor. If you don't have a doctor, gets doctor if you can. If you can't, make it a priority to try to see someone if you feel that is something you need. 6. Therapy, therapy, therapy. Talk therapy! Cognitive behavioral therapy! Any kind of mental health therapy! Make sure you give the person a chance, don't expect immediate results, and don't miss your appointments. Forgive yourself when or if you do miss one, and explain to them the REAL reason why you couldn't pick up the phone. You gotta be honest for therapy to be as effective as possible. 7. I know everybody hates this one, but forcing myself to do things when all I want to do is let sleep embrace me and give me an escape. I always feel worse after I take a depression nap. If I drink some extra tea and drag my body to do something, anything, wash my face, brush my teeth, it doesn't matter how long it takes. I always feel a tiny, tiny bit better afterwards. And then it makes it easier to keep on doing things. 8. Calmly and rationally (or sometimes, admittedly, not so calmly-which doesn't work as well) debating, reframing every single depressed thought I have. Asking it what it's intention with me is. Protecting myself from the harm they cause me.

It’s hard to tell the difference in me between having a mixed episode or just having a depressive episode with irritability and extreme anger and needing to move around and full of nervous energy, and this being caused by a different comorbid disorder causing these symptoms like BPD or ADHD. Hopefully when I finally get to see a doctor they might consider that. I miss mania and feeling good, or even depression which I can control. I just feel this intensity but I don’t know in what emotion, and this need to move and racing thoughts

I was originally diagnosed with bipolar disorder in 2009. I was prescribed Zoloft when they thought I was just going through post-partum depression but it made my symptoms worse and during a bad manic episode I drove all the way to Michigan(where I was born and raised and most of my family still lives)fromFlorida. My ex husband's family were the ones who saw the signs and encouraged me to get treatment. I did and was put on Meds that made me even worse. I was prescribed Seroquel,Trileptal,Buspar,and wellbutrin. I was a literal zombie. Fast forward to 2014 and I was again diagnosed as bipolar but with being type 1 with mixed episodes. I went through one episode where I was suicidal and homicidal at the same time. My mind was racing and yet my body couldn't move due to the depression. I have found sleep is the best cure and medicine. I still have bad nights but I force myself to sleep and when I can't I let my boyfriend know that I'm not doing well. Sleep is the best drug for bipolar. I couldn't sleep until almost 5am this morning,which is never a good sign,but having a partner who understands is very important as well. I haven't taken a drug since 2011 and am able to manage my bipolar by knowing and communicating when I'm going to have an episode. I refused to be put on Lithium.

Mixed episodes are truly uncomfortable. You could’ve have never said it better than “hyped up sad person”. I remember during mixed episodes, I am so productive at work, attending meetings and getting all things done, talking to all my colleagues and really just above it all, but all the while, I just want to break down and cry because of this gnawing sadness and emptiness. My mixed episodes are usually followed by depersonalisation/derealisation episodes. Is this usual? I am very careful about these since this is what eventually drove me to be hospitalized. Thank you for this video.

i know its a year later but anyway. My case is similar to yours; my mixed episodes are with hypomaniac and drepressed mood, i have energy to do a lot of things during the day (and at night) but im not happy, i just wanna give up and even die. I can be productive at work, be more communicative and sleep just a few hours, but at the end of the day i just feel depressive, even sometimes i wanna cry but with my meds i cant. Its really hard, because my family and friends seems me good, but im not ok and i just feel sad and misserable.

I'm a big fan of ECT. After many years of ineffective treatment of bipolar disorder, I asked about ECT. It worked wonders. There is a powerful effect on memory. You can have amnesia, usually for events right around the treatment. The memory returns after time, though possibly not 100%. In my case it was fine. I did forget what I had named my cats, but I figured they would ignore me no matter what I called them. The memory returned in a day or two.

Some might say that belief in “God” is a delusion. We are born without choosing to be born, we face nastiness and vicissitudes of this existence, and then we drop dead anyhow! At best, life will be filled with comfortable distractions. On the whole however, existence mostly sucks, and we’re probably all better off never having existed. Belief in “God” is just a societal construct and a delusion to give meaning to that which is ultimately meaningless...

A thousand years is but an instant. There's nothing new, nothing different; same pattern over and over. The same clouds, same music, the same things I felt an hour or an eternity ago. There's nothing here for me now, nothing at all. Now I remember, this happened to me before. This is why I left. You have begun to find your answers. Although it will seem difficult the rewards will be great. Exercise your human mind as fully as possible knowing that it is only an exercise. Build beautiful artifacts, solve problems, explore the secrets of the physical universe, savor the input from all the senses, filled with joy and sorrow and laughter, empathy, compassion, and tote the emotional memory in your travel bag. I remember where I came from, and how I became human, why I hung around, and now my final departure's scheduled. This way out, escaping velocity. Not just eternity, but Infinity.

My mother went through ECT. It sounded barbaric at the time. That said, it was COMPLETELY controlled. In her case, they had a cardiac team there, on stand by. They were not needed, but it helped us to know they were there. She experienced some memory loss for a bit of time. That was not permanent though. It was the ONLY thing that worked for her.... then. I got my mom back due to ECT. Now, 30 years later, i have no hope she will get better. She's been in the hospital and several nursing homes for the last 16 months. The doctors tell me that she is "stuck in a manic episode. Not likely to change." She's 75. To me, no age should be considered too old, to enjoy life. It hurts, very much, to watch others give up on her. She is currently on no psychotropic medications. In the past she had Depekote. Is geriatric bipolar treatable? She, like many, has other medical issues, requiring medications. I am sure that that complicates the matter greatly. Are there things i can do that would help? Covid, of course, dictates no visitors, AT ALL. Bipolar is ALWAYS hard for families. Helplessness equals hopelessness, and frustration is the bubble that keeps it all together. Bless you for sharing your knowledge. You make it easy to understand..... which is a rare gift. It is said, that there is power in knowledge. That's also where courage, strength and hope reside. Bless you❤

Seroquil was definitely miserable... that constant numbness was awful. not to mention when you are on it during a depressive episode you still have that melancholy feeling all the time. It's like depression where you can't cry it out to feel better... instead you are just numb. It permanently warped my depressive episodes into an apathetic mess and ever since my cycles have been scrambled and have all had more mixed features. I never thought I'd say it... but feeling unable to cry makes it so much worse.

A doctor here in Honolulu would not treat me with TMS for my depression because I am bipolar 1. He said Blue Cross/Blue Shield (my insurance) would refuse payment because my depression is part of Bipolar disorder. They will pay only when it is major depression not associated with bipolar. Frustrating because my sister had TMS and swears by it. She said her doctor simply changed her diagnosis. She is in Kansas City so it is what it is....she had done ECT quite a few times and her insurance company there dropped her. Her new insurer came thru for her but probably didn’t know about the Bipolar 1 diagnosis. ( I think). Main thing is she got the TMS and it worked great-she is off all psych meds and doing well. Excellent video Dr. Marks.

I remember telling my doctor that my depression is the worst part of my mental health issues, and made it clear I knew I suffer with mixed emotions. It’s an internal fight of emotions, and I once found it confusing, now I have an understanding, it doesn’t make it any easier 😢I always felt like wanting life to end, but had this irritability, anger and hypersexuality and impulsivity leads to bad decisions. And I can see it being a leading contributor to bipolar suiciceds

When I get mixed features it's usually between a manic and depressed episodes. When I'm depressed I'm just super said, hate myself and want to die. With mania I'm just super anxious and crawling out of my skin. When I think of mixed features. I think about when I'm sad, hyped and become extremely angry over things I have little to know control over. Btw thank you for your videos. It has helped me become educated and somewhat identify what's going on in my mind and body.

I really appreciate your mentioning ECT in such a level way. I'd been struggling with treatment-resistant Bipolar II with mixed features and, two years ago, at 19, 'all else' did fail for me and I ended up doing 18 rounds of ECT (unilateral.) It sort of set the 'reset' button on my life in more ways than one; my memory was affected to a degree, but all of the sudden it felt like someone had taken a giant weight off my shoulders. It was like I had been living in black and white and someone had just flipped a switch that put the color back in again! The treatment isn't perfect or without side effects but it's sure effective, and it saved my life. I wish people knew that about ECT-- it may be considered a last resort, but it is there, and because of that there is hope for even the toughest cases like mine. I know someday I'll probably have to go back for maintenance and I don't necessarily look forward to it, but I'm still so grateful that it is there. Thanks for presenting it in such a fair and honest way!

I’m so happy, my therapist and psychiatrist always asked me what my pattern was. I’d always say I’m inconstant. I would sleep 12 hours a day then the next 5 hours of sleep. Some days I would sleep for 5 hours because I’m restless but talk myself into sleeping because I’m also depressive. A super high in a negative sad space. I’m on lamotrigine, but I really want to try the shock therapy! Lamotrigine helps in depressive and manic episodes but when I feel both at the same time it’s just confusing. It’s like the lamotrigine doesn’t know what to do lol.

This is great. During a mixed episode its almost like hypomania and depression are struggling for supremacy and there are days in which one will "outmatch" the other resulting in a seemingly disparate display of symptoms. A friend described it as vibrating on the inside, but in slow motion on the outside. I know how to recognize and mitigate the depression for the most part (success rates vary) but don't really know how to mitigate the mixed episodes as they tend to only happen approximately once per year. Smaller sample size = less ability to test approaches. I have found that the transition from summer to fall is a reliable trigger for mixed episodes, winter after the holidays triggers depression, and the transition from spring to summer triggers hypomania which I can only attribute to the changing of seasons/amount of daylight hours, and the changes in my sleep and activity level. I'd love to see a video for the spouses and how best to recognize and support those of us who manage our diagnosis with minimal assistance.

I’m diagnosed with bipolar 2 with only mixed episodes. The more I look at your videos the less I understand. You haven’t brought up what I struggled with most at all. The first thing is how exhausted you are for sleeping very little, I throw up every morning because I’m beyond tired. The mind is racing so I don’t get any rest at all that becomes 100 times worse when you’re taking medication when I took Lithium I it became so bad that I was hospitalized. The elevated mood is an insane form av stress that keeps you from sitting still. You don’t talk about the sensory issues either (that’s always there though has been all my life) The stress (especially with medication) makes you don’t want to live anymore. After a few days I collapse into severe depression.

Hi Dr Tracy, thanks so much for another great video! Very informative. Unfortunately, I also suffer from bipolar 2 and mixed states are the worst! I was initially perscribed olanzapine which worked fantastic but I gained a ton of weight. Strangely, when I switched to Seroquel I was able to loose the extra pounds. I do notice that I get a bit hungry after taking my nightly dose, but nothing like the ravenous bottomless pit I was while taking olanzapine. I'm glad that my medication is working correctly but still struggle to accept the fact that I'll likely be taking it for the rest of my life. I hate taking medication of any kind and like to deal with things naturally. I am working on accepting this fact and not viewing it as a defect in my character or an immature ability to control my emotions. It's difficult but I have to keep reminding myself that I've gone off my meds on several occasions and thought "well I can just use my willpower to control myself". As you can imagine, this always ends in a huge meltdown. I just have to learn from these past mistakes. Thanks again for your videos, I really enjoy them!
@DrTraceyMarks
@DrTraceyMarks 5 лет назад
Hi Joanne, thanks for sharing your experience. It's so hard to deal with the self-talk and talk from others that your symptoms are a mental weakness and you just need to push through it. It takes some work to get past that and not judge yourself for needing a medication for your mind. Once you do, you can really have some periods of stability such that the illness doesn't ruin you. As for taking for the rest of your life...sigh...that's a tough one. Some people who have long interepisode periods can manage without meds for a while, but I think that's a minority of people. I have a future video answering the question of can you stop your meds. I'm still editing it. Stay tuned.

I wish I heard this video wayback in college. If I do I know it's likely I'm getting the info I needed to get the help I need. I might not have ruined my life, and myself. I'm still suffering but right now is that I'm getting educated why I act so different from other people. I experienced being roughly mistreated, discriminated, and bullied from schoolmates, coworkers, even from friends and some relatives. And sexual pleasures have became a way for me to escape severe depression. But i became destructive, having tantrums, having elevated mood swings, and I can even hardly get the sleep I need even I know I needed to go to sleep, sometimes i sleep 5am or even 7am because I don't feel tired at all. I'm trying to overcome when my mania comes but sometimes I can't control and sometimes I'm trying to lessen it at least but there are times it comes back again and again. I love your videos and I think they're really helpful.

I'm 55 and was diagnosed with mdd in my early 40s. I had 1 prior depressive episode about 36. I also have GAD and PTSD (I'm a veteran) I'm going through something confusing to me. I'm up almost all night. I'm tired but restless . I spend a lot of time playing games on my phone and feel anxious if I can't. I'm spending more money than usual and have a hard time focusing on anything for too long. I feel a lot of frustration, worry, empty, irritable, and like I'm going nowhere fast. My provider suggested this is a mixed episode. I've never had mania before. I have an upcoming appointment with a psychologist for a through mental health screening. Up until now, all I have experienced was deep dark depressive episodes that was treatment resistant, and I've been hospitalized a few times I'm scared of what it means if suddenly I'm diagnosed with bipolar disorder. This is a very uncomfortable feeling, but I'm not nearly as hopeless as when I'm in a major depressive episode. I feel stuck in neutral and going nowhere at a high rate of speed. Most of the time, I just don't feel well mentally or physically. This is affecting every area of my life.

tracy, if you experience symptoms of mania in the morning but symptoms of the depression every night, every day, for 12 days... is it mixed? I had this time last month I was like this and on top of all that I had psychosis. I didn't sleep for 3 days and that was my limit. but I wonder if it was only mania or... the hell it was. I am bp1 with psychosis specifier. on top of that. I only had 2 episodes on the entirety of 2019 so I don't cycle daily, monthly or in a short period, besides these cycling was under an antidepressant. but that episode that lasted 12 days was really weird.
