I've had a lanai procedure but have issues and probably need to go back to the dentist. I may have been a bad patient by accidentally brushing. I've had issues since and feel my #19 needs to be extracted and an implant placed after its healed.

Thank you for explaining that the main thing with those precautions is to not move the gum from the new position. I had pinhole surgery 2 days ago on tooth 25 and 24. The doctor said he never had to work on such thin gum and he seemed to really struggle during this procedure so I was really worried that it could tear. I had the first check up today and the doctor was happy with it and seemed kind of relieved. It was planed that I get an inlay on tooth 19 but the inlay was not delivered on time (this was the second attempt after the first inlay was not placed properly). The doctor said we should do this next week. I would not have been able to open my mouth due to swelling today, but also hope that one week after pinhole surgery will be enough. I'm worried that the gum could move. Also tooth 25 needs to be done again in about a year, because the doctor said he wasn't able to place the gum as high as tooth 24. I get it, the gum there was teared due to an accident about 15 years ago, so I'm glad he was able to lift it at all. Hope the gum stays in place, I'm trying my best to follow the instructions, even though it is not easy.

You're welcome. I hope you heal well after the procedure. The lower anterior is the most difficult area to perform pinhole surgery. @@VeryNiceSmileDental Thank you very much, hope so too. 5th day after surgery and today I recognized a white coating on the gums starting at teeth 28 and 21, but didn't look further to the front since I should not look. Hope that is part of the healing process and hope there is no need to be worried. Also saw a little teared gum at the edge at tooth 27. Hope this will not be a new beginning for another recession. Greetings from Europe!

@@VeryNiceSmileDental Thank you so much! It is healing well. I still don't try to pull my lips down, but I could see that the gum is far way up than it was before so I'm really relieved. Also no inflammation or what so ever, I'm glad.

Update: It's 4 weeks post op, I followed the instructions precisely, but sadly the gum on the front bottom teeth dropped, didn't hold up in the new position.

Two weeks ago i had PRF pinhole on upper jaw. They made me a new bone with sticky swiss bone, they put collagen stripes and my blood plasma. After 10 days my gums look beautiful! Yesterday i had the same thing on my lower jaw. I am now healing and hoping that it will also be great. 🤞🤞🤞

I got the pinhole procedure done about two weeks ago: two teeth on the sides, and my two front upper teeth. I smiled today and saw the gum line of my front two teeth - will this affect healing or push back the gum line? I'm having a ton of anxiety about it, and I'm worried I've messed everything up already. Any feedback or guidance for trusting the process?

Thumbs up! Thank u for this video, I was considering one fine day in the future to do pinhole surgery due to my gum recession because of aggressive brushing in the past. I am very very careful now but the past damage has been done. Seeing the list of don't dos...errr... kinda puts me off. Won't not brushing and flossing for 6 weeks cause cavities Dr?

Hi, you can still brush your other teeth, just not in the area of the surgery. Also, you are prescribed Peridex rinse to use twice a day while you're healing. I haven't had anyone develop cavities during the 6 week healing period.

@@VeryNiceSmileDental I see! i have gum recession on my sides where my canine all the way to the molars are, upper and lower jaw. that is a whole lot of place, and brushing teeth may be a bit too risky. Thanks for your kind reply.

I’ve had two gum grafts and a pinhole…none of them worked and if anything I think they made it worse. Been following all instructions too. This has been going on for months. So fed up.

Hi, eat soft foods and use a knife and fork to eat. Don't bite into things likes sandwiches, fruits, etc.

I’m 6 weeks post pinhole. They did all my top teeth. He said to brush lightly but didn’t give me a soft toothbrush like I thought he was supposed to. I bought one and have been brushing. Is that ok? Is flossing gently ok? He said it would be but there’s so much conflicting information out there is so hard to know what’s right!

Hi, at 6 weeks after surgery, you should be able to brush and floss but use a very light touch and an ultra soft brush. What caused your gum recession in the first place?

A combination of clenching at night (I now have a nightguard for the past 10 years), and brushing too hard when I was younger. I’m only 34 and never thought I’d need a procedure like this until much later down the road.

I had the pinhole procedure on my two canines and one of my first molars five weeks ago that was succesful, but I've noticed that a small space has formed in between my central incisors directly posterior to the gumline, and I'm concerned my central incisor interdental papilla has receded slightly since the procedure (no issues with recession on the tooth before), could this be due to the procedure? If so, how and do you think it will go away?

Why isn't the gingiva sutured in the new position to prevent any receding to the previous position? Doesn't that help the patient?

Hi, this technique was developed to avoid using sutures. There are other surgical techniques that use sutures, but there can be other complications with using sutures.

3 teeth one mouth out. Unfortunately this procedure hasn't worked as well for me 😞 but aside from the continued swelling I've experienced, the procedure itself was painless

What happened? Was the recession not corrected? After a month the remaining swelling is most likely due to the collagen strips that are used to hold your gum in place. It will go down as your body breaks down the collagen strips. Thank you very much for watching.

@@VeryNiceSmileDental 2 of the 3 teeth have begun to receed by 2mm. The area this was performed probably isn't the best for great coverage. I did require 2 stitches immediately. The swelling varies day to day. Odd. No s/s of infection

@@VeryNiceSmileDental 26,27,28. Mind you #26 had about 7mm recession prior to procedure. Regarding the stitches for possible tear- I didn't ask so I'm not sure why. During the consult it was agreed that a traditional graft using alloderm would be done. Why the day of surgery she decided to do the pinhole is beyond me.

@@ryan041984 Lower anterior teeth (tooth #26 is a lower right lateral incisor) are the most difficult place to do pinhole surgery. The gum tissue tends to be very thin in that are for most people. In that area I usually perform pinhole treatment on all the lower anterior teeth (from lower canine to lower canine) to free the gum enough to move it up high enough. I hope that makes sense.

I just had gum graft on 2 front middle lower teeth. The gum was recessed from havung a peircing in my youth. Im 44 now. They numbed me up alot which hurt a little. Then they procedure took 25 minutes. There was blood but I didnt feel anything. The pressure and sounds are not fun. None of this is fun. It sucks actually. I went to cvs after to get medicated rinse and pennicillion to protect from infection. Im home now laying in bed 3 hours after. Pain has settled in and its pretty shitty. I can tell eating is going to be next to impossible the next few days. Im scared to move my mouth at all cause i want this to heal. Im going to bring a que card with me to work saying" had gum graft, cant talk" i think ill use a sick day tommorow and rest

I have receding gums but I truly dont know if I want to do this surgery or not because of the high fail rate I read online. Of course, people are much more likely to complain if the surgery didn't work out for them. However, it leaves me with a completely unclear idea of what the true succes rate could be. :(

Hi, off the top of my head, pinhole has a success rate of 85 % ish in the proper situation. But like all gingival grafts, it depends on a lot of factors for success. Biggest is, what caused your recession in the first place?

@@VeryNiceSmileDental Thanks for replying! It is 100% due to because of brushing way too hard. It makes me sad cause all I wanted was to make my teeth more healthy and now it backfired with recessed gums. I also think my enamel has somewhat thinned because of it. It is a huge mistake I regret in my life.... I drastacally changed the way I treated my teeth. Now my routine is: 0) Drink alot more water throughout the day to keep salvation level high. 1) Message my gums with some toothpaste on my index finger to get a good blood flow in it ONCE a day. 2) Flossing my teeth gently, with the zig-zag method ONCE a day. 3) Brush my teeth TWICE a day as lightly as possible. Let the bristle just touch my teeth. Focussing on the gum line (as that is where most bacteria hold). 4) Use mouthwash ONCE a day, but don't rinse with water afterwards. Furtheremore, I am a non-smoker, 27 years old, no croocked teeth, overall living a healthy live (no drinking and exercising dialy). The recessed gums are the lower canines and 8 molars. The molars are regressed the worst. Maybe 4mm, The gums betweens the affected teeth are not regressed. Furthermore I never had any real issues with my teeth, no fillings whatsoever and going to the dentist every 6 months. I am planning to do this surgery in about 2 years. Do you think I am a good candidate for this surgery? Do you have any advice what I can do in the meantime to make the succes rate bigger?

You might be a good candidate for the surgery, hard for me to say without seeing you in person. Do you grind your teeth or clench your teeth? That can cause recession as well. A nightguard will help. Before you get the surgery it's important that you have control over what could cause the recession. Although sometimes it's just genetics.

@@thomasm3236 Im in same situation , overbrushing, and I cant decide what to do about it, my recession is about 3.5mm currently I want to stop it from getting worse. I am thinking of the pinhole or just bonding at this point, but worried about the recovery process and if I have to go through all that just to have failure of the procedure. If I go through it and it is a success I wouldnt mind any recovery procedure, but the failure rate is what has me worried. I just dont want to lose teth or destroy my smile.

I paid 6500€ for pinhole surgery in Austria/Vienna and lost all my money and have a worse situation than before. I didn't find many comments on RU-vid maybe they get deleted. But i searched the Internet and found a thread where many people write they lost thousands of dollars or euros and for nothing. They all regret it. I only found two comments from ppl who said it has slightly improved but hundrets who regret it. I shouldn't have done it, it's one of the biggest regrets of my life and i cry everyday because it's so much worse than before. The dentist said it's a bad result but didn't really care. The surgery itself was very brutal and painful torture. I also found comments from professionals on the Internet who warn people, one doctor had it done on himself and ist warning people not to do it. My Front lower teeth got lose from the surgery and i found a comment from another person who wrote the same. The pinhole dentist said they might have been lose before but my main dentist doesn't believe it. No matter how much you wish to find Help: Stay away from this surgery!

Well I guess technically it's no longer Pinhole surgery if you use sutures. ;). At the end of the day, you want the gums to stay in their new position and heal. Pinhole is a variation of older tunneling surgeries that used sutures to keep the gums in place. Since Pinhole came out, there are now new variations of it such as Gum Drop surgery which is similar to Pinhole but uses PRF instead of collagen.

I got pinhole procedure done today. I am an active athletic guy I’m used to running everyday, how long do you recommend I wait before my next run ? And is walking ok

Hi, consult with you dentist. I usually tell patients that walking is okay but to avoid really strenuous exercise for 6 weeks. As I mentioned in the video, you don't want to be moving your mouth in a way that will cause the gums to move after the procedure. Heavy breathing can cause you to stretch your cheeks which can cause the gums to move.

Yes, wear your nightguard. Bruxism (clenching and grinding your teeth) can cause gum recession.

Hi, once you can start brushing the surgical site, the choice of toothpaste isn't as important as using an ultra soft brush.

You are better off doing the gum graft work instead. Minor swelling, really painless & good to go to pretty much normal after one week, not six like this technique. Recover time for pinhole technique to me isn't worth it.

The gum graft is probably more painful an I think you have to be under general anesthesia, this , from what I understand is local anesthetic. Both have around same "toss up " success rate". Its insane to think with all the new advancements in technology and procedure that there has not been any sure way to restore the gumline. Its hard to believe.

Yes the surgery can be repeated once the area heals fully. Make sure whatever caused the recession in the first place is addressed.

@@VeryNiceSmileDental Thank you so much for getting back to me. I've recession due to braces and hard brushing. I did it on 3 teeth left canine, and 2 left molars. The canine was a Miller 3. I have a thin gum type. The result from pinhole immediately post-op seemed amazing - now 2.5 weeks later I am seeing the recession back on the canine and mild in one molar. I am not very happy about the result. Is there any hope in re-doing it? Should I try a graft? I barely eaten in 2.5 weeks and mostly mushy food.

I have a dental appointment next week and have plans to do the pinhole surgery. Who wants to pay thousands of dollars only for something to go wrong within the next 6 weeks post surgery. That is almost 2 month we are talking about. Just save your money people.

@@VeryNiceSmileDental all of them. I'm 51 with bad recession. Growing up we were never told about gum recession. My dentist as a teenager never even mentioned about using a soft toothbrush. Over the years, I've always used hard toothbrush thinking this was the best way to clean my teeth. Nowadays with internet, we're just finding these things now of what not to do.

Sorry to hear about your situation. If you decide to go with any type of surgery for the recession, make sure you identify the causes of the recession and take steps to avoid further recession. For most people the causes of recession are improper brushing and flossing techniques, grinding or clenching their teeth, prior orthodontics, or simply genetics.

I truly think that if your and the end of your useful teeth life , it's probably lots more healthy and less invasive to just have them removed and get dentures. Trust me that will be the final outcome anyway and having that done recovering when your young would be lots easier that after say 60 or 70. And very few will be able to not touch the site one way or another and the patient will alway get blamed for the failure of these procedures.
