I recently had cataract surgery. I chose a monofocal lens set for distance implanted in both eyes. No regrets and my vision is just fine. Just need glasses for small print reading. Premium lenses is a out of pocket expense that insurance will not cover. Usually, one is charged 8 to 10 times the cost of the lens itself. I wish everyone good luck and educate yourself before chosing a proper lens for your eyes. Thank you for the video and take care everyone.

I had this surgery one year ago and chose the Panoptix lenses for both eyes. I totally regret having these lenses installed. All auto lights have such a halo and glow that it is distracting. I didn't have this much glare and halo when driving with cataracts. Over XMAS all the "twinkle lights" drove my eyes crazy. I am told I am one of a small percentage who have this reaction. However, I have talked with many who are aware of this problem from friends and relatives reporting it to them. Lastly, the near vision is not as good as I had hoped. The lenses cost me $9,000.00 out of pocket.

If these lenses are causing you such issues you need to seriously consider exchanging them before it's too late. Talk to your doctor about it and make sure that he has done many exchanges in the past. Taking them out is not as easy as putting them in. You may consider exchanging with a lens like Vivity. I've done this before for my patients with great success. Some doctors believe in neuro-adaptation (basically saying you'll get used to it), but I believe that there is a real physical reason for these issues in some patients and it needs to be addressed as such. Your doctor needs to make the call.

vivity is an excellent lens and can be installed as a mini monovision so I have excellent vision at all distances and no glasses

Light Adjustable Lens is 2x more likely to give you 20/20, no halos and top visual quality. I have been researching for my own cataract eye surgery. Loss of contrast is a feature in many of the diffractive, premium lenses. Even with the new IC-8. The LAL offers best visual quality and can be set to enhance intermediate and near vision in one eye if the other is plano for distance giving great blened vision it seems…but ask me again after my surgery.

I'm looking to have Cataract surgery but I have Keratoconus. I have had collagen crosslinking and also PRK eye surgery 5 years after my crosslinking. Which lenses are best for Me? I would love to see far and don't mind wearing reading glasses but it be great if i didn't need glasses at all.

I imagine that you still have a significant amount of astigmatism. Depending on that you should either consider a toric lens or a small aperture lens like Aphtera.

I'd love to hear about the (new for the US market as of July 2022) Lenstec SBL-3 (which has been used in Europe for a decade), and which at least one ophthalmologist here in Austin has recommended. However, it's nearly $5K per eye! Next to that, based on my research, it would be the newer Clearview Panoptix (the older Panoptix lenses, older than just 2-3 years, I believe, apparently had some issues with microscopic air-bubbles in them that were due to how they were manufactured, specifically a type of material they were using at that time which has since been corrected in the newer "Clearview" version). Third place for me would likely be the Vivity, which I've also heard from multiple first-hand accounts is a good lens, and does not have glare or halo issues (which I currently have with my cataracts, and which I hate with a passion!). however, it's been known to not be so great in low-light situations.

The Lenstec is the associated with a new type of glare called wing glare. This could potentially be worse than traditional types of glares. However, you should know that the newer generation of difftractive multifocal lenses are significantly less likely to cause glare and halos. Vivity, has a completely different design that is not diffractive. Therefore, not causing any glare or haloes any more than Monofocals. That’s the reason that it is my preferred lens for pilots and truck drivers. The only downside is that it does not give you perfect near vision. However, the intermediate and the distance are excellent and make up for it.

@@doctorilancohen thank you kindly for your thoughtful response. may i ask, what's your opinion about the J&J Tecnis Eyehance? For those that don't know, it's a type of monofocal that gives just a little bit more in the intermediate range, but not as much as the Vivity, and is not an EDOF but uses a similar type of approach.

After reviewing the characteristics of the premium lenses I prefer PanOptix on one eye and Vivity on the other eye to complement each other.

I can't get the vivify iol because the power goes to 30D, and I need 32 or 33D. I am therefor considering PureSee from J&J. Do you think the two iols are alike? (ps: I have large pupils)

I’m not sure what the power range for puresee will be since it’s not yet available. It’s due for release next year. Why not Panoptix? It has a wider range than vivity and is available up to 35. Very well tried.

@@doctorilancohen Thank you for replying. PureSee is available where I live. And it goes up to 34D. I can't get Panoptix bc. I already have a monofocal in the other eye (and before that I had a multifocal, and that was quite bad with a lot of dysphotopsia)

I'm a 75 yo woman who reads, crafts, quilts and uses the computer. I don't drive much anymore. I am considering the PanOptix. My goal is the achieve the best near/mid range vision with the lens, even if I have to enhance it with readers. I hope we have made the right decision for me. My surgery is in 2 weeks, then 2 weeks after that.

I wish there was a clear answer. I have seen doctor videos and they think the light optic lens is the best and they say their patients like them the most. I have heard that the panoptic lenses give more halos at night. This is all confusing and frustrating.

The Light adjustable Lens is a monofocal lens. It does not offer any near vision except through mono-vision which is also possible with ordinary monofocal lenses. Glare and Haloes with multifocal lenses are an issue but only in a small minority of cases, about 5% (which are the ones that you'll read about). If you're paying out of pocket, you should get some real benefit for it.

Vivity or a monofocal. It can vary greatly according to individual factors, such as presence and/or extent of Diabetic retinopathy etc.

This is not enough information to make a decision. It depends on many factors related to your eyes which are uncovered through an eye exam.

How long can you wait? Accommodating would be the very best option if they turn out to work over time…but no long studies and only a handful of patients so far.

These are great technologies, but they are still at their infancy. I suspect it will take another at least 5 to 10 years for them to become mainstream practice

Hello sir. I am from India.. My mother underwent PHACO with PCIOL with PANOPTIX lens. Vision is great and there is no complain of halos and glare .Next surgery planned in June with PANOPTIX. Sir. Should I go for panoptix or any other lens in the second eye . ??

After watching a number videos on post cataract surgery lenses, I believe I will opt for the Light Adjusted Lenses. I play a lot of pool, and I hate that my monofocal lens glasses either can’t see the cue ball close up, but can see the ball at the end of the table or I can see the cue ball close up, but can’t see the ball at the end of the table. I have a set of multi-focal lens glasses, but I have to hold my head in a weird position to see both extremes at the same time. I also see the balls bigger than they really are which has caused some weird aiming anomalies. I believe that having the LAL’s set to different powers is the answer to my prayers. I want to go to India to get the surgery done, as I cannot afford to fly round trip to the US in money and flight time health reasons, pay for the LAL surgery since my Advantage plan will not cover the LAL’s, and stay there for up to six weeks. Going to Punjab, India means my entire cost will be at least 1/3 the cost of going back to the US for the procedure, and it’s a lot closer to the Philippines. My question is, does anyone know if I can get the RX Sight LAL lenses in India? Or do the LAL’s exist only in the US?

I would recommend considering a multifocal lens. Their mechanism of action is different than glasses and I think for what you need will provide a better solution. However, I think you will have excellent outcome with LAL as well. It will just be many visits back and forth to adjust the power.

Enhance is a monofocal lens. And is an excellent one in that. The manufacturer claims that it does have some extended range of focus because of spherical aberration. Its near vision is certainly not a s good as Vivity or Panoptix. But of course, it is much more affordable. Most near vision obtained with it is because of induced Myopia - which could be achieved with any monofocal lens - and not true multifocality or extended depth of vision.

I had Panoptix lenses installed Jan 2024 - So much better than before but not perfect. I would probably choose Vivity as night vision haloes and glare are definite drawbacks. I was adamant about becoming glasses free but in retrospect I'd trade up close vision for absence of glare and haloes. As long as I could see clearly from 18" out and wear readers for up close I'd be happy. I can read the smallest print on a medicine bottle (boring) lol. At age 73 I'm seeing better than I ever expected to again but haloes and glare are very real concerns with panoptix. Complaining aside, I have not worn glasses since my 1st eye was done Jan 3rd, 2024 and that is a definition of freedom!

Yes. Glare and haloes could be a problem in about 5% of patients with Panoptix. An IOL exchange with vivity could be considered based on your doctors' recommendations.

Vivity is the lens that is least likely to cause any glare or haloes. It has excellent distance and intermediate. For near vision, you may need some extra help.

I've used mono vision with contacts for many years; +6 in my left eye for reading; +3.5 in my right eye for distance. I had a Vivity lens placed in right eye 4 days ago. The intermediate vision is fantastic and the near vision is better than I had with the +6 contact in my left eye. I'm seeing between 2020 and 2015 on an eye chart, but I the implant is still settling in. It's a little more difficult to evaluate my real distance vision but at this point it seams similar to what I had with the +3.5 contact. The left eye is scheduled for surgery next Tuesday.

I had the Tecnis Synergy multifocal toric lenses implanted 10 months ago. It has been a nightmare. Day 1 post-op i have great vision. Day two it deteriorated. It never got better. It worsened. Awful blurred vision for distances in daylight. The glare in daylight is blinding. I can see better in artifical lighting indoors and through tinted windows and wearing expensive polarized sunglasses. But it is still not clear. I developed PCO and 9 months later the ophthalmologist decided on YAG laser. Result - worse vision. Blurrier than before. Daylight glare is unbearable. I cannot open my eyes without pain. I cannot make out faces of people until maybe a few metres away. Signs and hard to read, if not impossible. I can read up close well, but since YAG it is worse and black colours look more grey now, so have lost contrast sharpness. I now face a long drawn out time to look at LASIK to fix some residual astigmatism, that the ophthalmologist stated I probably won't need after YAG. Discussions so far are alluding to possible need for a lens extraction and surgery to implant new lens - one eye at a time , as the visual outcome of the new lens will need to be determined before the 2nd eye is considered. It has been a nightmare of blurred vision. I have dysphotopsia which I had mostly accomodated for, but need to be extremely careful when riding motorcycle at night due to halos and glare. My eyes before surgery were deemed healthy and no issues to preclude me from lens implants. The Synergy was recommended for my needs (ditch glasses). Now since YAG was done, it complicates lens replacement as the posterior capsule bag is burnt open so the lens has only sides of the remaining capsule wall to anchor onto. Many ophthalmologists won't replace a multifocal/EDOF/trifocal after YAG and only option is monofocal which is a safer lens to implant after YAG.

I am very sorry for your negative experience. I myself do not use this particular lens because of the side effect profile. If you're still having problems, you should consider exchanging the lens. I agree that after YAG capsulotomy, it is a safer to implant a monofocal lens.

If I get monofocal lens 4 near how far will I be able to see without glasses I stay in the house most of the time will I be able to see everything in the house refrigerator the sink be able to do dishes watch TV my computer laptop and tablet and all that kind of stuff and will I have to have glasses and just as soon as I go out the door

If you get your eyes tuned for Near, you will not be able to see very far unless you wear corrective glasses for distance. This is not a practice that will make most people happy with the vision. To most the distance, vision is more useful than near.

The choice of lens cannot be determined only by knowing that she is diabetic, but is a decision that is based on multiple factors and has to be reached during the consultation with your surgeon based on your mother's condition and your surgeons experience. However, in order to give you some guidance, I think that in a patient that is diabetic it is best to use a monofocal lens or a Toric lens. this is a very general guideline and does not apply to everybody.

The clear view lenstec has an interesting mechanism of action. It divides optical zones to upper for far and lower for near. It creates a whole new type of haloes that are not similar to anything we’ve seen before. It is not very popular among surgeons.

@@doctorilancohen Thanks for your reply. I know this lens isn’t widely used due to it being fairly new in the U.S. but the surgeons i can find that are using the lens seem to be very pleased with the outcome. I guess more time is needed.
