Correction: Kayo canonically didn’t mistake Kai for her husband, she killed him and his family because he was the one that set fire to Enbizaka, killing many people and also her husband and child, aka killed her family.

She knew he wasn't her husband the whole time, and faked insanity on purpose. She saw a vision of what happened if she killed him right off the bat, and her actual husband did survive the fire but got amnesia and didn't recognize due to the switch
