Altough the description says it is a bug, I think that it might be a "random encounter" event or quest that is this exact scenario. Somewhere some Player might have started a quest and the game "forced" this encounter. Or because the encounter happend, as in the Muggers try to mug this NPC, a Quest pops up somewhere that directs the Players to save him. They stay in the loop till someone breaks it.
