This is an excellent presentation. I'm 65, and wore hearing aids for about 15 years. They helped for awhile, and I needed stronger ones after about 5 years. However I really struggled with speech, and my world got smaller as I withdrew from the stress and challenges of social life, travel, volunteering. I was finally considered a candidate for a CI and I jumped at the opportunity. I have been activated for six months and it really has been nothing short of a miracle for me. I feel normal again and my anxiety and fatigue have dramatically improved without the frustration and mental work to hear. I am beyond grateful for this life-changing procedure.

This is exactly how my audiologist explained it. I’m 42 and Diagnosed bilateral sensorineural hearing loss, with Profound deafness in both ears. Had my surgery one year ago. Best decision I have ever made. Went from 35% speech perception to 88%. I was missing about 85% on conversation. I became depressed and socially awkward. I felt left out and frustrated having to constantly ask ppl to repeat themselves. The one blessing I had was my ability to read lips. But that was taken away from me with the start of the pandemic. I became painfully aware of just how bad my hearing had become. I’m truly grateful for my CI…..

Wow, this is this best cochlear implant video I’ve ever seen! My 81 year old Mom just had an implant and we’re still going through the programs..it’s been three weeks since it was activated. I’m still waiting for the day she can hear me say “Mom” ❤️

I agree about the look,.. I was always so self conscious of the way hearing aids looked but I have tried so many invisible ones, RIC domes, etc,. And im just like at this rate, I really just need something thats going to help me hear the best and have the best sound quality,,... Ive learnt that thats way more important at this rate.. Ive always been thinking about implants as well but I don't think id be a candidate because my WRS is like 100 and my hearing loss is moderate range. I would certainly be worried about loosing residual hearing as well... But I always had thought the implants would deliver a natural hearing experience as well but some people say not

My surgery was two weeks ago. My activation is at the end of next week. I am 64 years old and have had almost no pain at all. Nothing regular Tylenol has not covered. I had very minor vertigo and dizziness for a couple of brief instances. The ear in question still feels numb, but even that seems to be decreasing. I'd say I have had nothing at all to complain about. It has all gone well so far. Looking forward to next week. I went with Advanced Bionics, over Cochlear because my perception is that from my understanding, Cochlear's technology just sees to have less room for growth and this is kinda a "rest of your life" kind of thing, so just seems to be a good idea to be able to upgrade in future without surgery. Am to get a matching hearing aid for the other ear for the use of the "added gizmos" to be able to work with both ears. I can keep your audience updated should there be any interest. My journey has begun and it's "So far, so good."

First audiologist that openly talk about cochlear implants. For that kudos to Dr Cliff. Theme for another clip would be having an implant when the main complaint is tinnitus. In Europe are having CI procedures for people whose main reason is getting relieve for T.

Great video. I was a CI candidate and sat on the fence for a year before having surgery 3 mo ago. My hearing is SO much better and I'm very happy I did this. I am 70 years old. Never too late.

Dr. Cliff you always seem to post these videos right when I need them! I am a hearing aid dispenser and am trying to suggest pts to see an otologist for approval on a cochlear but there seems to be some really bad information out there and keeps pts away as they continue to struggle with BTEs and custom fit hearing aids. Thank you for this great video. I started following you when my audiologist was pushing me towards CI because my hearing was almost gone at that point. I was subconscious about my hearing aids and knew cochlear implants would stand out more. I ended up deciding being able to hear my family was far more important than the perception I had of myself. As you mentioned, the surgery isn’t that bad. There is some mild discomfort afterwards. I didn’t use any over the counter pain medication for it either. Going forward with these implants was the best thing I could have ever done. If I had to do it over again, I would not have waited as long as I did. Even though they are not perfect, being able to hear again is a blessing for me. If anyone is interested, the movie, Sound of Metal, does a fantastic job of describing how the implants sound. The sound isn’t the same as natural hearing but the brain does adjust to the new sounds. A perfect example, even after wearing cochlear implants for 2 years, Michael Jackson’s voice is deeper now than it was prior to the implants.

I wanted to thank you for explaining what the candidacy process is. I was recently referred to an Otologist in UCSF to discuss with him about traditional ear drum repair surgery to try to correct my hearing, but the discussion turned to cochlear implants, BAHA and BAHS. The Otologist told me I should do as much research as possible on the devices and what the surgery was like, which I've naturally done. Part of my research was an earlier video that you released on the Oticon bone anchored hearing device. After about a week of researching the actual devices and surgeries, I messaged my Otologist and said let's go the BAHA/BAHS route. So I have an appointment with the UCSF audiology team on March 22nd. I'm happy that you discussed what the the audiology team will be looking for during this upcoming test. It'll be interesting to see if I am a candidate or if I'll have to wait a little longer for my hearing to worsen, with my traditional hearing aides. I also didn't know there would be a seminar, but that's very good to know, I want to have the most information available to me so I know exactly what I'm going to go through.

Your explanation of how the cochlea works was the clearest simplified explanation I have ever encountered. My question is this: I have been told that tinnitus is caused by damage to hairs inside the cochlea. I have had tinnitus for over 50 years yet when I recently was tested for word recognition, I scored high in spite of my mild to moderate hearing loss. So is tinnitus caused by partial damage to the hair cells or damage to the outer but not to the inner hair cells? Why is my speech hearing ability almost normal in spite of the constant noise in my ears? Second question. Why is it enough to have a cochlea implant for one ear only when typically hearing aids are used on both ears?

An excellent presentation. I've been activated for about 9 weeks now. I have had Meniere's for 15 years and this was my third surgery. I joked to my neurotologist that he spent more time inside my head than my psychoanalyst! My hearing became very erratic and I became a non traditional candidate for the CI. They also did what I call 'roto rooter ' of my ear which eliminated fully the drop attacks from Meniere's. I am still adjusting but it is going well

Thanks for the in depth explanation, Cliff. I feel very lucky to have only mild to moderate hearing loss at this time. So my Phonak Paradise Audeo 90R HA's are working wonderfully. But it's good to know that if my hearing deteriorates in the future, I will still have options to retain my ability to hear. 👍💕

Thank Dr. Cliff for this video. This is the first time I understand what the cochlear implant procedure involves. In the past, I had tried many times to learn about this procedure but the videos were too technical to understand. At this time, I don't need this type of implant but it is good to know just in case.

This process you described is pretty much explains my experience with my single side cochlear implant that I had in May of 2021. I am 69 years old. I have always been deaf in my left ear and had 40% hearing loss in my right ear. In October of 2020 I had a bout with vertigo and within 3 days I was totally deaf in my right ear. With the covid environment it was difficult to get any help. By the time I could see anyone it was too late. After my surgery I needed no pain meds and about 4 weeks later I got my processor working. I wasn't one that had great success at first and had a lot of work to do. I hear and that is so much better than total silence. Hear is a challenge but a challenge that has benefits over the option. Please get checked it can change your life for the better.

Great presentation. I am one of those who realize my hearing is diminishing and will likely need a CI.

Dr. Cliff: thanks to your excellent videos, I've decided to try hearing AIDS once again. I've just gotten a new referral to a private audiologist in Orange County, and will report back. My earlier efforts, using Kaiser Permanente's recommended HearUSA, and then my own foray to Costco were not successful. Try again!

@Doctor Cliff,AuD I started the process of getting tested for CI on my left side October 21st, 2021 where I had 16% word understanding at 110 to 115 decibels, I had my CI surgery December 23rd, 2021 (the BEST gift ever) and then I had my first mapping appointment January 26th, 2022 or my first hearing Birthday! Just 2 weeks into my activation, I went back to my doctor where she tested me and I was hearing things at 25 decibels and could understand some word’s/sentences! On my 3rd/ 3 weeks Mapping appointment because I was doing Remarkably well (better than most who were further in the mapping process) my Audiologist decided to test me again to see where my word understanding was and I tested at 68% word understanding at 50 decibels and with that being said it’s only going to get better/clearer as time goes on!!!! So, I went from 16% at 110 to 115 decibels to 68% at 50 decibels! I am 46 years old and I am hearing sounds I have never heard before It is proof that we serve a God of Miracles! Don’t put it off, if you are candidate get the test, go through with the surgery (btw, there isn’t much pain involved) you owe it to yourself! I am now just waiting for my “good” ear to reach the point where I can get that one done as well. Please, I really hope this helps at least one person take that leap, you are worth it and deserve it!

Oh my goodness!! Hands down the absolute BEST explanation of how hearing aids work, cochlear implants work, and what audiologist are trying to determine with the different sections of the hearing test. I just turned 60 and wear hearing aids. I’ve worn them now for at least 10 years-same pair. Anyway my hearing has gotten progressively worse. So I need these hearing aids reprogrammed or purchase new. My problem is I can’t seem to get them to stay in my ear. If I hold my finger in there and push, I can hear. I just don’t want to pay a lot and still have this problem because right now, I’m missing a lot of speech and finding myself less social.

I use my old hearing aids with all my life and it get harder to hear as I get older until I can’t hear anymore and finally I get my Cochlear Implant in 2016 and I wish I get it years ago, because Cochlear Implant is a lot better than hearing aids, they need to stop making hearing aids anymore. Time to move forward.

Thank you for putting this information out there. I went through this exactly in this manner (except for it being turbo charged to here I went from hearing aids to a CI in less than a year).. I've had my CI for almost two years now and have had great success with it! We need to get more education out there to destigmatize the use of a CI.

This is so interesting. I'd heard of cochlear implants but had no idea they were like this. Hope I never need one, but it's comforting to know what's involved, just in case.

I recently had total hearing loss in one of my ears. the audiologist has been pushing me to buy “the best” cross hearing aides available and they are not working. All they do is make my tinnitus worse and make me feel like I’m in a giant auditorium with lots of echoes. It feels like if someone with normal hearing would put a earbud in one ear playing loud tinnitus noise and then trying to watch a specific tv program on one television but having 5 other tvs playing 5 different programs at the same volume as the one your trying to focus on. I can’t understand the words many colleagues are saying to me so I just tun them out and I miss some important conversations I should have had. I’ve had the ENT tell me, well you never know, you might start to recover some of your hearing in the future. He doesn’t say what he means by future. I lost my hearing about 7 months ago and haven’t gotten any of it back yet. I have know idea if they would even consider a cochlear since mine is single sided but the audiologist did say it could be the only option to help get some hearing back on that side and help with the tinnitus, which is debilitating sometimes. I feel it gets so loud it gives me a headache. It is exhausting trying to ignore it and trying to pay attention to people talking to me. So frustrating.

Dr. Cliff I really appreciate this video and your explanation about CIs. I have bilateral Meniere's Disease with incredibly loud bilateral tinnitus. I wear Phonak bilateral hearing aids but still have great difficulty understanding speech especially if multiple people are speaking or there's any background noise. I also have balance issues as well. I will be seeing a Neurotologist for the first time this week Thursday then balance testing starts next month... My CI seminar is next week. My ENT has already suggested CI back in early January. I am nervous about CI because it will not stop the tinnitus or other symptoms. BUT, I want to hear and understand again.

This was a great video Cliff. I went through the evaluation in December and was found to qualify for the procedure in my right ear, but just barely. I chose not to get the procedure at this point for a couple of reasons, one logical and the other completely illogical. The logical reason was that when I asked about how likely it was to increase my ability to enjoy music as well as hear in crowded environments, I was told the improvement would be very minimal, if any. Those were two of my big reasons for wanting to get an implant and since the improvement for those things would likely be negligible, it just didn’t seem worthwhile. The illogical reason was that I had intestinal surgery in 2004 and again in 2010. I had a surgery to remove a small cancerous lesion on my kidney in 2016. So, since 2022 would have continued the six year intervals for getting cut, I wanted to break that cycle. LOL. Well, I told you it was illogical. By the way Cliff, your description of minor surgery made me recall what my dad said to his doctor when told the operation he was supposed to have was minor surgery. He said, and I quote, “Doc, the only minor surgery is on someone else.”

oh, wow I didn't know it was such a process to get a cochlear implant. The surgery sounds very similar to the ear drum surgeries I would have done to try and fix the hole in my ear drum. (didn't work even had a couple of these surgeries so I now have a hearing aid for my left ear because I have mild-moderate hearing loss in that ear) These sound really cool and I'm glad there's a way for people with really bad hearing to still hear ^-^

I have suffered for 15 year with AutoImmune Inner Ear disease. My right ear was initially effected but over time my left ( my good ear ) has scum to it as well. But life was livable. Until about two month ago when something melted down in my good ear. I cannot function in society now. I cannot understand anyone. I can place an order at a restaurant, I can’t make a transaction at the bank. I can’t go to the post office. Life kind of sucks right now. I have about a 60% ability to understand words in each ear. But I am told this is too good to be considered a candidate for a CI. I can’t function as I am. How bad do I have to get, how miserable do I have to be to qualify?

I think most patients find any type of surgery scary as the reason why they don't follow up with a subspecialist in ENT.

I recently got my cochlear implant surgery done and soon to have the processor turned on come may 4th and I can say I'm glad that all these videos are here to help. I am getting the sky CI M with the Sky M link hearing aid to be able to hear better I am deaf in one ear and moderate to severe in other ear. I'm ready to start my hearing journey into sounds.

This was a very helpful video. I might need a CI. I’m hearing impeared since birth and I’m almost 45 years old now. My hearing loss from birth is behind the cochlea, the hairs! But now I’m older and having chronic sinusitis my hearing loss is complex, according to my doctors. They have done several tests to find out which hearing aid is the best for me. But I’m not satisfied with these aids, for the first time in my whole life. During my process of getting these aids the doctors told me that other people with the same kind of hearing loss already have a CI only my scores in the tests are still too good. But in real life i don’t hear quite well.

thanks to a cochlear implant I hear it again. It's the best, thanks to God and the doctor, I can hear my daughters and my husband again and music and many things that hearing aids didn't help, they just made me spend my money

Hi doctor Cliff. I really appreciate you sharing this. I have a brother who has become a perfect candidate for this procedure. However, he is very overweight, and they are afraid of performing the operation because it could be bad on his heart. I've heard great things about people using this implant. Currently going thru all the testing so insurance will cover CI.

I had an implant , I was deaf on one side, whilst I was really happy in the first weeks, I now regret it. My sleeping pattern has been affected and I have these shooting pains around the implant from time to time.

Cliff you should also address tinnitus and hyperacusis to sound that happens with individuals that go abruptly deaf in an ear. I had both ears in perfect working order and then I had an intralabrinthine bleed (we believe). Bam DEAF in the ear like a light switch. Doing cochlear shortly. I want to hear in that ear and orient sound, lose aural fullness and the nightmare of tinnitus.

Thank you for this video, I have seen an ENT, Audiologist and I feel like they didn’t have my best interest on the table. Even when I mentioned the burning and 20/24 Tinitus for over a year I felt like the didn’t want me to get further help. I read about Otology, and thought why didn’t the ENT’s and Audiologist mention this to me? When depression, and isolation hit you it’s hard. I hope to see an Otologist soon. I know this post is from a while ago but hopefully this question will make its way to you. What if you're testing a two year olds hearing with their hearing aids on and it comes back with mild to moderate hearing loss. Would they be a good candidate for cochlear. An audiologist is leaning in that direction for my daughter. She's also being tested for autism which I believe plays a part in her inconsistent aided hearing test. I am bi-aural since 2019. It is totally amazing and hearing is back for me. One was a revelation to begin to hear again. two CI was no comparison to one. It went from hearing sounds again to like a full orchestra, the sound was full and exceptionally improved. My question is, My wife is quite deaf and uses one aid. It is hard to talk to her, she just misses it. She turns the volumes up (TV, etc.) Itis hard to deal with since I hear close to normal. She is 83 and is afraid of another surgery and says she doesn't want a magnet in her head. Now the question, Can the ear without the aid be implanted? Of course the other questio is, what about the ear with the aid. My answer would be. both can be implanted. It may take more time with the worst ear, but the preferred of the surgeon is to first do the worst ear first if it is deemed workable. That is why they do evaluations and scans and MRI. The biggest problem is to get my wife to be willing to try. It was a no-brainer for me. She was once told that she is nerve dead. I disagree, she uses an aid, that is not nerve dead. The process is much advanced and somewhere along the auditory pathway there is help available. The aid is not getting it.

I am actually in the process of finding out if I need hearing aid or cochlear implants. on my right ear I have no hearing from 2kHz up and on the left its in the profound hearing loss area. boosting 110-120 db seam a little pointless to me, but I have yet to try hearing aids. I have only done the audiogram test and made the in ear mold? don't know the right name. I also had an MR scan of my head that I don't know the results of yet. when I explain to people about my hearing loss I use lines normal people would hear like this --------- and mine is more like ---______ The results have actually put me in a depression because as a musician for 30+ years I now realised I can no longer make music and do the mixing myself, simply because I can't hear what other people hear in the higher range of the frequencies.

I am currently awaiting the Otolaryngologist's return call. I first had to send him my medical records to be reviewed. Then, if he thinks I am a candidate for a cochlear implant, I get an appointment which will be a waiting period of 2 to 3 months more. He is the only Doctor in this state that does the surgery. Is it possible that someone else is qualified for the consultation of the candidacy of said implant, such as the company that manufactures the implant?

I truly thought this was brain surgery. Could you recommend a video so that we could see what the process looks like?

My question is, what about bone conduction hearing aid? Pros and cons and will you have ability to tell direction of the sounds?

Can you do a video on Eustachian tube dis function what it is and how it may cause hearing issues?

What about when it’s a toddler who hasn’t even started speaking yet? Saw a Dr today and discovered through a couple tests that they believe he isn’t speaking because he has abnormal hearing . They want to do some sedated testing, what might that show since he is so young?? Haven’t seen many of your videos that talk about younger patients. Thank you!!

Does it take all the ringing away getting this done. It's the noise that drives me crazy.

In this video Dr Cliff admits that cochlear hair cells regeneration is not around the corner. This comment comes in contrast with another clip where he puts a lot of hopes on regenerative hearing cells drugs (namely fx-322 and fx-345).

I have had hearing aids but they can only do so much to hear and understand speech and My Audiologist wanted me to go to ENT Specialist now for some more testing and my next hearing aids or CIs now because when you test low in word recognition testing I think that is what they have to do. Even amplified a lot it is still difficult to hear some sounds of speech especially with background noise so my hearing aids need to mute background noise around me. CIs are supposed to be better and maybe after my next hearing aids it will have to be done if they do not work for you at all. I can understand why people do not want it done because you are afraid with CIs your level of hearing will not be better than with my hearing aids.

Hi Dr. Cliff, Thank you for the covering this amazing hearing technology and exciting subject. I have single-sided severe hearing loss. Word-recognition when tested years back was only 20% in my affected ear. Unfortunately cannot receive CI surgery for regulatory reasons where I live (might be years off). Currently I use an adjusted hearing aid in my bad ear only, just to pick-up some sounds, not comprehend; but also so my weakened hearing nerves continue to get some stimulation. Until CI surgery becomes available to me, do you have any recommendations on how to keep my bad ear "in shape" for the surgery? Or maybe you have some other recommendation?

You always have great information. You explain in depth and at some point I will need hearing aids. Understanding some people speech is driving me crazy. Most people I understand but there are some I have no clue what is being said. This happened with a Dr. and with some women.

Very clear explanation doctor. Well done! One question if you don’t mind.......my left ear is totally deaf as a result of a virus infection at the age of 5 or 6 (probably German measles). I’m now 74 and the hearing in my other ear, initially only slightly damaged by the virus, has now deteriorated to about 40-50% of normal. Any point in considering a cochlear implant for the deaf ear.?

The bad this about CI is its expensive buy here in India no insurance companies cover the C🤐😒

I am going for surgery 06/15/23 for implants. The hearing aids just aint cutting it anymore. I am 71.

What's your thoughts on drug 322 and 345? Will it cure tinnitus and hyperacusis too along with sensorineural hearing loss as this drug regenerate damage inner s hair? I'm from Nepal 🇳🇵 😊😇

Can cohlea implants still be good if someone really wants it instead of hearing aids? If a person wants to achieve a better understanding of speech with a cochlea. for example. If a person can understand 80% speech with hearing aids, could they increase that percentage with Cohlea Implats? Is price one of the obstacles? Are the candidates those below 50% speech comprehension, because insurance does not want to pay for installation and all costs? What if we have the money, can we still get a better understanding with Cohle than hearing aids?

Unless you've taking these tests forever and AUD's say "guess to the best of your ability" well I can hear enough sounds that I can guess correctly, it doesn't mean I heard it. It also is never like my real life. Last test (a few weeks ago) the background noises never got louder than the sentences /words. So technically I heard too well. But reality...nope a huge struggle.

Can you tell full story with a pictures so we can understand better, because i understand you say it but its shortcut msgs, and is it good cochlear implant is better for 2yrs and 6months old?

This is really good information. I have seen some with a single CI, I have seen some with dual CI's. There are some that talk about over stimulation with the CI's. What happens if there is problems like migraines? I may not ever qualify for a CI but so far I have one sided deafness, or HOH but they recommend a hearing aid.

I have profound hearing loss on my left ear at the high frequency level. I recently have steroids injected into my ears via grommet. 3 doses one week intervals. Just now I tried out for hearing aid in my local audiologist but seems like even programmed properly, the hearing aid did not perform as expected. They suspect the grommet is interfering with the hearing aid. Is this a possibility?

I've been waiting for a video like this for a very long time. I have lost 98% of my hearing and have only 2 lower levels out of the 8 levels of hearing left, and was told by 2 different audiologist that I am a prime candidate for the cochlear implant. I have been on the fence for the very reasons you mentioned in your video. My concern, if you can call it that, is I'd love to know just how it speech and everything sounds thru the cochlear. is it clear and concise or robotic or?

I was very disappointed that you said almost nothing about the many potential disadvantages and/or complications that may be experienced with cochlear implants, other than comparatively superficial issues such as appearance. The FDA lists these issues in their article "Benefits and Risks of Cochlear Implants". Yes, for some candidates with profound hearing loss implants can be life-changing, but results vary. I also find it significant that the FDA states quite clearly that the long-term effects of being fitted with cochlear implants is simply not known due to the relatively recent development of this technology. No disrespect intended, but not a very balanced presentation in my opinion sir.
