When is All on 4 a Bad Idea?

All on 4 (all on four) or same-day teeth dental implant treatment is a life-changing and fantastic treatment.
However, it is a drastic treatment, and sometimes, there is a better way to bring back health and beauty.
In the last month, I have seen 4 people who wanted All on 4, but we discussed better (and often cheaper) solutions that could provide excellent results.
To find out more about me, and arrange a consultation, you can have a look at my website.

I am a pretty experienced dentist myself and I work in an area with high dentistry demand/volume. I see extensively destroyed mouth everyday to help these people get the maximum result from the most efficient treatment. I don't know if this dentist in the video specializes in surgery only and therefore lacks knowledge/experience in restorative field, but this idea of "the less treatment, the better" is unfortunately not true for most extensively damaged mouth. The first photo shown 0:37. Patient missing all lower posteriors. Upper posteriors already supraerupted. I bet the upper posteriors are already about to touch the ridge on the lower and the ridge on lower is significantly resorbed and has no adequate width for implants. Upper anteriors significantly worn down and not enough ferrules for proper restorations. Even if a dentist manages to restore the upper anteriors somehow and open the VDO, what do you do from there to bring back the missing posterior? The patient is now forced to use partial that won't be able to support the posterior effectively. More failure towards anterior will continue with lacking support in posterior. Why have patients suffer until they realize they are heading towards full edentulism no matter what they do? Photo shown 0:50. Another compromised VDO from entire dentitions worn down. Full mouth rehab case with multiple non restorable teeth being replaced with implants or bridges. Maybe this is one case among all shown here that can be restored. The amount of treatment fee will be enormous. And the hygiene level of this patient that lead to this result most likely will continue to fail over twenty expensive restorations. Photo shown 1:00 What? Whaatt?? How in the world are you going to restore those teeth? I see about 4-5 barely restorable teeth there. Good luck telling your patient to wear partials hanging on to 4-5 crowned teeth. Clinical crown length too short. No way a partial will get any retention grabbing on to those short teeth anyway. I am not even considering this patient's perio situation here. Even if this patient has no boneloss at all, no sane dentist should choose any other option than full mouth extraction. 1:14 Upper is gone. Full upper extractions. If you want to be an "ethical" dentist by not recommending full mouth extraction, you will then say you will save lower premolar to premolar and replace those lower molars with.. whatever you choose to do. But what is the point showing this photo when you know upper is full extraction anyway. Calling other dentists they are putting their financial gain over patients' needs and claiming these patients should go for restorations is you being unethical just to gain the title of good and popular dentist. We have to admit that some patients have gone beyond the restorable stage and help them go through full mouth extractions so they stop experiencing continuous failures and frustrations.

Thank you Dr Gurs for pretty much confirming the treatment recommended by a Dentist to me. I was exactly that person. Early 40s, teeth in a bad shape and had all on 4 in my own mind, but the dentist I visited immediately disregarded the all on 4 route and said that it would be quite a bad move due to my age and the salvageable condition onf my teeth. I am more than happy I trusted his recommendation and had crown bridges and veneers. The internet is a minefield with clinics pushing for treatment that is not the best option as you say.

Dr. Gurs. You have no idea how badly I wished I was in the U.K. and could ask you to solve my issues. I am losing my ten (yes ten) implants and the rest of my teeth because the practice that did my work missed the occlusion. I went back a half dozen times to fix it but the fact is they missed it so badly they couldn’t repair it unless they replaced all the crowns and decided to dismiss me instead. Let another dentist try to equilibrate things and he burred all of it down 4x and collapsed my bite by several mm’s. Now I have no bite, crowns hit at bad angles, teeth are loose and ready to fall out, and bone is lost around everything. It’s truly been life crushing.

Hi Dr Gurs Sehmi, I've been watching your videos for hours now, I went to a dentist, she sent me to a Prosthodontist, and from there I went to a group that does same day all on four. I don't fit your description here, with nightmare thoughts of dentistry and quite frankly I'm devastated by their prognosis. I had a wonderful childhood dentist, and have seen a dentist pretty regularly all my life. Unfortunately I've also have asthma and allergies and sinus problems all my life. I spent about $6,000 on my teeth just two years ago, and spent at least $1,000 per year before that, due to my open mouth breathing, they said. I have 3 implants as well. I'm 60 years old, a very active wildlife biologist, I have a great smile and use it often, but I'm also often in pain with dental issues, most of my teeth are now crowned or bridges or implants, and now they say I need this all on four for $55,000. I'm just not ready to do this, but I have major issues I know. I have an abscess in a tooth that is part of a bridge, several crowned teeth that are decaying underneath the crowns, and several cavities in the few teeth that are left. After watching this video, I really wish I could get a consultation with you. But at the same time, I realize that if I replaced 4 or 5 crowns, got 3 or 4 implants, and got a few more new crowns, that would be upwards of $20,000 to fix things, and it seems it wouldn't last 5 years, more less 20 years! ( just a year and a half ago I spent $4,000 on a bridge that I was told now has decay) I feel that if I am looking at getting this done in 5 years, then why spend $20,000 now to save everything? Granted the thought of loosing all my teeth is literally keeping me awake at night! Whew, any suggestions?
@Jacksonabean

Far better in this video! You are correct, so many clinicians are heavy-handed when it comes to treatment planning all on four. Our natural teeth are the best option in 90% of all cases. The case like you displayed, where biomechanical damage is the culprit, all on Four can be a great treatment option as it eliminates the risk of future biomechanical damage. It takes just as much home care to clean a fixed implant bridge like all on four as it does natural teeth. In America, most people understand this premise but it’s always best to reiterate the amount of time they will need to spend cleaning the implants and prosthesis. All we can do is educate and inform the patient on the benefits and risks of the different treatment options. It is not our decision to make. Hopefully, you have CariFree as an option across the pond like we do so that you can prescribe it to the case you restored on the natural teeth. It sure is a nice insurance policy to help the patient neutralize decay and erosion.

I was lucky enough to meet with a Dr. Eslami, who gave me a less expensive alternative plan to all in four. I did not understand and was so convinced from the tv ads that I overrode him and wanted the all in four because I didn't want to see two dentists and go several times. I am going to ask Dr. Eslami of he will consider the original plan. Thanks for open honest advice.

I am planning to do a couple of implants (two in the lower left on one implant and three in upper right/two implants). It will be done by a graduate student who is specializing in implant dentistry at the UNC-CH Adams School of Dentistry. The teeth have been pulled out last year and seven years ago. After taking a 3D x-ray they are going to determine if a bone graft is needed. I would appreciate as to what is your opinion about doing it in this way and about the school and any advise?

I fully understand dentists are a business and in the current climate its been reported some are not going survive, lots of research and different opinions through multiple consultations may be best policy. Extremely useful information. I recently had all of my upper and lower teeth removed with encouragement from my dentist, periodontist and oral surgeon. I do have periodontal disease (deteriorating bone, etc.) I had canceled a few appointments to have my back teeth removed because I figured I would have a deep cleaning, keep up with regular cleanings with my dentist and keep my teeth as long as I could. None of my back teeth were causing me pain or discomfort. I did have a "fang" tooth on the top that was barely attached/extremely loose. I did want that tooth removed of course. My front upper and lower teeth are as professionals put it "the best teeth I have". They are however crowded, twisting, leaning and are actually being possibly pushed out on the bottom front. This appears to be getting worse fairly quickly. The idea of having my rear teeth removed was that I would keep my front teeth and wear partial dentures for the back of my mouth. I don't have any teeth that have peaks and valley's for the partials to connect to which is the only partial example I've been shown as far as what I would have. I have darn near decided I mise well get full dentures. What my oral surgeon said has been of help "try eating steak, etc., with your front teeth while your extractions heal and see what you think". It works well with small bites. Now I need to find out exactly how partials will attach to my smooth "single layer" front teeth. I appreciate your video's because you are thoughtful and in depth regarding your patients best overall outcome. I believe this is how dental professionals should present their education, experience and expertise. Thank you.

Dr Semhi, why not come to the US and talk to some of us who really appreciate you. I don't know a dentist as honest as You. Most dentists here in the US are only here to make money, they don't care about our dental health. I had a dentist tell me to let my kids eat candy so that he'll stay in business. I l just got the all on four procedure done, I am currently still in my temp's, and let me tell you, it is the worst decision of my life! I saved for 8 years to have this procedure done, and It was going to be a new life for me! I am more depressed then I ave ever been! The cannot get the teeth right for my face shape, and they created my bottom arch too narrow, it is not giving me any jaw/ facial support! The dentist is telling me that he cannot add more acrylic to the bottom piece to fill out my face, for this is not something that can be achieved with teeth or dentures. This were his exact word's! I know it's probably because of the placement of the implant's! I'm heart broken!

Thank you for you honest options. I am lost in dental “hell”. I have 2 implants on the bottom with snap in dentures that will not stay in thus I can not eat. 2 implants on the top with a partial plat on the top that I can not talk with these in much less eat. The last dental visit it was suggested that I have a new denture plat made for the bottom(that would hopefully fit), have my remaining 6 teeth removed from the top and have 2 more implants put in the top. All of this or have everything removed and have full set of dentures. Any words of advice would be greatly appreciated. I am 70 years old but would still like to go out in public and eating would be wonderful. Thank you😊

It of curiosity how are you doing now? Id love to have some help for you, I just want to say I can relate on some level. I struggle to eat too, but because of my own awful teeth.!. It's difficult to know what to do, pull out those teeth and be pain free (for once), or try to keep saving them at a financial and emotional cost, both have their pitfalls. It's just one tooth after the next, feels like a dog chasing it's tail... I'm just so over the pain, anxiety, misery and struggles with food, it's should everything. Everywhere you look there's perfect teeth, which only makes you feel worse. I'm told my teeth need to last another 40 years, might mean nothing to a dentist, just words of "helpful" advice, but sure sounds like a miserable sentance to me, more pain for 40 years.. yay :( I wish we'd evolved to replace teeth like sharks!! Anyway, I ramble. I wish you well and hope you've resolved things x

I broke one tooth and found a dentist to see me and they pulled all my teeth and gave me the biggest ill fitting denture ever. I do now have the all on 4. I just recently had to purchase another set of teeth to go on my implants and that was very expensive. But over time the teeth wear out and have problems, teeth break out, the whole bridge cracks and breaks etc. and you have to wait to get in to see someone who can take it out and fix it for you. So what I am saying is it would be better in the long run to have the snap on ones that you can take out yourself when there are problems with the teeth.

Chief where are you based? Also I could only get this procedure if I am fully unconscious. My last trip to the dentist, I was given sedation, just an injection. I wasn’t fully aware of what was going on, however I did feel some pain and I was awake. Probably my own fault, I was on some beak the night before and I am prescribed diazepam by my doctors so I have a tolerance to a lot of medicines. This was only for a filling, so I could not possibly accept sedation for the implant procedure I would need to be completely knocked out. Do you offer this option?

Because if you get all on 4s They have to remove alot of bone and gum for the all on 4 to be placed and if for what ever reason they fail even getting a half decent fitting denture may not be possible. And as you've said be4 Titanium can only do so well for so long in your body........Realistically how long would you in your own opinion give a time span on all on 4s with good care and hygiene .......
@drgurssehmi

With great care and hygiene, they should (in theory) last a lifetime. The teeth but may wear and need replacing, but this is easy compared to replacing the implants. In the same way as natural teeth should last a lifetime with good health and maintenance.

@@drgurssehmi I think it amounts to what is considered to be *...better long term solutions...* It's my own personal experience that the psychological trauma caused by the awful appearance is more debilitating than the pain or other associated problems and no amount of good dentistry care will assuage those problems for those unfortunate enough to have such poor teeth. I always took really good care of my teeth and saw a dentist every three months for a check up and did a lot of work and paid a lot of money. In later years stress had me grind my teeth and break some then a family drama did the rest with infections costing me more teeth. I have seen two 'good' dentists last year and asked them for implants and they both said that what I wanted was irrelevant as I would get false teeth. They see my age (66) and thought that it was 'normal' but I come from a cutlture of false teeth and have sworn never to get them. I ended up planning a trip to Turkey as a friend went there and his teeth are amazing. In France where I live they are asking me for 40,000 euros for full overhall and in Turkey they are asking 8000 euros for the best quality crowns. I cannot afford NOT to go.

I had all my teeth pulled out in one step which was very easy but unfortunately several weeks later I had an infection and I didn't go back to him for 8 months. I honestly don't know what was worse the paint or him yelling at me. he really is a good dentist though. he tried for 2 hours and pull that in plan out but he couldn't so he had to cut a window in my gum and operate that way. it's been a week and a half and I'll have to go back and see him in 2 days and then it will be a couple more months before I complete the operation. the only reason I couldn't get there soon it was because it's too hot in Arizona and I just can't leave my animals to drive two and a half hours to go see him. infection is totally my fault but I'm glad I watch this man's videos cuz now I know how to clean them when I get them.

So..let me get this straight - . its better to spend thousands of dollars getting crowns and bridges when someone is in their 40', then after those fail.. spend thousands of more dollars getting implants when your in 60's and 70's... which is at an age that takes longer for your body to heal and for bone to grow? I'm sorry if I seem skeptical. Wouldn't it be better to get implant treatment when you are still younger, and bonding with the implant would result in a higher rate of success? Dr. Gurs you didn't really talk about pain tho, I noticed throughout this whole thing you dont talk about pain management. Even if you do a root canal therapy on every single tooth there, there can still be some pain and this is why the implant option is really looking great now in a lot of peoples opinion, They dont have to worry about pain in the future of a tooth going bad, and so its less pain and less worry and such. I have had a lot of root canal therapy and fillings on my teeth and now suffer from sensitivity in those teeth that is sometimes unbearable, so what I would suggest is that if you're susceptible to pain and having such pain all the time, you should consider other avenues.

For the record, teeth removal is also the CHEAPEST route (yes, even vs fillings from the dentists I've gone too). That's where I got tripped up. When I was younger I had 3 teeth that required root canals. I had no insurance and a poverty level job. So what did you think I went with??? Yeah. That's the state of dental care. I have a sob story of when I was younger, blah blah blah, but the fact of the matter is, there is almost zero incentive to want to take care of teeth in the long run, especially when you could go to 4 different dentists and receive 4 different treatment plans. All costing upwards of tens of thousands of dollars. Basically to restore my teeth at the end, would've been a cool 30 grand. OR. For an additional 10, just get em all pulled and start over. Ultimately dental work has absolutely nothing to do with dental advice because 1. It's inconsistent as all get out and 2. At the end of the day, it's all about cost effectiveness and not what's best for your mouth since we live in a country that hasn't improved dental insurance in over 60 years. And the costs are outrageous. On a side note, had I taken the restoration route at 30k with insurance only covering 2k a year, it would've taken 15 years to completely restore my mouth. Additionally, there's a TON of scam artist dentists out there who will do things like try to restore unrestorable teeth which makes the tooth fail again which requires you to go back and pay MORE for an operation you never should have had in the first place. To be honest, I really don't care if my teeth were ACTUALLY restorable in today's climate. I'm much, much happier with all on 4s

I have had 4 regular implants originally 5 but 1 had to come out since before I served in the military in 2008. These things have not been to positive to say the least. Every 6 months adjustments have to be made for comfort. It’s really uncomfortable with just that 1 for an entire upper denture. Also there is 1 in the back on the left which is where I chew most of the time snd nothing on the back right. They have offered to insert another one on the back right but I have been extremely fearful due to the nerve and what the side effects of hitting that nerve are. I’m curious if I did have that vital other implant on the back right could these then be used for the all on 4 for a bottom denture? Also if that nerve is aggitated or affected would it not heal over time or have you seen permanent nerve damage from this? I really wish to god I had someone like uou in the states that I could talk to. The VA sends me to the lowest bidder which is not reassuring and I’m now in a blessed state financially where I could do something like this now. Mind you I’ve been dealing with this life altering experiance since 2008 I’ve had full upper and lower denture. God I tell people all the time even if your teeth are in bad shape do anything to keep your real teeth NOMATTER what. As bad as mine were from dealing with cancer as a child I wish I would have gotten root canals on them all and capped them all rather than goin the impact route. But now I’m here and have been and I have no choice. Maybe there is a decent implant doctor you can refer me to in the states? I served in Afghanistan for 15 months just a month after that surgery snd had to deal with making adjustments with my own tools during the curing and healing. I wanted so bad to serve my family and country only death would have made me leave my brothers I served with. @@OG-Ghost33 I'd talk it over with multiple dentists on this issue, their opinions may differ but the treatment option you go with is for you to decide. That cow bone tissue stuff they used is most likely a bone graft, that's used to promote bone growth in your gums because after you get your teeth extraction you start losing a lot of bone in your gums overtime which also results in a number of different potential issues although in your case since all your teeth are extracted, you may only have facial shape changes that may give you a premature aged look and your risk of developing gum diseases also increases

My comment on this issue is (even I do agree with you that there are dentists who treatment plan too agressively for full arch teeth replacement) is that some cases can be savable through restorative dentistry. But patients get there for a reason. For patients who hve advanced problem with decay, put their money towatd RCT, crowns bridges may get them back in 2,3 with gross decay again. Something to consider. And sometimes 2 3unit bridges on implants will cost almost a whole mouth???!!! We still have to work based on case to case basis

Interesting video, really need all the help i can get at moment, as i will be having implants this year, in the next few months, i am already corresponding with one dental practice at present! Not sure i agree with this video, as he mentioned dentists are just trying to make money by taking all your teeth out and doing the all on 4 treatment, so my situation is i have 7 apparently ok teeth on the bottom, and 6 on top and 4 got to be removed,found this out by local dentist today. This is a private dentist i went with today, as my national health one wouldn't fit me in, and i needed an appointment asap, i have an abscess, ended up paying £73 for some antibiotics basically, and a treatment plan, that is apparently going to cost me just over £1000 for 2 fillings and to take 4 teeth out, seriously? get me all ready for if i want implants, so first off right there all that is a total waste of money,and a rip off as obviously the place that i choose to do my implants can take those teeth out. i'm not stupid, he has even added a hygienist appointment to the list of crap at £70, really? with the state of my teeth! The point i am trying to make, in a long winded way, is i think dentists like to draw out the work sometimes, more appointments then needed, and defiantly unnecessary work, so i have to strongly disagree with Dr Gurs Also going on from this where i am talking to about my implants, they have said they want to put in individual ones and won't take out my bottom teeth, as it is against the law, i desperately want all on 4, iv'e been through hell with my teeth, pain and suffering for years, no confidence, i want a bright white Hollywood smile that people notice, never had that, i deserve it i think, how the hell do they intend to match up my very off white verging on yellow failing teeth, they can't without using more expensive veneers, and i am not paying thousands of pounds for a yellowish smile, who the hell would, this is heartbreaking to me, and i don't see how Dr Gurs argument holds up here really, please inform me how i am wrong!

Dentists are so out of touch as to what this actually costs people; time, money, pain, self-confidence. How many of your patients with these long term plans actually complete the whole plan? Why would people spend $10k if they can spend 50% more and have a permanent solution now that eliminates pain, inconvenience, the additional financial burden in the future? If someone in their 30-40s needs dental treatment and they'll have to eventually have implants there is no need to go through dozens for dental appointments and spend $5-10k a year to save teeth that will eventually be removed anyway.

My teeth are in rough shape due to a combination of genetics, eating sugary foods, and not going to see a dentist/hygenist nearly enough. I'm 44 I live in NYC and now my teeth are getting worse & worse. I am in photo/video production. This often puts me in front of big money clients. I don't smile anymore without covering my face. I enjoy laughing.. but I cover my face. I don't fear the dentist but the costs here in the US are abhorrent & steep compared to the rest of the world. So, I just want the same quality care without shelling out enough money to buy a new car. I had been told that most all of my top teeth will have to be removed. And the options I'm ok with if that is the case we're implants or the All on 4 overdenture or permanent implants. Which I was quoted between $25000 - $60000 depending on certain procedures & options. Costs in the $10k plus range years ago is what kept me from doing things sooner and my teeth continue to decline. So now I'm extremely depressed and have heard about Medical tourism. Your video is one of the videos I found on my search so here is my comment. I'm not sure where you are located but any feedback would be helpful. I recently had all of my teeth CT scanned in the full head machine March 22, 2022. Of course I'd be open to other options if possible. Thank You.

The problem with your theory is that doing the piece mill treatment can cost much more over time that just having the one time treatment that deals with a lot of the long term problems that people will have with their natural teeth. For instance, I went to a cosmetic dentist to get a plan started. They did a deep cleaning, all the imaging and x rays, did a root canal on one of my upper teeth and extracted all four of my lower back molars. Those teeth were rotting and in bad shape. Then came the bone graphs on both lower sides in the back. By that time, the bill was already 10K and I could see that getting the back lower implants put in was at least going to be another 10K, maybe more. That would mean that I would have spent 20K and none of the teeth that are visible, when I smile would have even been worked on or touched. At that point, I got cold feet when they would not give me a price for the overall plan to do my whole mouth. So, I left, having spend 10K and I still had no teeth on both sides of my back lower jaw. I know another person who got four implants put on one side of his upper gums and it was 22K. He still has teeth missing on the other side of his mouth but cannot continue because of the expense. So there that.
