When the COVID-19 Pandemic Leaves Us Feeling Helpless

For most patients, the COVID-19 crisis has created a “new normal.” They may be stuck at home, unable to work, or feeling isolated from dear friends and family.
This all can leave people feeling helpless.
So what can we do to help patients regain a sense of agency during the pandemic? Bessel van der Kolk, MD has several ideas.
According to Bessel, there are insights we can draw from trauma therapy that could help patients when they’re feeling helpless or reeling from the unpredictability of life during a pandemic.
Now think of the patients you’ll be seeing this week. Is there a strategy from the video that one of them might find particularly helpful?
We understand that not everyone will agree with Bessel’s politics, and we appreciate that we have a community of practitioners from both sides of the aisle. But for the comments we’d like to focus on what we all have in common: our work with patients.

experienced a trauma and I experienced something that cannot be lived, this experience changed me forever. The structure of the day is the basis for trusting yourself. He knows what he is talking about ... I would love to work with him, I think we could learn a lot from each other ...

I was the victim of pretty massive childhood sexual abuse (along with one of my sisters). Sexual trafficking played the worst part in that. The men (mostly) involved in that were the very types of men who are running this country now so this current political situation - especially this past year - despite years of therapy, etc. and living exactly the way you describe for decades - seems to have devastated me. The crazier it's become, the worse my ability to 'digest' it has become despite my attempt at distancing myself from it as much as possible. (Having an upper GI series this week.) So thank you for validating what has seemed very obvious to me... just that means a lot. PS: the book is helping me to feel validated in so many ways.

Interesting. I find myself almost in the opposite situation. Now that all external obligations have been removed, I feel a sense of freedom to do my own thing & be my own self. I feel like I've been quarantined my whole life, but now the usual pressures, rejection accompanied with negative criticism are gone, and I can structure the way I want, the way I've always wanted to do things. No more exterior control, or a least a lot less. But it's so true that organising our lives gives a sense of predictability, security, a place of reference. Thanks

I agree with you; I'm truly enjoying all the free time, no obligations. Yet I'm doing things for myself and others. And, I was traumatized as a child.

I'm glad the video was not edited. My life DOES feel even less predictable with that leader of the country! The hatred he spews adds more unrest in my day as I wonder who he will anger so much that they will attack our country! Unpredictability, lack of trust in a leader, feeling lack of safety, etc. I like the things Dr. Van Der Kolk recommended, especially for a sense of agency. Thank you!!

Thank you, Doctor! Such sage advice. Structure has been a saving grace for me, so I am glad I found your video today. We are still in lockdown in NYC, and every night, I make a list of everything I need/want to do, even if I know that I plan to do it. I go through my day and cross things off the list. This includes exercise, weight lifting and Qi Gong, brief daily reading on several different topics, spiritual practices, time on the phone, text, or online with friends,. meal planning and cooking, gardening and caring for indoor plants, and one small action to improve my environment. It's not perfect, but it is definitely helping.

This is excellent! Thank you so much - I couldn't understand why somehow I felt thrown back into my childhood - where as an only child with a severely bi-polar mother I never knew what each day would be like + lived w a constant sense of hyper-vigilance + terror. Thank-you Bessel, it all makes so much sense; I'm not small anymore I can give myself that structure. Thank you for articulating so clearly our unseen, hidden needs.!

I've been experiencing exactly the same thing. As lockdown has continued I've increasingly been reminded of traumatic events of my childhood. Structure is certainly what's missing.

Really great information! I live in Austria, we had the lockdown pretty early on, and now we are all getting masks on Wednesday they are gonna be handed out in front of the supermarkets. I'm wondering about schedules when a client struggles with feelings of failure when they can't keep a schedule or accomplish what they often set out to do - that narrative that they will fail? Do you still suggest they try to be so structured?

81 million had trauma lifted on Nov 4th, then cemented on February 20. 74 million then got stressed/traumatized. That net shift of 14 million is a positive shift.

Wonderful straight forward advice from Heart. I am watching this video on Jun 24, 2022.. wish I had watched this much earlier.. I would have saved myself from so much trauma, Covid has caused. Great Video and advice. I will create structure from now.🙏🏻

I love this! Thank you for articulating this. I came up with an acronym, EMBARK. E, exercise. M, mindfulness. B, beautification. A, assignments. R, recreation. K, kindness. I came up with it initially to help my clients with Autism, ADHD, and Intellectual or developmental differences. I need this with this open-ended time we have. My fear is that I am going to dread going back to the busy-ness and distractions when this quarantine lifts! 😅 Please provide another video for that time. Thank you.

Life is ALWAYS unpredictable. Whoever thinks they have control is mistaken. You can control yourself only. You can create healthy boundaries, healthy lifestyle regimens like nutrition and exercise/movement, and spiritual practices that create a sense of agency and self-efficacy.. OR You can always blame someone or something outside yourself, however, don't be fooled, blame is a base emotion that holds you hostage. No one who is blaming the president, the government, the economy, their spouse etc... has the ability and power to transform a negative into a positive. When you give into blame you also simultaneously, give away your power. Period. I expected so much more from this.

I would respectfully suggest that uncontrollability is at the root of trauma and that unpredictability follows from that. Managing to participate efficiently in the situation of what is happening now returns the sense of personal agency. The idea of "managing the situation" supports an illusion of routine of controllability and denies the facticity of the situation which leads to a sense of helplessness anyway. Weightlifting and pushups will not alleviate the distress that a person experiences when their loved one is dying and you cannot visit them - no matter how delicious the meal you make.

I randomly watched a video of this doctor talking about war and how horrible it is that men come back and commit suicide, so I decided to look him up and see what he thought of Donald Trump who has started zero foreign conflicts. I was shocked at his opinion. How did he feel about Obama and his wars or Clinton and Bush and their wars? There seems to be a disconnect here.

If this so-called professional thinks he has the right to call anyone, let alone the President of the United States, "crazy," I'm sure he won't mind if I "diagnose" him with TDS - Trump Derangement Syndrome. The point was about trauma and the fact that lack of trust and predictability is very important to responding to trauma. You don’t have a predictable or trustworthy leader - and this leads to more fear.

I understand how you feel about the latest blog, as Bessel’s statements were out of the norm for our usual content. This was a judgement call that I had to make. I usually decide to exclude all talk of politics, but this time Bessel specifically asked me to keep it, and I wanted to respect his wishes. I was really up in the air on this decision but ultimately decided to allow people to make their own judgements on the matter. I want to thank you for reaching out about this. @@nicabm I really think you made the right decision here. The statement can be interpreted as political but to me there is a whole layer worth considering. Imagine if the person who is being criticized was a parent in a family where the family is in danger and the children as scared and anxious. In a traumatic situation, there are certain things that a parent needs to enact in order to guide their children back towards a sense of safety and trust. This world leader is not a parent in this context, obviously, but he is in a very powerful role as a guide and facilitator. He sets the tone. I see Bessel's words as a commentary on this leader's approach to his role and influence with the people who are looking to this leader for guidance. If this was a parent speaking to his child, one might come to the same conclusion Bessel is expressing.
