That's if they are forced to admit it. A lot of it just gets swept under the rug. Hospitals don't have to do autopsies any more (they used to have to do at least 50% of their deaths) so the doctor can write anything they want as the cause of death.

To be fair, there is usually a reason that you are in medical care. If a doctor fails to cure your disease and you die, you died because of "medical error" but it wasn't the error that killed you, it was whatever you already had. Diagnosing and treating illness isn't as simple and easy as people think it is. There is rarely definite and universal indicators that point exclusively to any specific malady, people lie and miscommunicate, and you don't always just get to say "oh, okay is this, do do that and it will fix it. Problem solved". Different people can require different measures, what works on one person doesn't work on every person. It's really still a lot of guesswork, or going by statistical results in any case.

I went through that nonsense with womanly issues. Found a doctor who agreed to do an exploratory surgery (ultra sounds showed nothing). Sure enough, it all needed to come out. I think it's important to listen to our own bodies, and sometimes you do have to push until you find a doctor who will listen and help.
