I think the most critical bit of information is that if you pick anything else than a monofocal, the lens has to split the light around to different focus points, usually 50/30/20 far, intermediate and near severely reducing contrast sensitivity. Because the amount of photons entering the eye is constant, monofocals let all the light through to your retina. Everything else is a compromise of light splitting so you will lose visual quality, color and brightness.

I picked monofical lens. It still is the "gold standard" for sharpest, highest contrast and great night and dim light vision. If you pick multifocal just to rid yourself of readers you will never realize what you missed out by not picking monofocals.

@@MALU0327 You can easily use your cellphone with monofocal lenses. It's not super sharp but easy to read texts, make calls etc.

Just had both eyes done with Toric lenses and so far, so good! Eyes tested better after 24 hours!! I'm a very happy camper!

As a 71 year old senior with cataracts, I will get the regular lens so I have to buy a new prescription eyeglasses because I look younger wearing eyeglasses than without.

Your comment is something I noticed recently on a video podcast personality. Ordinarily she wears glasses. But in a recent interview, I noticed she looked terrible, much older. She was not wearing her glasses! Then I looked at myself with and without glasses. Guess what! My glasses hide the bags under my eyes! Now you’ve confirmed what I suspected all along. Har my right eye done a week ago. Went with a Toric Vivity Lens. Blown away. Seeing doc tomorrow but thinking 20/20. Using off the shelf readers for near but computer is crystal clear. I think doc is planning a bit of blended vision for the left one. Can't wait. So happy

@@AlexAlex-cr1kd Hey mate. Just had my post op checkup (1 week) today. My vision is better than 20/20. Very happy with my eyes and booked for second eye in a few weeks. My Doc only offered panoptix or monofocal or vivity but with all my research and online videos that would have been my choice anyway. I seem to have no visual artifacts or problems at night with contrast. This was the reason I didn't get panoptix. Doc is planning to make me a little nearsighted in non dominant eye for reading. Very happy with vivity. I also saw a bunch or doctors recommending panoptix but when a patient is then unhappy for whatever reason the replacement seemed to always be a vivity. It seems to me the only reason they recommend panoptix is because it gives near vision as well but I don't mind readers and when my second eye is done I probably won't need them anymore.

Glad to hear that the Vivity was a good fit. I have my consultation with the ophthalmologist in a few weeks for cataract surgery. I’m very nearsighted and have astigmatism and I need to be able to see on my laptop and desktop for work. Leaning towards the Vivity lenses.

@@aprilc.3697 My wife is also looking at getting her cataracts done and leaning towards vivity. But she is also myopic and currently can read without glasses as I suppose you can. You are likely going to lose that superpower hahahaha. Doc says I am better than 20/20 for distance and I can use a computer without glasses. Reading I am using readers at the moment. My second eye is in 2 weeks and he's going for blended vision with vivity again and making my second eye slightly near sighted. He says it won't affect my distance vision and I shouldn't need glasses.

I am myopic but need good near vision as I do a lot of detailed up close work 16-18 inches and I do love my up close superpower but I am tired of having 4 sets of glasses! Thanks

I recently had the third and most expensive cateracts surgery and still am having some irritation in one eye. Feels like I have stitches in one. My dr. said it may thaw up to 90 days to clear up. Has anyone else had this problem? However I can see👏👍👍👍

Informative video. My bozo of an eye doc never mentioned the "accommodative" option, and did a poor job of describing the features, limitations, and competing implementations of the "multifocal" option, but he did try to upsell me on the multifocal implant, which is what I ultimately chose. He didn't do a good job of choosing the correct power, however, so I ended up having to do PRK to get the distance focus approximately correct. Prior to correcting the distance focus I couldn't see well enough to pass a driver's license exam without glasses, and the "halo" was bad enough at night that I couldn't read standard road signs (like speed limits) until I was very close to the sign. He never told me how many focal zones the multifocal lens has -- I gather from doing my own research that it probably has either two or three zones, which means that my current contrast ratio is roughly one-half to one-third what it would be with a monofocal lens (another limitation he didn't tell me). Practically this means that I need more light to see fine detail than someone with healthy natural eyes ... kinda like I still have cataracts. I suspect, based on the diffraction patterns I see around any bright point source, that my implants utilize a Fresnel-type lens, but I've played with some multi-focal type options in a J&J simulator that seem to be based on something other than a Fresnel lens (maybe gradient index). My assessment is that the multifocals are convenient -- I can get by without glasses 95% of the time. But my vision is not crisp and clean at most distances; I still need reading glasses (or just cheap "cheaters") for small type (like medicine containers); and I notice significant diffraction rings around bright lights at night. It's possible that I might like a monofocal (set for distance) in my dominant eye and a multifocal in the other eye, but I wouldn't know for sure without trying it, and they really don't like the idea of removing an IOL and replacing it with something else.

I would love to know if you did the multi focal in one eye and the mono focal in your dominant eye and how that worked for you? That is the plan my surgeon and I came up with and I’m scheduled in 10 days. Thank you!

I did not do that, because the eye doc did not properly describe the options available to me, but ... based on my experience with multifocal lenses in both eyes, I expect I'd like an infinity-focused monofocal in my dominant eye and a multifocal in my non-dominant eye. Still a compromise, but that would give me crisp, high-contrast distance vision and still be able to get by without readers the vast majority of the time. So ... based on my experience, I'd say, good plan. @@HeyYou0602

I got Johnson and Johnson eyhance Toric lenses. My understanding is they are enhanced vision monofocal lenses. I love them. I have only had one done so far but I can drive and read my phone. Watch tv etc.

Thank you for this very interesting, informative lecture . I am a 76 yr old male and just had both eyes done. I chose the "Mono Focal" lens and like everything else, we always wonder if we made the right decision - now, after listening to your talk , I am very happy that I did !. . . It's now 4 months since my implants and it's all good. Mind you, I still have the "dry eyes" problem (excessive unwanted tearing) but am putting up with it, like everyone else !

Well done -- THANKS! This reduces any concerns I have, being very myopic, prior to surgeries coming up quite soon. May I ask what your general experience has been with patients who are very nearsighted (I am now - 9 in dominant eye and - 10.5 in the other, and have worn contacts for a long time) if they opt for monofocal or "monovision". I see very well with single-vision correction now except under about 2 feet (dollar store readers at +1.25 do the trick, like right now while I am typing and watching videos on the desktop). My ONLY BIG CONCERN is being badly out of focus under several feet. Thanks again.

Bifocal right eye, monofocal left eye. Wear readers when doing close work or extensive reading- couple of times a day. Works great.

I'm having surgery next month...I chose the Toric lens. I remember him mentioning I had many floaters in my left eye...will the Toric lens get rid of the floaters...will the floaters still interfere with my vision? Will I be able to drive at night too? I have floaters, astigmatism, double vision in left eye, I had rectina surgery on the left eye, eyes run sometimes, everything else...the muscles etc are good! Is the Toric the best one for me with these conditions?

Hi. Can u answer this question please? My story: Say 10 plus years a go I had my right eye cataract removed. After surgery, 3 days later surgeon removed bandages I was able to see clearly right away but double vision . Yes my left ieye had become lazy. Back than but after it corrected no more lazy eye. Luckily my vision corrected aon its own so both eyes worked very well no glases no glare no short Orr long distance issue. I went back to same surgeon last month for left eye. Yes I haD cataract in left eye even than was recommended to have it removed but I have waited 10 plus years yes my left I was very developed dense cataract. Removed bandaid 1 day latter absolutely no vision as if no surgery was performed I. My left eye. 5 days after surgery I have gained vision gradually has gotten better now 11 th day. However, left eye has the generic lense I chose, whatever my insurance paid. Since my right eye also has a simple lense from 10 years plus Now left eye yes was told by surgeon more astigmatism than right eye R is ..7 and l is 1.7 astigmatism. With normal lense I have 11 since my right eye has a near and far distance is vibrantly but no clarity in Either short or long distance. With both eyes I can see 20/15 to 20/20. But left eye I have only sharpness no details on short or long term. Can u help with this problem I need bifocals on left eye or need to put in multiform lense re cataract surgery?

I had narrow eye glaucoma surgery done about 10 years ago. Since then I see Halos and the rays of light get in my eyes and are very bothersome. I have consulted many Ophtsmologists but they could not figure out the problem. However one Ophtsmologist told me that the hole was made under my eyelid that is why I have this problem. He said he can put a stitch on the hole and close it and remove my cataract. I waited four years I just went back to him for cataract surgery. He gave me two choices. Standard or panoptics. Medicare does not cover panoptics. I also read that you see halos. I want to ask your advice on it.

I am not her, but I would suggest getting one eye set tor far and the other for near as she mentions. I am about to do that myself as it is the best option for those on medicare which only pays for the one type.

My cataract surgery caused eyelid droop in the corner of my right eye and why can't I just have that droopy skin taken off instead of complete eyelid lift and forehead lift as was suggested? Also, now I have dry eye.

I wanted to be able to see without glasses after cataract surgery but still need a contact in one eye to see up close. Also, I ended up with ptosis in right eyelid and numbness on sides of eyes? And why should I have to pay for eyelid lift if caused by the cataract surgery? I have Medicare. I was not given a choice which implant I wanted. I just said I wanted to see without glasses.

I had cataract surgery last year. I was very near sided and hada lot of astigmatism I got the most expensive lense so i could see near,far and in between. After the original surgery, I had to return to clean up the new lense. I still didn’t test well, so I had lasik. I can see. In distance, but need reading glasses. They say nothing more can be done. What to do next?

I got the panoptix and can see 20/20 near and far no glasses for anything. The night drive is not that bad after I adjusted to the new eyes. I am glad I went with panoptix no regrets

@@ekitt7629 I had glasses, but did not use them very much. as my cataract got worse, my right eye got worse at distance but could read with it at close up, My brain had to adjust to seeing distance with left eye. I did not want to have to keep up with readers and you only get one chance with cataract surgery. So I decided to go with the panoptix. I can watch tv at a distance and the picture is so bright and clear, the colors pop and can read the small print on tv. At the same time I can look at my phone and read the smallest print. I am now 74 years old. Got this done about 3 1/2 years ago. I am very happy I did.

IM considering the vivity because of my lifestyle and don't mind readers for close up however in my left eye i have a slight epiretinal pucker been wearing contacts since i was 19 and readers in 40's so im ok with readers for close up but sharp crisp distance is what I would like anyone know if the macular pucker will be an issue .?

Two days out from doing laser with a Multifocal lens. Halos are an issue, but hopefully I will adjust soon for that. Near vision is slowly coming in. Far vision is pretty good. I can tell a light and color difference between my eyes as the other one has a cataract that will be replaced as it gets worse. I find it better to view black text on a white background as opposed to white on a black background. Only two day out so far. Let’s see where it goes in the next month.

Hi Danny, I had multifocals 2 weeks ago and reading vision is still a bit blurry and using 2.5 reading glasses which helps. How are your eyes now?

@@jenjen836 reading printed text is pretty good. Reading on electronics is not as good, as has a bit of a glare or flare. Guessing that is caused by the rings on the lens. I have not gotten any new glasses yet, but the new prescription does have a +2.50 for both eyes.

@Eunice Gonzalez I will not get that for my other eye. I will go with the mono vision lens. Reading black print is ok. But it is not so good on electronic devices. LEDs and other light have rings that may be distracting. Reading text on my tablet that is a light color on a darker gives a blurry effect. You can find RU-vid videos about these things.

It's been 5 and a half weeks since my multifocal lense exchange surgery. My distance vision is good but I still need reading glasses for reading mail, tablet and books etc. My intermediate vision is terrible and it's hard to see the VDU screen at work unless I wear the 2.5 reading glasses, increase the text size on my screen and put the screen very close to my face. I'm extremely disappointed with the result. I have all the cons of multifocals but without the multifocal benefits. My colleagues keep asking why my eye sight is so bed. I couldn't do training off my colleagues VDU last week as I could not see her screen. The Opthalmologist said wait another 6 weeks to see if it improves. If no improvement it's stuck like this

Can cataract surgery itself cause increased astigmatism? I went for monodical, distance, in my left eye. After three weeks my distance vision is blurry/cloudy and oddly enough I can read intermediate. I was now told I have astigmatism, which I had corrected with lasik years ago. I have very slight M.D. I have another 3 week check-up tomorrow. Not real happy at the moment.

I would think carefully about spending the extra for a Toric if you’re going to wear glasses anyway after the surgery. I knew I would be wearing progressives after because I like them and knew I needed a prescription for the near and intermediate field. My Toric did not correct my astigmatism for as long as I’d hoped. In less than 6 months I needed a new prescription because my astigmatism had changed that much. So the $1700 I paid for the Toric in my left eye was a waste of money.

I was a young cataract in one eye, had monofocal toric for near. Worst decision ever, since my other eye is still normal my iol eye gets ignored and is effectively as useful as with my cataract. Even with a multi focal contact it was a huge disappointment.

I had both eyes done in December. I got the mono focal implants for distance in both eyes. I still wear bifocals to correct for astigmatism and for up close vision. My insurance wouldn’t pay for toric implants.

I got monofocal lenses, for maximum sharpness. I chose to get my prime focus at about 2 feet, or 60 cm. This is where I do things with my hands. The depth of field with these is sufficient that I can function quite well without glasses in most situations. I can read and I can drive without glasses. To do anything closer than about a foot, I use reading glasses. My old distance glasses still serve me well, with only -0.5 diopter plus a bit of astigmatism correction. They help to see good detail at a distance at night. This intermediate focal distance is not often discussed, but it should be.

I did exactly the same thing. My old eyes were -5. My new monofocal lenses give me a -2. I can read, cook, and get around fine without glasses. Focus is about arms length. I wear bifocals that are somewhat odd, according to my optician. I had them set the "add" lower than standard by about 2mm. A "little sliver" as she called it. And the "add" is more than normal at +2.75.

I'm going with the monofocal---won't drive at night, so that's not a consideration, but the extreme cost of the "better" lenses is, so I'm going with what my insurance will cover.

If I'm not mistaken, mono focal distance lenses are the best option for night driving. I'm getting my 1st eye done Wednesday and I'm going with mono/distance lens. My biggest problem is light glare and night driving is brutal because of headlight glare and halos. My best understanding is, mono focal is the right choice.. And medicare and most other insurances cover the mono lenses..

@@alvinalexander9868 Hi Alvin, I had my 2nd eye done this past Wednesday. I am very very happy with improvement regarding bright sunlight glare. That was my main issue. The last year+ I really struggled in bright sunlight. I mean I not only couldn't look horizontally in bright sunlight I had to look down. Now it's much much better. Night driving is also much more comfortable and easier now. I have only tested my night driving vision 2 times since I had my 1st eye done and 1 more time since Wednesday with both eyes done and halo's around traffic lights and street lights are still visible but barely and not a problem at all. Oncoming headlights are also much easier to handle and not anywhere near as glaring and uncomfortable as before the new lenses. I'm still adjusting and I expect to use readers for small print reading but other than that my vision is pretty good. In a month I'll get a regular vision test. I have no problem on the computer, phone or watching tv. Distance vision is very good. I don't know if it's as good as when I was 18 (50yrs ago) but I'm very satisfied and happy with what I'm seeing now. I first realized colors are brighter and more vibrant with the new eyes when I had the one new lens and one old. I would close the new eye gently, and look down the street 100-150 yards at houses, cars and flowers etc then switch to the new eye looking at the same things and the difference was remarkable. Bottom line is I'm very happy with the lenses and of course the surgeries that went so well. Total out of pocket was $520 bucks. $200 per eye (Co-Pay)for the surgery center/surgeon. And I purchased 2 bottles of medicine drops at 60 bucks each not covered by insurance. I hope this helps you..

hello Dr D! I am 42 and am monocular! in my good eyeball I had a retinal detachment and I have a buckle. now I have some kind of crystal cataract. I need advice. I know just about everything about all these lenses including the spec sheets that I've studied. no where can I find info on how EDOF lenses or Multi focus lenses affect monocular patients. I am probably going to get the ZCB00 or Eyhance. I'm nervous. any advice? money is not a concern as this is an investment in my well-being. I appreciate you being here and helping us noobs out!

Vivity lense in right eye, I did as much research as I could and I did not want to go with the other premium lense. So far so good, however side effects for me are dizziness and nausea from the differences in my vision in my treated versus untreated eye. I don't regret my decision however, this is the first time I've ever been able to see without my glasses. Right eye is at Plano so I'm thrilled 😄!

As an update I have both eyes done now and have 20/20 vision no problem cooking, reading, working on the computer, or cell phone, or driving at night. I can say that if I'm in bright light it's much easier to see tiny print then in low light, but I'm okay with that it's better than cataracts. It really pays to research!

Vivity contrast at night seeing a projector in the dark is great or halos? I think great. But surfing saw you say at night seeing light with Vivity will see halos? Might wear a 😷 achu 😁

I had a Johnson and Johnson monofocal lens four weeks ago. Everything is violet. Going to see about cloudy vision that has started. I guess that it is because I have Fuch's. The first couple of weeks were awesome. So clear. I wonder if tinted glasses would help with the blueish purple when painting?

I had the Crystalens implanted in 2005. It was outstanding for a few years. But 20 years later I need glasses to drive and I need to remove my glasses to read.

I currently wear monovision contacts. I do have cataracs, but not bad enough yet to have surgery. Would you recomend going mono vision since i am already doing it with contacts?

That's the lens diopter or power. A bigger + number should be for distance. Do you have good distance vision now?

I have right eye operated for cataract at a distance with monofocal and will be operated for the other(left eye) for cataract. Can I choose multifocal for the left eye. What is the pro/con compared to monovision? I play table tennis a lot(2 hours/day) and use computer for work about 6 hours/day. Thanks

I am having cataract surgery in a couple of weeks because my eye isn't really of any use. I am paying alot of money for a toric lens. I guess the worse that can happen is I will need a patch for that eye because I still won't be able to use it after surgery and it might interfere with my good eye and be worse. I may just buy a patch to cover it. I feel I have about a ten percent chance of a good outcome. I hope I am wrong. It is very depressing because if I need glasses they will be a big expense and the prescription will not work to help me to see. I have spent about a thousand dollars on glasses I can't see out of and they blame me. I have always been better off with a five dollar pair of Wal-Mart glasses which I can't understand why I can see with a five dollar pair but not $500 pair. I feel hopeless and why did I ever believe I could be helped?

I’m 32, had cataract surgery on my left eye when I was 16 (they implanted a ReZoom multifocal IOL). Side effects are awful with ghosting, however uncorrected near vision and intermediate are excellent (I wear single vision -2 SPH glasses for distance). My other eye (dominant) is starting to have cataract issues, and I’m thinking of getting a monofocal also set for -2 in that eye. This is a mix/match situation with different IOL types. Do you think I could have a good outcome with this pairing? I need to be able to drive at night and work at a computer and read my iPhone at 19” away

I had RK surgery in 1996. My left eye is bad and I almost see double at times. Right eye is corrected well with glasses. I am concerned as I do have cataracts and not sure how my left eye vision will be with an IOL. Do you have any recommendations? Thanks

This can be a tough situation due to RK. Multifocals don’t typically do well, I’d stick to a monofocal in general.

If you value your close-in vision...i.e. seeing your watch, doing DIY work, seeing instruments or people inside your car, seeing your phone (texts, etc.)? Please DO NOT opt for the Toric Lens. Since October 2023 my Toric Lens from Johnson & Johnson has left me CRIPPLED. Yes...I can read a license plate at 2-blocks, However, this morning I fell down 8 concrete stairs, head-first, because I no longer have depth perception OR balance. I was shamed into paying $1700 extra for the Toric Lens with the medical staff making me feel like a "cheapskate" for not choosing the so- called "premium lens." The worst medical decision of my life.

If you have cataracts you will eventually go blind. With surgery you may go blind or you may not. Surgery is the better option in my opinion.

So this is a very intrusive OP on an extremely sensitive place. I asked the clinic about pain they said "ya just take a few advise you'll be OK". That alone shut me down in my decision, Absolutely no way can an EYE get worked on and the pain isn't severe for the first day or three needing A real pain relief for it, not a useless worthless very weak Advil . So some HONESTY is needed here.

Legit no pain. They use eye numbing drops and a mild IV sedative to keep you calm. You don't really realize they're doing something to your eye. They're done in 5 to 10 minutes. I was in and out in 1.5 to 2 hours total. Most time is spent in pre-op prep with eyedrops, setting up to monitor your vitals and waiting for my turn. I only had some eye irritation the first evening in one eye that caused tearing. It was fine by the next morning. I forgot that they did give me two Acetazolamide (Diamox) 250mg to use if I was experiencing eye ache or pain, but I didn't need to use them. It treats edema or swelling in the eye after surgery. They also said I could use Tylenol or Advil as needed for pain/discomfort.
