Who is the Doctor and What are his Patients? (Little Nightmares 2 Theory) 

Actually, I don’t believe he is wearing a mask. I think his skin has simply begun sagging since he’s been hanging down for so long (explaining why his upper lip is hanging so high above his gums)

The Doctor literally looks like a Little Nightmares interpretation of Homer Simpson

When I saw the doctor HANGING UP from the CEILING, it almost looked like a CATERPILLAR to me and I thought he was gonna turn into a BUTTERFLY

If the signal tower can only fully infected adults, then it made sense why a child is considered a threat. Their body might not be fully developed to be fully infected by the static of the signal tower which would explain why most children we see are way normal than the adults. However, these children are also seen to possessed some supernatural ability like as Six and Mono. This why these children are captured, to avoid them from sabotaging the signal tower. Most abnormal children (like the shadow children and the Teacher's students) seemed to be monstrous because the adults changed them that way to do their bidding. The Lady changing the runaway children into gnomes would avoid them from disturbing her as she knows that children are less to get manipulated than adults. (This is just some speculation anyway)

its intresting that the extra limbs in the hospital resemble that of ball jointed dolls or prosthethics, maybe the doctor was a doll maker at some point? it would explain why he would provide the school with the "students"

Actually I think the Doctor used the light to keep the Patients still while he operates in them. So the same light from the Flashlight has the same effect of the light in the Operating Table

What I like about the antagonists of little nightmares are their motivations, despite their heartless acts, they do everything to quench their inner emptiness, remaining stuck in a vision of addictions and dissatisfaction, always forcing them to want more than they have

Interesting video as always. But I noticed something that all of the patients have in common.They don’t have a real face to show or a head.If they had a head,it’s covered with bandages while others are either replaced with items like teeth or some sort of masks and even lamps(s),its like the features that they once had are taken away from them by the Doctor when they came to the Hospital.The doctor may have used their faces and skin to create those masks since I also noticed most of the residents of the pale city already have their faces distorted in a way that there can be no place for eyes,nose and mouth.The people that may have come here possibly still had "normal" faces to show and the doctor took advantage of it... I’m sorry for typing this long theory,but this is just my thoughts and opinions about the patients' appearance.

Little theory: The Mannequins freezing up in light could have something to do with the All-Seeing Eyes aboard the Maw, which cause all who they see to freeze up.

I was thinking that the mannequin guys are heatseekers, seeing that they know where you are no matter where you are or how headless they are. If that were the case, could it be that the warmth of the light from the flashlight is what stops them?

So to add to your theory about Little Nightmares as a whole. The universe of Little Nightmares is about control. Different types of control. Going off of that makes me think that the doctor is some sort of final boss similar to the masked woman in LN 1. It doesnt make sense for him to be a simple level boss because neither the Janitor nor the cooks had any reasoning for any type of "control". The Masked Woman had total control of the boat through a type of control called greeded fear. She distilled fear into everyone around because of her greed of beauty. To prove this the people eating the food didnt really have masks on. It seems that that is their actual face. The cooks however had masks on to make them look ugly. This makes sense cause the Masked Woman didnt want anyone to have any beauty or be more beautiful than her. Same with the Janitor. The mask in the demo proves that he had a mask on so he didnt have a choice to wear one. The Doctor here in this demo gives off a different type of "control". The control that he does is control through helping. When a person hears that someone is a doctor, they think of a person able to help with health problems. In this case it was help through physical surgery. It made sense that people came to him willingly, especially after a long period of time. A question to ask would be "Why did people keep going to the Doctor after they should have heard about his 'surgeries'?" The answer to this would be. They didnt care. They wanted to be changed, to be turned into a mindless drone. The doctor gave them a purpose by removing their sense of purpose. Thus the Doctor's way of controlling the hospital.

I believe the souls for the dolls come from the maw, and those bottles in seven’s story. The lady removes children’s souls, puts them in bottles, where they are shipped to the city. When a bottle breaks in the maw, it forms a shadow child enemy. I also think those theories about the light are cool, but the signal energy one seems like a bit too much of an assumption, and while the trauma one is cool, the one problem is that mannequins freeze in all light. We see one stop when overhead lights go on, which, unlike the flashlight, do not resemble an operating light

If he makes the prosthetics, how come they move? Someone must control them. Do they move from muscle memory? If so, how would they know that what they need to do - is catch the things that donˋt belong there. Someone must control them, they donˋt move on their own

There’s so many new bosses in LN2!!

My first assumption when thinking about the reasons the mannequins freeze because of light was not only because of the soothing hypnotic power of the TV screens, but I believe the doctor may have utilized the same power when doing his surgeries. Even when in their restraints, we've seen in the demo that they still writhe and are unstable. I'm thinking the doctor could've used the big light on the operating table to keep them still while the surgery was happening to stop them from flailing about. So not only is the light hypnotic to them, but they've all been trained to remain still when the light is shining on them just like in their surgeries. (Not sure how to explain that from a psychological perspective cause well... most of em don't have heads)

Possible idea, what if the Doctor's light was specifically made to freeze his patients, because the passage said they couldn't say anything once the process started. So maybe the first thing he did was make bright lights freeze his patients, so they couldn't fight back

Hey Mike a video showed that there was a secret room in the hallway of the hands that tried to grab Mono. I don't know what the video is called cause it's in another language but it showed the secret room. If you pick up the cheese and go to the back of the hallway and see jail bars and go to the right side and turn on you flashlight you'll see the bars are chop off by a saw on the floor go through the broken bars and make a left turn and you'll find a door and it will show you the secret room and there's a hole in the center of the secret room and if you throw the cheese in the hole it will make a weird sound also I think there's a weird monster in the hole. You won't be able to see the monster and if you go down in the hole you'll die. Anyway that is it I hope you make a theory about the monster in the hole and your theory's are the best!!!!

The janitor doesn’t actually wear a mask but rather his own skin that has peeled off his skull that’s why he is completely blind and focuses fully on his hearing to catch six. In the game that’s been realised there is a man strapped to a table he appears to have no arms and short feet peeking around the curtain which is lit by a light. In the mask room there is a large mask that hangs close to the centre in the first game set in the maw a picture was found of the janitor with his skin on his face this mask looks like the face of the janitor. And the man in the surgical table was struggling for his life desperately. Could the janitor still be alive? And have became a victim to the doctor?

it could be that the doctor made it so that the patience don't move when there is light on them so they cant move when he is operating on them and it said that they "can't complain" and that's because they cant move. so when mono shines the light on them they don't move because they where made so that they don't move when light shines on them. that is my theory!
