I'd like to provide a counter comment to @MOAB-UT, if you have a rotator cuff injury like I do and are facing into surgery and a long rehab an 8 minute video that I can play at 1.5X with CC is not too long. I was literally hanging on every word and the clarity here is incredible, speaking in simple language so I can fully understand and can grasp the issue. Thank you for this video, again...IF ONLY I had seen it immediately after injuring my shoulder what could have been. Well done!
You always have some great advice! I'm five weeks into the healing process and in pain. I'm going through PT & OT for my shoulder and a broken finger on the same arm! Thank you, Dr. Orkin 👏👏
Thank you so much for this information. Very educational. You're the best you make it to where we can understand it easy and you don't use those big words that we don't understand.

6 years after a acl/ tightening of the socket and just had a revision surgery . Will find out on follow up this week the extent my surgeon went thru for repairs but best recovery I’ve had and this is my 3rd right shoulder surgery no pain so far after a double nerve block
Great info. I’m 54 but I’m in good shape. I had surgery last week for a sub acromial decompression, bone spur shave. However, a partial tear was found. It didn’t show up on the MRI prior to the procedure. He did repair it. Is this a common situation? Also, in ur experience, think chances would be pretty good for full healing in ppl in a scenario like that? Thank you again!
Craig , it is pretty common to find a partial tear in surgery that is not definitively seen on MRI. There is a good change for a successful recovery. Good luck Craig. Let us know how you are doing in your recovery
Wow does it hurt to have a rotator cuff tear. Some days pain shots down my bicep I can't wait for surgery .1 yr ago I could do 15 push ups and was a strong. Now I fall to the floor and my arm trembles with I try to do a push up.
Hey Heather, Yes it does. I hope you get to recovering soon. That too is a long process, but at least it is recovery. Please consider joining our email newsletter for more information and insights and we will send you 2 Free PDF fact sheets. One is specifically how to ease the pain after surgery. Send an email to info@mybodyprotector.com just put "yes" in the subject line and we will get you signed up!
I’m with you brother, same loss of strength and quite depressed. However, after complicated surgery, at least 9 mos PT in future before I find out if complete shoulder replacement is needed. I know I have to be strong mentally and be able to push through the painful PT and focus on how great it will be to one day get my shoulder back. Hang in there friend, lots of work and patience.
Thank you so much Dr. Your words and advice are so comforting and inspiring. You are GOD sent especially for guys like myself who just went through RCS this week. GOD BLESS YOU!
At what age would you NOT suggest surgery. Im 70 and have been told I need repairs. But my arthritis all over is so bad I dont think I have the stomach for it. Had hip replavement last year and did by muself..but to be in a sling for 6 weeks thats seems horrible.
Kandace, I don't have an age to suggest not surgery if they are healthy. I may err on a reverse total shoulder if there is a big tear and arthritis. I'm almost 15 weeks post op from large rotator cuff tear repair (4.5cm) and almost certain that I have a retear or it didn't heal. I can't raise arm without pain and this is my dominant arm. I see my surgeon on Thurs to go over what's next. I would assume he wants another MRI. I'm only 39 and still active and mostly healthy, so I definitely want my arm back if possible. Should I ask any specific questions to my surgeon? This was a planned slow recovery and I followed all instructions with PT and home exercises. I hate that I'll probably have to go through this again. I hope it can be fixed the 2nd time around. Thanks for your videos!
So sorry to hear about your pain and weakness that far out from surgery. If it re-torn there are alot of options. Please watch my videos on the options if the rotator cuff doesn't heal/retears. That will provide you with some questions to ask your surgeon. Good luck and let us know how you are doing.
@@rotatorcuffexpert I saw my Dr. today and I'm getting another MRI soon. He's hoping that scar tissue is preventing me from progressing and limiting my ROM and causing pain. He seemed confident that he did all he could do to fix my large tear and is hoping it's not a retear. He said in close to 5000 repairs, mine was the largest he's seen for someone in their 30's. I'm really trying to stay positive.
@@rotatorcuffexpert so had another MRI and it wasn't good news, evidence of retear or just didn't heal like it was supposed to. Some of the tendon actually still looked ok according to Dr. but the tissue must have been weird or too thin to heal properly. Dr. is now recommending superior capsular reconstruction as an option and another option that might use a smaller patch or graft if he gets in there and feels confident that it will heal well this time. Again I'm 39 and would still like as much out of my arm as possible. Being able to do pushups and bowling occasionally would be nice. Do these options from my Dr. sound reasonable for someone my age and still fairly healthy and in good shape?
Hey doc. I had a Rotator Cuff repair, with Biceps Tenodesis (type 2 SLAP) sub pectoral LHBT, superior labral repair (Torn from 2 - 10 o'clock) and a subacromial decompression. It's been 18 months and my shoulder is stable but I have a lot of pain and severely restricted ROM. My surgeon told me he's unsure about the pain but the ROM is because of how tight he had to repair the RC. Do you have any tips or recommendations for increasing my ROM or am I stuck like this now haha. Any advice is much appreciated, love your channel!
I assume that you have had significant physical therapy, if so there can be reasons to manipulate or do a lysis of adhesion surgery. May be time for a second opinion.
@@rotatorcuffexpert thank you so much! I never even thought old scar tissue could be a reason for pain or reduced ROM. I'll have to see my surgeon again.
I myself am in the Frozen stage of frozen shoulder right now, could @beedubss shoulder have become Frozen after the repair? Because my ROM is so restricted and my frozen shoulder had been in the painful stage for 5 months (much better the last 4 weeks, month 6), after MRI (which two orthos have differing opinions on) it's suggested that I have surgery to release my frozen shoulder (Both orthos Rx this) and also to possibly have a biceps tenodesis? I need to definitely ask more questions, I'm trying to put off surgery right now if it's not certain I need it. Thx for your great videos!
@@JennyLu2003 Jennifer, stay away from these guys if you can, they take it very lightly when their surgery fails. Look at this guy's responses to women who're like "hey I'm still in pain, this didn't work" -- he's just shrugging them off with "watch my vid" or "dunno, see yr surgeon". The failure rate on tenodesis is as high as 30% longterm and it'll be a year or so of your life lost to rehab. Frozen shoulder tends to resolve on its own. All their surgical models have men's bodies in mind, you have to dig hard to find methods that take middle-aged women's anatomy as the default. Take good care of the rest of your body, get lots of sleep if you can and stop doing anything extra for your husband and/or kids if you have them. Put all that tlc into your health. And keep in mind that the injuries are chronic for us because we're the ones who do everything. All those little motions defined as nonexistent by men and yet they're what keep everything going.
Wow! Shane that is a long time to go with a torn RCR. Certainly worth seeing a doc about it and seeing what shape it is in now. Good luck and let us know how its going.
Hi Hans! I am not exactly sure what you mean by brushing your shoulder. In general outside stimulation has a very difficult time getting to the deep muscles of the rotator cuff.
@@rotatorcuffexpert thank you for ansering, yes i meaned brushing my skin on the schoulder, outsight stimulation. my surgery was good, less and less pain after the first week fromm the op, , i'm a 53 old "lion"...looking foreward to get fit again. thank you for your excelent videos. many greetings from austria, wish you the besst . hans
Before I precede to the next video in your series, a question comes to mind. Are there possibilities in shoulder surgery that haven't been considered or invented yet? Surely, surgeons have thought about the possibility of an artificial (prosthesis) supraspinatus muscle with a tendon-type attachment that could fastened to the head of the humorous. Such a prosthesis wouldn't be able to contract like a muscle does. But it might provide stability.
So I’m confused, after my retear surgery, after I start therapy should I try to move a lot to prevent stiffness or very little to prevent ware, hmm this is bad 😮
I'm having my 3rd surgery in 5 days and I'm feeling defeated like I'm never going to get better. 1 surgery on left and about to have 2nd on right. After I heal from this second surgery on right, we will have to do the left again. It's been 2½ years into all this. What are my chances of getting better after all these rotator cuff and bicep surgeries?
Oh wow Nikki, that is a lot of surgery on your poor shoulders. Please remember to be kind and patient with yourself in recovery. Certainly you can get better! Please consider joining our private FB group facebook.com/groups/409240847744184/
A big tear can be described differently. The size can be described as partial or full thickness and then if it is retracted or not. As far as the tear length, (how much is torn off the attachment site) it is generally accepted that a large tear is 3-5 cm in length. But again the size is not the only consideration. It is also the quality of the tendon and the amount of retraction that also matters. Good luck, do you have a large tear?
Please reply 😭😭 I am 17 years old , 7 months (18th September) passed to my shoulder tear surgery. After 4 months of my surgery I was doing well. But i got a jerk by a fall on back on 5th month (Feb end) of my surgery .Then i had stiffness, pain etc. Then i stopped physiotherapy for few weeks then I again started my physiotherapy, my physiotherapy power and range improved but , when I stand straight without sling I have pain and stiffness . When will my stiffness go?? Is my surgery damaged ?? 😭😭😭 I want to go back school happily again 😭😭😭 Please reply.
Shoulder surgery in a teenager is much different than an adult. I would definitely return to your surgeon so they can evaluate your shoulder. Hopefully it is nothing, but it is best to get it checked out.
Tammy I am so sorry that you are not doing better. Check out my video about rotator cuff tear and numbness. Sounds like you need to see your surgeon again for a reeval.
I dont know what is best ,it iß Feb 19 i had RCS on Dec 27 ive been to therapy 1x a wk so 6 trips went to Dr today and hes saying about he thinks its going frozen and put me on Prednisone for 12 days and said unless i was fighting him ,he acts like im lieing how bad it hurts and he said for me to raise in air and i cant i said therapist just started having me do exercises and he saying about people going back to work at this stage i said i couldnt do a computer he said well they do ,my job is lifting 50-70 lb pushing carts several hundred pounds and picking up containers weighing allot and awkward,i went to my car upset and about crying i cant go to work next month doing my job ,end of March would be 3 months he says so you want me to put in a month extension ,i cant afford not to go back to work and if i have to go back like this its all wasted and my right shoulder isnt right from last year and the 2 biggest ortho places just joined and isnt another place to go ,im so down
It all depends on the severity of the tear. Big tears with poor quality tendons have a harder time healing and do not heal more often then smaller tears. From a surgeons perspective it happens way more than we want!
It is hard to say, in general the least powerful the better before surgery because you may need the "big guns" after surgery. It is best to discuss this with your surgeon so you can be on the same page.
Do you by any chance now about a medical research center with the most advanced cartilage repair in the world . They can use me for research. Meaning a alternative to Joint replacements and also rotator cuff repair with the most advanced treatment without screws. I can't even work anymore. I have just too many injuries.I am only 42 and need a sponsor and the best treatment. I can't live with my pain in my pain anymore!!!!!
i had a rotator cuff last march been to phisio until yesterday once a week plus my excersises at home when my surgeon saw the physio report he said i cant do anything for you and walked out what are my options
High Shauna, I subscribed to your Channel! Thanks for sharing. Obviously it depends, but I tell my patients that I anticipate them using pain meds for around 3 months. As time goes by they will use them less and less. Usually just for sleep or before/after PT. Good luck!

Tears a a bit difficult to quantify because some are full thickness and some are partial thickness. In general a tear of less than 1 cm is considered small. Is what you have? Just because its small doesn't mean it shouldn't be fixed in the right setting.
hey doc, I had rotator cuff repair last feb 2021, supraspinatus full thickness tear. I stopped my theraphy after 5 months. i’m now at my 8 month postop, active ROM is still 90 degrees but passive ROM is at least 150 degrees. there is pain and stiffness when i try to move my arm more than 90 degrees. i also feel crackling sounds when i move my arm. i dont know what happened to me, so disappointed. my surgeon said, there are adhesions needed to be removed by lacer, another arrhroscopic procedure. is that correct or there is something wrong with my surgery? before i had my surgery, my ROM is much higher than today. the best part is pain now is minimal unlike before. please tell me what to do, thank you so much.
I'm glad your pain is better. Having good passive ROM but poor active ROM there may be concern for a retear of the rotator cuff. Did you have a repeat MRI? Talk to your surgeon so you can make an educated decision on how to proceed. Good Luck!
Surgery is a poor choice for many people. It's unfortunate that stem cells are so expensive and that hgh and anabolics are illegal. I have torm my rotator cuff a few times, i was a bodybuilder for decades and it a common issue in guys, especially competitive athletes that dry out and have more opportunity due to this and constantly lifting weights. First stop training, working or whatever your doing aggravating it. Yes it sucks, yes you lose money, but if you don't stop it can get far worse. Hopefully you dont have a complete tear and with enough time, rest, water, vitamins and minerals to repair and self heal injuries, there are ways to get ahold of deca and hgh, it isnt hard. I have had doctors tell me i needed surgery more times then i can honestly tell you. Rotaters , knees, and my C1-C7 discs had a compression fracture. Ive seof healed them all, ive had mris after time and made sure they were. I woukd absolutely get surgery, but only if everythung else fails. I dint have complete faith in the medical practices in America. It bkows my mind a basketball player can blow a knee and be back in a few weeks, but they cant do that for us. In Europe, they dont want to do surgery , first option is always treating the injury , getting stem cells and hgh. Success rates are 10 times higher for full recovery without surgery , once tgey go in, your never the same again, unless your a sports player, then they can get your ass back on court in a few weeks. I am not mad at the doctors, they do what they are trained to do and use whats available to them. Obviously he cant tell you go get anabolics or suggest stem cells if he diesnt provide them, which he probably doesnt , its a highly specialized and expensive treatment, especially in America. Mexico has them though, and you can get great hgh and anabolics online of you look for them.
BTDT, thanks again for your comments. And I agree that surgery should be the last option. I hope as time goes on we can have a better understanding on how to use various products including stem cells to heal and avoid surgery.
