Definitely do a video on the problems with training at the moment! You have a really clear way of explaining things and the more people that understand what is going on the better. The current situation with training bottlenecks is a travesty. You simultaneously have the Royal College of Radiologists saying that "the UK now has a 29% shortfall of clinical radiologists, which will rise to 40% in five years without action", meanwhile the competition ratio for Radiology Traning is 11:1 . Its awful.

Thanks for discussing this. There are many different options but they’re not really explored that often. I’m a medical student myself and have thought about this topic myself. It’s great to see, and remind yourself that it’s okay to prioritise different things in life, and maybe come back when you are ready.

Hi Kenji, thanks for the video. Definitely agree with you. I'm a final year med student and I am also already planning to do an F3- potentially becoming a CTF and locuming on the side. I want to do surgery so I know its difficult to get into CST straight from F2... and the competition ratios arent looking great

Good luck with the time off. I’m a first year student nurse, aged 32, after switching careers. I start my first placement tomorrow and I already have a reason for why I want to become a nurse and which pathways are the ones I’d like to look into, those being either an A&E nurse, cardiac specialist or genetic screening. I wish student nurses were paid because yeah, I understand the financial aspects. That’s why I work on the staff bank as a HCA to support myself a bit better.

Kenji awesome video. Really think you are much wiser than I was at f2 stage. I think being a graduate, although expensive, gives you a lot of life experience that puts you in a good position to be unapologetic about putting yourself first. Really like your points about the timing not being great too. Hopefully by the time you apply they will have a better contract. What specialties are you thinking about? Well done bro - nearly done with f2🎉

Hey Kenji, really enjoyed this video as always. Im in my penultimate year of grad med and looking to take a similar route to you to build up some finances in F3. I've heard that it's becoming increasingly harder to get locum jobs now? Any truth in this?

Please have a look at the job market right now for FY3s, it is not the same as it was when I was an FY3 (good 5 years ago). Have a look at JCF roles being taken up left right and centre, I would hedge your bets to either save a tonne now as an FY2 if you want to chance it with locumming or start applying for jobs now. I would have a look at Australia as well in all honesty (not as much admin burden costs as a fresh PGY unlike me as a GP who would be looking at around minimum 10k in costs to transfer my qualifications). I am so sorry that the job market is so shit for you right now, FY3 (and 4) were some of my best years and allowed me time to be me. I am just looking out for you, and the realities of the job market right now. All the best xxx

Mate you really need to do a video on the issues surrounding training. I don't think its at all controversial. Radiology had a 10:1 ratio, however we don't have enough radiologists to get scans reported. There is a short fall of 10000 GPs, yet they have reduced the places for GPT, an now the competition for this is 5:1. This is in addition to the issues of the allocation which means you may get moved to a completely different area at every bottleneck, and the lack of pay protection (your earnings can go down for example if you swap from a ED FY2 in London to a psych CT1 in a rural area!). The whole system is a scam, and we NEED to talk about it.

Kanji I wanted to ask I’m thinking or going abroad to do med I did a level in maths further maths and physics and computer science and got AAAA but I really regret my decision and want to do med because of my bad a level decisions I’ll either have to do a foundation year to bio med than after that degree go into med and it’s not even guaranteed. It’s so disappointing because I had the ability to get into med but made wrong decisions my q is if I get a degree from abroad like Bulgaria and come back to the nhs as an fy2 will I get looked down upon-on and will how will it work if I want to specialise will I not be allowed to specialise because my degree is looked down upon. I also like dermatology but because its super competitive will it be impossible because my degree will be looked as worse degree ?

this is something i want to do but here in singapore we are bonded to the government for 6 years, meaning we can't take this period of "sabbatical" to locum and buildup more financial reserves before starting on specialty training

Absolutely, but as a locum. This is during the foundation stage or core training phase depending on you choice, you can locum at either level

50+ / hour consistently isn't realistic for big cities in my experience. Tends to be the less popular areas that are more desperate. Very very unlikely to see that kind of money in London and South East

Yes London is usually capped at much lower rates. A lot of doctors including myself choose to locum outside of London. There are good rates (£45-55/hour) in SE England in my experience
